Hurt his back using a pry bar on a tire. (He did not report it to anybody, we thought he had quit until he showed up to work later)
He was moving dust line that was covered with coal slouphage, rib rolled and hit him in the left hand.
Was pulling a pump out of the mud, and hurt his shoulder, (He did not report to his foreman, or have any witnesses) May not have been hurt. He was given light duty at doctors office, and did not come to work.
Workers tramming feeder on tailpiece when feeder jumped to the right pinning employee against the rib.
A truck got stuck. The loader backed up to the truck to pull it out. The loader operator and the truck operator both got out to hook the chain up. The loader impacted the loader operator in the back, he moved out of the way.

He was tightening a pipe, the pipe wrench slipped and hit him in the ribs.
Continuous miner was tramming and turned into a crosscut without realizing roof bolter operator was there and pinned him between miner tail and roof bolter.
EE was pulling cable down & he fell & twisted his knee.
EE was bolting. He bent a steel and it whipped out of the drill head. His abdomen was hit on the left side.
Tripped and fell on the catwalk. He cut open knee and had stitches.
EE was lifting a bucket of oil out of the oil trailer and hurt his back.
Employee was lifting cable and hurt his shoulder muscle.
EE WAS LIFTING A BREAKER BOX INTO PLACE WHEN IT SLIPPED AND SMASHED HIS THUMB LENGTHWISE.
Upon entering the secondary escapeway (return entry) in the 6th right section, smoke and carbon monoxide were encountered. It was determined that a thermal event had occurred in the #2 panel gob area.
EE WAS PICKING ROCK INSIDE TIPPLE SHACK. HE SLIPPED AND FELL TO FLOOR. HE TRIED TO CATCH HIMSELF AND HE LANDED ON ELBOW.
He was lifting a gear box and had sharp pain in lower abdomen.
EE WAS WATCHING A CABLE SO THE COAL HAULER WOULDN'T HIT THE CABLE. THE COAL HAULER HIT HIM.
He was changing battery on a tractor and slipped off and bumped his tailbone.
Car was leaving the miner, car driver thought he was past both the miner op. and his helper but the tailgate of the car caught his left leg and smashed it against the rib.
A ROOF FALL OCCURRED INTHE HIAWATHA SEAM AT CROSS CUT NO.16 IN THE NO.3 ENTRY. THE FALL IS ABOUT5'HIGH, 18'WIDE, & 10' LONG
THE ROOF FELL IN THE #2 ENTRY BELT ENTRY #15 CROSS CUT IN THE HIAWATHA SEAM. THE FALL IS 4 TO 8' HIGH AND 20' WIDE AND 20' LONG.
EE WAS STANDING BY FEEDER TO GET OUT OF WAY OF SCOOP. THE SCOOP HIT A RIB OF COAL. THE COAL CAME DOWN TRAPPING EE BETWEEN COAL AND FEEDER. EE HAS ABRASIONS AND BROKEN RIBS.
WAS WALKING IN TO CALIBRATE METHAN MONITOR ON 14 CM 15 MINER. SLIPPED ON ICE AND FELL BACK, TRIED TO CATCH HIMSELF AND PULLED LIGAMENTS IN RIGHT ARM AND SHOULDER.
ROOF FALL IN 1ST LEFT ENTRY AT #4 AND #5 X-CUT APPROX 130' X 20'HIGH. A COAL HAULER, A D-BOX AND A SHOP TRAILER WERE LOST IN THE FALL.
PULLING SPRING MOUNT BOLTS OFF WITH AIR WRENCH, SPRING SPRUNG DOWN ON AIR WRENCH AND SMASHED FINGERS ON HOIST.
THE TIPPLE CRUSHER, PLUGGED UP, AFTER IT WAS CLEANED OUT, THEY CLOSED THE DOOR AND EE FINGRS GOT SMASHED IN THE DOOR.

SOMETIME PRIOR TO GRAVEYARD SHIFT ON 5-26-02 A ROOF FALL OCCURRED AT THE INTERSECTION OF THE #3 ROOM AND #2 CROSS CUT. THE ROOF FELL OUT APPROX. 1' ABOVE THE 4' ROOF BOLTS BEING USED.
