Close Clearance and not keeping all body parts inside of mantrip. Employee crushed EE's middle and pointer fingers of EE's right hand when EE grabbed the canopy post to reposition self in the mantrip. The mantrip was traveling around a tight corner where clearance is close and the employees right hand was in a pinch point between the canopy post and the coal/rock rib.

A non-injury roof fall occurred on the #3 Belt Conveyor on Northwest Mains at survey station #546. The fall measured approximately 20 feet wide by 5 feet thick by 50 feet in length. A roof fall clean up plan has been submitted to MSHA and has been approved for clean up of the fall. This fall was immediately reported to MSHA. No person(s) were involved in the fall.

Roof Fall--70' in by the survey station #1042 in the cross cut between #4 entry and #3 entry on the 2nd Northwest Mains 3,400 feet from our section. This is an area where we pulled back from. It measured 6' thick by 16' wide by 35' in length. It is not affecting ventilation. We will not be cleaning the fall up, we are dangering it off. No employees or equipment was involved.

The accident took place on the haul road at #1 Pond area. EE came to the mine to pick up paycheck. EE left to go home traveling on the haul road. The location where the accident occurred was at the #1 Pond area, EE hit a coal truck head on in a curve driving EE's personal vehicle. EE was treated by Elite ambulance technicians and transported to ARH hospital in South Williamson.

Fall was outby survey station in the #2 neutral entry on Southwest Mains. It fell in an intersection and measured 30' long by 30' wide by 8' in thickness. It was located approximately 1800' outby the 010 mmu. No persons or equipment were affected by the fall. Set at least 2 rows of timbers on each side of the fall. Do not intend on cleaning the fall up. No ventilation was impaired.

The fall occurred in the #1 return entry of Southwest Mains 70 ft inby survey station 970. The fall was in an intersection and measured 20 ft wide by 20 ft long by 8 ft thick. The fall did not impair ventilation and we do not intend to clean up the fall. It was blocked off by setting 8 breaker timbers in all entrances. No persons or equipment involved. Roof fall only.

The fall occurred at the crosscut entry between #5 and #6 intake entry 140ft inby survey station #983 located on the Southwest Mains. The fall measured approximately 30ft long by 18 to 19ft wide by 7 to 8ft thick. The fall did not impair ventilation or obstruct the primary escape way. No persons or equipment were involved. The area was blocked off and isn't intended to be cleared.

A piece of draw slate approximately (3 foot by 2 foot by 3 inches thick) broke across the strap and fell striking ee. Injured miner was transported to a medical center. Due to medical complications from the injuries ee sustained ee died on 4/6/17

EE was tramming the miner from #1 entry to #5 entry, he stopped to move the miner cable and the water line out of the road. When he picked it up to move it out of the road so the miner and buggy could get by, he felt something in his chest. His chest was hurting and he left the entry and went outside.

Employee was hanging fly pads between #5 and #6 and a piece of top rock fell out from the rib bolt to the rib about 7 or 9 foot long and about 3 foot wide and about 3 or 4 inches thick braking the fly board and hitting employee on his left side in the head, neck, back and shoulder areas knocking him to the ground.

This fall is a continuance of a fall reported on 1/5/12 on the 2nd North West Mains @ Spad #1341 in the #4 and #5 entries. This fall does not impede ventilation or travel-The fall will not be cleaned up-There were no injuries.

He was getting a tire for the man trip when he slipped and fell (on ice). He did not cease work, he went to the doctor on 2/2/11. He still worked until 3/22/11 (that was his 1st day off work). He went to the hospital and they operated on his left shoulder on 3/22/11.

The scoop man looked away and when he looked back toward ee light going down as if he had stumbled and fell. EE was transported by ambulance to the hospital after he stumbled and fell

EE was hanging a fly pad in the face of the section. He stepped in a hole and twisted his lower back. EE did not work on September 11, 2010. He did not work on September 13th, 14th 15th. He did work on September 16th, 17th, 18th, 19th & 20th. He did not again till October 4th, 5th, 6th. On Oct 6th he worked 6 1/2 hours. He has not worked since Oct 6th to date.

2 EEs & Foreman were making a splice in #1 Belt. They were using two 1 1/2 tons chain hoists to pull belt back together. 1 man on each side of belt with 1 man operating each hoist. Belt was almost back together when EE stopped ratcheting the hoist & said he'd done something to his rt shoulder. This happened 7/12/10, he worked thru 7/24/10, went to dr & didn't come back to work.
