Caught in, under or between a moving and a stationary object

A truck got stuck. The loader backed up to the truck to pull it out. The loader operator and the truck operator both got out to hook the chain up. The loader rolled back and crushed the truck operator between the truck and loader. THIS PERSON IS NOT AN EMPLOYEE OF OURS, WE DO NOT HAVE HIS INFORMATION. The loader impacted the loader operator in the back, he moved out of the way.

Accident type, without injuries

A severe rainstorm occurred at the head of the canyon where the mine is located resulting in a flash flood on the road leading to the mine. The miners traveling the road were involved in the flood.

walking out of conex, step out onto the ground and knee popped. Walking around the forklift on level ground and knee popped and gave out.

Over-exertion in pulling or pushing objects

While replacing conveyor belt fabric employee was pushing on pull rope with their back against railing, strained lower back. Employee worked at regular job until 2/15/2021. Doctor recommended to cease work activities until an MRI was conducted.

Walking down road, slipped on snow, fell and hurt right shoulder.

Helped lift a drill box out of a pickup truck bed. Later in the day neck and shoulder started hurting.

Struck against a moving object

Dozer operator was pushing coal on the raw coal stockpile. As EE backed up the Dozer fell into a void that had bridged over. The operator continued working EE's regular shift performing regular duties until 12/8/2017, when EE was examined by a doctor and was put on restricted duty.

Over-exertion in pulling or pushing objects

On 2/6/2017 employee was making repairs on a diesel man trip. Pushed up on a ratchet and felt left shoulder pop. Worked on full duty until 4/24/2017, when EE was scheduled for surgery.

Closing door on Man Trip, got two middle fingers caught in door causing lacerations to both fingers.

He was hitting a chisel with a hammer, missed the chisel and hit his thumb. Fractured his left thumb.

Pushing material with a ""Skid Steer"" loader. Hit something solid, the loader came to an abrupt stop and the employee hit his head on the windshield.

EE was removing a bearing race from a spindle with a hammer and chisel, a piece of race broke off and went into his left bicep.
