EMPLOYEE SAID THAT A LOOSE PIECE OF DRAW ROCK, MEASURING 24" X 17" BY 1/2" TO 2", STRUCK THEM IN THE BACK OF HEAD AND NECK AREA.
Close Clearance and not keeping all body parts inside of mantrip. Employee crushed EE's middle and pointer fingers of EE's right hand when EE grabbed the canopy post to reposition self in the mantrip. The mantrip was traveling around a tight corner where clearance is close and the employees right hand was in a pinch point between the canopy post and the coal/rock rib.
Employee operating shuttle car attempted to get off of shuttle car and was shocked.
Employee was walking the #5 Escapeway entry @ Bk. #14 on Elk Lick Mains when employee twisted left knee. The employee could not bear weight on left knee.
EMPLOYEE WAS WALKING ON THE OFF-SIDE OF THE BELT ENTRY AND FELL HURTING THEIR RIGHT KNEE.
While changing the cutter drums on a continuous mining machine, the employee got hand caught between the impact gun and a crib block bruising the left index finger.
Employee complained that EE was feeling stiff and having lower back pain. Employee said EE was unsure what happened exactly that caused the Pain.
A non-injury roof fall occurred in the #4 on the Southwest Mains at survey station #976. The fall measured approximately 20 feet wide by 8 feet high by 50 feet in length. It is not affecting or impairing ventilation. It is located in a return air course. We do not intend on cleaning the fall up. We are dangering it off. No person(s) or equipment was involved in the fall.
A non-injury roof fall occurred in the #3 entry on the Southwest Mains at survey station #994. The fall measured approximately 20 feet wide by 8 feet high by 80 feet in length. It is not affecting or impairing ventilation. It is located in a return air course. We do not intend on cleaning the fall up. We are dangering it off. No person(s) or equipment was involved in the fall.
A non-injury roof fall occurred on the #3 Belt Conveyor on Northwest Mains at survey station #546. The fall measured approximately 20 feet wide by 5 feet thick by 50 feet in length. A roof fall clean up plan has been submitted to MSHA and has been approved for clean up of the fall. This fall was immediately reported to MSHA. No person(s) were involved in the fall.
Roof Fall--70' in by the survey station #1042 in the cross cut between #4 entry and #3 entry on the 2nd Northwest Mains 3,400 feet from our section. This is an area where we pulled back from. It measured 6' thick by 16' wide by 35' in length. It is not affecting ventilation. We will not be cleaning the fall up, we are dangering it off. No employees or equipment was involved.
The accident took place on the haul road at #1 Pond area. EE came to the mine to pick up paycheck. EE left to go home traveling on the haul road. The location where the accident occurred was at the #1 Pond area, EE hit a coal truck head on in a curve driving EE's personal vehicle. EE was treated by Elite ambulance technicians and transported to ARH hospital in South Williamson.
The fall was located at the Northwest Mains 70' inby survey station 777 in the #6 intake entry. 30' wide by 30' long by 4' thick. Dangered off not cleaning up. No persons or equipment involved.
The location the fall took place was #1 entry 20 feet inby survey station 1021 of the Southwest Mains. The fall was 19' wide by 30' long by 8' thick. Dangered off not cleaning up. No persons or equipment involved.
Fall was outby survey station in the #2 neutral entry on Southwest Mains. It fell in an intersection and measured 30' long by 30' wide by 8' in thickness. It was located approximately 1800' outby the 010 mmu. No persons or equipment were affected by the fall. Set at least 2 rows of timbers on each side of the fall. Do not intend on cleaning the fall up. No ventilation was impaired.
The fall occurred in the #1 return entry of Southwest Mains 70 ft inby survey station 970. The fall was in an intersection and measured 20 ft wide by 20 ft long by 8 ft thick. The fall did not impair ventilation and we do not intend to clean up the fall. It was blocked off by setting 8 breaker timbers in all entrances. No persons or equipment involved. Roof fall only.
The fall occurred at the crosscut entry between #5 and #6 intake entry 140ft inby survey station #983 located on the Southwest Mains. The fall measured approximately 30ft long by 18 to 19ft wide by 7 to 8ft thick. The fall did not impair ventilation or obstruct the primary escape way. No persons or equipment were involved. The area was blocked off and isn't intended to be cleared.
Completed project, carrying survey equipment slipped and fell
A fall measuring 20' wide X 25' long X 4.5' thick occurred at 140' inby survey station 925 in the #7 right return entry on the Northwest Mains. The fall will not be cleaned up. We are re-routing the return around the fall. No persons of equipment were effected. All entrances to the fall will be barricaded with timbers or cribbed off
A piece of draw slate approximately (3 foot by 2 foot by 3 inches thick) broke across the strap and fell striking ee. Injured miner was transported to a medical center. Due to medical complications from the injuries ee sustained ee died on 4/6/17
While walking into the guard shack, the door was flung open, hitting the employee in the left arm, causing employee to fall back, jarring the shoulder and neck.
Employee was operating a shuttle car. He ran over a bump in the road and it threw him out of the seat and up into the canopy and back down into the floor of the shuttle car.