TRANSPORTING EQUIPMENT WITH FORKLIFT ON HILL, FORKLIFT ROLLED OVER ONTO LEFT HAND PINNING IT TO THE ROAD SURFACE
HOISTING A PLATE UP TO THE CAT WALK. HIS HAND GOT SMASHED BETWEEN THE BASKET AND A PIPE, SMASHING HIS 1ST & 2ND LEFT FINGERS.
MAN WAS PULLING ROCK FROM FEEDER. OTHER EE DIDN'T KNOW HE WAS IN THERE & TURNED THE FEEDER ON. IT PULLED HIM INTO THE CLAWS, BREAKING HIS BONE & CUTTING HIS LEG & FOOT. (LEFT LEG).
ROOF FALL BETWEEN #1 AND #2 ENTRY IN XCUT #11-HIAWATHAT SEAM EAST MAINS INACTIVE- A 3" HOLE WAS DRILLED DOWN FROM THE BLIND CANYON SEAM TO DRAIN WATER, ROOF BECAME WET AND FELL, APPRX. 18' X 3 0' X 4' HIGH.
WAS HAVING STOMACHE PAIN. WENT INTO DOCTOR. DIAGNOSED WITH HERNIA. EE ASSUMES IT HAPPENED AT WORK WHILE HE WAS LIFTING AND MOVING SUPPLIES.
WAS CARRYING 2 BUCKETS OF SEALANT, STEPPED IN A HOLE OR ON A PIECE OF COAL AND STUMBLED AND HURT HIS BACK. (STRAINED)
SLIDING A 3' X 1' PIECE OF I-BEAM ACROSS A LARGER PIECE OF I-BEAM. IT TWISTED THE PLACE HE WAS HOLDING INTO THE SIDE OF THE LOWER I-BEAM AND SMASHED HIS TWO MIDDLE FINGERS ON HIS LEFT HAND.
WAS WATCHING MINER CABLE. A SMALL ROCK FELL FROM TOP, LANDED ON HIS FOOT AND BROKE FOOT IN 2 PLACES.
CLEANING FACES WITH SCOOP. A RIB OF COAL FELL AND SMASHED RIGHT INDEX FINGER AGAINST SCOOP.
RIB BLEW OUT AND HIT JOSE, BREAKING LEFT FOREARM, BRUISING HIS LEFT FOOT AND LEG.
WAS HELPING UNPLUG THE CRUSHER THEY WERE LETTINGTHE SIDE COVER SWING SHUT, IT CAUGHT HIS GLOVE AND TOOK HIS HAND WITH IT, SMASHING HIS THUMB
EE WAS SWINGING A HAMMER TO PULL THE GEAR OFF FROM THE FEEDER HEAD SHAFT. A PIECE OF METAL CAME OFF AND LODGED INTO HIS NECK.
FIXING BELT. EE WAS CLIMBING ACROSS BELT TO OTHER SIDE.ANOTHER EE TURNED BELT ON AND INJ EE HIT INTO CROSS OVER & BENT OVER BACKWARDS. HE WAS BRUISED UP & POSSIBLY HAS A BROKEN RIB
WAS ROOF BOLTING. THE ROOF WRENCH SLIPPED OFF THE ROOF BOLT AND HIT HIS HAND AND BROKE IT.
WAS GUIDING A GEAR BOX DOWN THE TIPPLE HILL. THE GEAR BOX GOT LODGED ON SOME ROCKS. WHEN THE GEAR BOX WAS UNLODGED, IT CAME DOWN ON HIS RIGHT FOOT AND SMASHED IT. IT IS BRUISED UP AND POSSIBLE FRACTURED.
ROOF FALL OCCURRED ONE XCUT OUTBY, #3 BLEEDER MEASUREMENT POINT IN THE NORTH BLEEDER ENTRY. THE FALL WAS CONDINED TO THE XCUT AND DID NOT AFFECT THE AIR OR TRAVELWAY.
WAS WATCHING MINER CABLE. HAD HAND ON SHUTTLE CAR WHILE IT WAS BEING LOADED. THE TAIL OF THE MINER SWUNG AROUND AND SMASHED FINGERS INTO SHUTTLE CAR. TIPS OF THE MIDDLE AND RING FINGERS WERE AMPUTATED.
