FATALLY INJURED3EMPLOYEE WAS EITHER IN A ROTARY BREAKER AND THE BREAKER STARTED UP THROUGH A SHORT IN THE POWER OR HEWAS CLEARING THE ENTRANCE CHUTE TO THE ROTARY BREAKER FROM THE FEED BELTS A ND FELL INTO TH9 BREAKER WHILE IT WAS IN MOTION.