Location: Stacker belt outside. Trimming a splice, changing out a splice on belt. He cut his left hand on the pinky finger. Required 8 stitches.
EE was tramming the miner from #1 entry to #5 entry, he stopped to move the miner cable and the water line out of the road. When he picked it up to move it out of the road so the miner and buggy could get by, he felt something in his chest. His chest was hurting and he left the entry and went outside.
Employee was stepping down (approximately 2ft) off of the #2 Belt Head, he twisted his left ankle and leg.
Employee and a co-worker were putting guards on the miner. They went to reposition the guard and it slid and caught employee's third finger on his left hand smashing finger.
The injured states he was repairing a hole in a stopping when a block fell from above catching his hand against another block.
Employee was standing beside of the roof bolter, bolting roof. The roof bolter was placed in reverse, employee stepped and stumbled over a small pile of coal and fell to his knees jarring his lower back.
Employee was moving a rock out of the roadway so his Shuttle car could pass
Employee was hanging fly pads between #5 and #6 and a piece of top rock fell out from the rib bolt to the rib about 7 or 9 foot long and about 3 foot wide and about 3 or 4 inches thick braking the fly board and hitting employee on his left side in the head, neck, back and shoulder areas knocking him to the ground.
A roof fall measuring approximately 20' wide by 20' long by 6' thick was discovered 60' inby spad 403 in the #2 entry of west - sub mains. The fall will not be cleaned up.
Location/Section-Mains, #6 entry at the face. EE bent over to remove a rock off of the scoop charging cable. He injured his lower back.
The mine had a roof fall at survey station #10 in an intersection. The fall measured 30' L. X 7' Thick X 12' W. An addendum to the approved roof control plan will be submitted to MSHA to rehab the fall area.
EE said he strained his back when lifting a shive wheel for a belt take up unit.
EE said mantrip hit rock or hole and jolted him causing lower pain.
We had a roof fall measuring 4 to 5 feet thick X 19 to 20 feet wide X 70' in distance. This fall is a continuance of a fall reported on 1/5/12 on the 2nd North West Mains @ Spad #1341 in the #4 and #5 entries. This fall does not impede ventilation or travel-The fall will not be cleaned up-There were no injuries.
Employee was working on a belt move on the section when his accident happened. He was placing Belt hanger with bolt when the wrench slipped and hit his hand.
In #e Entry, miner cable caught his legs and threw him against the rib. Right knee is swollen. Left knee was twisted back hurting. He was transferred to hospital by ambulance.
A roof fall in old works in No 7 Entry 70 ft inby spad No 1130 approx 10 ft wide, 10 ft long, about 5 ft high. the fall will not be cleaned up.
A roof fall occurred in the old works (in rooms) in 5th Right off 2nd North West Mains at spad No. 1364 No. 6 Entry. The fall is approx 30 to 35 ft long, 20 ft wide and 4 1/2 ft to 5 ft thick (Ht). The fall will not be cleaned up.
A roof fall occurred on 1-5-12 in 2nd North West Mains at spad #1341. The fall is approx 30 to 35 ft long, 20 ft wide and 4 1/2 to 5 ft thick (no injuries) and the fall occurred in old works and will not be cleaned up (in #5 Entry)
Mechanic was performing maintenance on John Deere 850c Dozer when he slipped and fell from the tracks to the ground injuring left leg.
EE was installing hydraulic cylinder on a roof bolter. He was lying on his back lifting the cylinder up on to the roof bolter then he had a sharp pain in the middle of his back. EE did not quit working till Monday, November 7, 2011.
He was getting a tire for the man trip when he slipped and fell (on ice). He did not cease work, he went to the doctor on 2/2/11. He still worked until 3/22/11 (that was his 1st day off work). He went to the hospital and they operated on his left shoulder on 3/22/11.
The scoop man had knocked the stopping down. He was backing the scoop up so that he and another ee could pick up the loose blocks. EE was walking toward him. The scoop man looked away and when he looked back toward ee light going down as if he had stumbled and fell. EE was transported by ambulance to the hospital after he stumbled and fell
EE was using a torch to cut bracket to change water hose,accidently cut hydraulic hose and threw oil in his face and hands (burns).
Going to check the stuck roller behind the head drive. Lost footing and fell.
EE was hanging a fly pad in the face of the section. He stepped in a hole and twisted his lower back. EE did not work on September 11, 2010. He did not work on September 13th, 14th 15th. He did work on September 16th, 17th, 18th, 19th & 20th. He did not again till October 4th, 5th, 6th. On Oct 6th he worked 6 1/2 hours. He has not worked since Oct 6th to date.
2 EEs & Foreman were making a splice in #1 Belt. They were using two 1 1/2 tons chain hoists to pull belt back together. 1 man on each side of belt with 1 man operating each hoist. Belt was almost back together when EE stopped ratcheting the hoist & said he'd done something to his rt shoulder. This happened 7/12/10, he worked thru 7/24/10, went to dr & didn't come back to work.
He was hanging a cable when a piece of draw rock fell and hit him in the back.
