Was putting cable behind anchor. The shuttle car lost power. Went and put power back in, and the stop, start switch was stuck and the cable started taking up and hit employee on the back of the legs and flipped employee. Employee landed on left wrist and head bounced off the bottom of the mine floor.

Employee was last observed as employee began to ascend an extension ladder. There is no eye witness, it is apparent the employee fell approximately 20ft. onto a moving conveyor belt and was fatally injured.
The medical examiner: (Preliminary report) Reported the victim died of a pulmonary embolism, natural cause. According to co-workers, the victim complained feeling ill throughout the shift. Became unconscious with a faint pulse and respiration. EMS was summoned. Efforts to revive were unsuccessful.
Area inby fan house had 2 mud seams come together with freezing and thawing weather. Area 3 ft. x 9 ft. x4.5 ft. thick raveled out and fell. Did not block air way or travel ways.
Was walking to scoop and felt pain in lower back
Allegedly the hydraulics locked up on the machine causing it to raise off the ground and drop.
Employee was pulling on a hose and allegedly sat on or strained testicle
Working under a Cat 980 loader crawled out from under machine and right knee popped
While edging up coal pit the loader bucket allegedly dropped off the coal and hit the pit floor
EE was installing a thrown track on a D10R when a job mechanic struck metal on metal with a sledge hammer resulting in a metal shard puncturing EE's chest.
Employee was cleaning mirrors on truck and allegedly fell.
Operator was cleaning top of Powelton Coal when allegedly was stopped by a rock and was jarred.
Allegedly when backing under loader a rock was dumped on the tail shoot and jarred the driver.
Tech was loading sheet metal and hood assemble into truck. Gust of wind pushed hood and hit tech in chest.
Employee alleges that the boom of the loader stuck in the up position and when EE went forward the boom dropped causing the rear of loader to raise and upon the rear setting back down was jarred. Employee did not report the incident to management till 8/1/2016 and did not miss work until 8/11/16 due to the alleged injury.
Removing hose from AC on 992G Wheel Loader. Pag Oil and Freon shot from the fitting spraying into employee's eye. Given Rx eye drops and written Out of work for 1 day.
Cutting left hand equalizer bar end pin out of roller frame and slag popped off went into right ear causing a thermal burn.
The articulating truck blew a steering hose near the turbo causing a fire. The operator attempted to extinguish the fire but had to call the fire department. The rock truck cab was a total loss but no one was injured.
EE left work on 02/02/2016 without reporting an accident. EE reported to the hospital the following afternoon and was treated and released for a back sprain. Dr excuse said to return to work the following day 02/04/2016. EE has not yet returned to work. EE did call on 02/15/2016 and said EE would return next day. Has not returned to work as of 02/21/2016.
EE had just finished fueling the grader, as EE was dismounting the grader EE's hand slipped and EE tried to grab back a hold when EE's glove got caught on the corner of a guard, cutting EE's right hand.
Was jarred when the truck was being loaded.
A second EE backed his hauler into the hauler injured EE was sitting in waiting to get loaded. Proper safety procedures were not used in this incident. Both men said they were OK & did not need medical attention. On 12-15-15 injured EE came into work and said his back was hurting and wanted to get checked out. EE went to ER & was told to follow up with his Dr.
EE arrived on the job at approx. 5"50 am. EE told the foreman his leg was hurting & he could not run the dozer. The foreman told ee to go to the parking lot & when he got the job going he would be back. When the foreman returned he found ee unconscious. He started CPR & continued it until the rescues squad arrived. EE was pronounced dead at Hospital
Employee's work shift ended at 3:00 pm. At approx. 4:40 pm while still on mine property picking up plastic pipe for his personal use, he suffered a massive heart attack, per Medical Examiner at Wellmont Lonesome Pine Hospital in Big Stone, VA. The Security Guard witnessed him having chest pains and going unconscious. Will send copy of death cert. when available.
Torqueing mount bolts for 992G wheel loader unit and ratchet gear stripped causing employee to fall backwards straining lower back
Employee stated he was in the porta trailer. He was attempting to exit the trailer when he slipped and fell. It was raining, his boots were wet & muddy. Lost his balance.
The hauler was on a slick incline and could not pull up the hill the hauler driver requested that the dozer give him assistance, the hauler tires were spinning and when the tires got traction, the hauler jerked, causing it to jerk the operator, hurting his back.
Employee was doing maintenance on a dozer, when he slipped & twisted his ankle - ankle sprain.
Employee was using a 980H loader to bust up coal, when he said he felt a sharp pain in his neck. (Back of neck).
Employee was operating a CAT-777D Hauler. He stated the bed of the truck came down & hit the frame of the truck hard, causing it to shake him.
Employee was walking down a flight of steps, when he stated he slipped and fell, falling backwards, landing on his back and R elbow. Employee is working light duty since (11-28-2014) (Missed 5 days work)
Employee was working on a service truck while laying under it the truck rolled back and ran onto the employees arm and chest.

EE was lifting cable on corner to hang miner cable one more time when he felt pain in groin.
EMPLOYEE SLIPPED ON ICE IN THE PARKING LOT AND FELL.
Employee was closing open doors on train cars, prying them shut with a bar, when bar slipped out of door, releasing pressure on bar causing employee to stumble, causing pain to shoulder, back, & neck area - On 1-6-14 EE was treated in the ER with a return to work with no limitations. On 2-7-14 EE was seen by a specialist. On 2-10-14 EE would be off work for 3 weeks.
Repair man changing boom swing jack on continuous miner. Trying to loosen hose fitting when wrench slipped and cut left hand. Went to doctor had four stitches. No time lost due to injury.
Employee was pulling high voltage cable out of cable car when he said Oh I hurt my shoulder to witness. Kept on working. EE said he told foreman, foreman said he didn't. I was told on 12-19-13. No one can tell me what date injury happened.
EE was bolting on 002 section in 6 left heading when a piece of draw rock fell striking his canopy then fell from canopy striking EE on top right forearm. The injury required 9 stitches.
