A piece of draw slate approximately (3 foot by 2 foot by 3 inches thick) broke across the strap and fell striking ee. Injured miner was transported to a medical center. Due to medical complications from the injuries ee sustained ee died on 4/6/17

Employee was plastering a return stopping when the ladder employee was standing on fell over. Employee fell to the ground trying to catch themselves by putting out right hand. Employee hit the ground injuring right shoulder and right hip.

A roof fall clean up plan has been submitted to MSHA and has been approved for clean up of the fall. This fall was immediately reported to MSHA. No person(s) were involved in the fall.

Roof Fall--70' in by the survey station #1042 in the cross cut between #4 entry and #3 entry on the 2nd Northwest Mains 3,400 feet from our section. This is an area where we pulled back from. It measured 6' thick by 16' wide by 35' in length. It is not affecting ventilation. We will not be cleaning the fall up, we are dangering it off. No employees or equipment was involved.
The accident took place on the haul road at #1 Pond area. EE came to the mine to pick up paycheck. EE left to go home traveling on the haul road. The location where the accident occurred was at the #1 Pond area, EE hit a coal truck head on in a curve driving EE's personal vehicle. EE was treated by Elite ambulance technicians and transported to ARH hospital in South Williamson.

Do not intend on cleaning the fall up. No ventilation was impaired.
The fall occurred in the #1 return entry of Southwest Mains 70 ft inby survey station 970. The fall was in an intersection and measured 20 ft wide by 20 ft long by 8 ft thick. The fall did not impair ventilation and we do not intend to clean up the fall. It was blocked off by setting 8 breaker timbers in all entrances. No persons or equipment involved. Roof fall only.

The area was blocked off and isn't intended to be cleared.

A fall measuring 20' wide X 25' long X 4.5' thick occurred at 140' inby survey station 925 in the #7 right return entry on the Northwest Mains. The fall will not be cleaned up. We are re-routing the return around the fall. No persons of equipment were effected. All entrances to the fall will be barricaded with timbers or cribbed off

The mine had a roof fall at survey station #10 in an intersection. The fall measured 30' L. X 7' Thick X 12' W. An addendum to the approved roof control plan will be submitted to MSHA to rehab the fall area.
