In #e Entry, miner cable caught his legs and threw him against the rib. Right knee is swollen. Left knee was twisted back hurting. He was transferred to hospital by ambulance.

EE was installing hydraulic cylinder on a roof bolter. He was lying on his back lifting the cylinder up on to the roof bolter then he had a sharp pain in the middle of his back. EE did not quit working till Monday, November 7, 2011.
The scoop man had knocked the stopping down. He was backing the scoop up so that he and another ee could pick up the loose blocks. EE was walking toward him. The scoop man looked away and when he looked back toward ee light going down as if he had stumbled and fell. EE was transported by ambulance to the hospital after he stumbled and fell

Going to check the stuck roller behind the head drive. Lost footing and fell.
EE was hanging a fly pad in the face of the section. He stepped in a hole and twisted his lower back. EE did not work on September 11, 2010. He did not work on September 13th, 14th 15th. He did work on September 16th, 17th, 18th, 19th & 20th. He did not again till October 4th, 5th, 6th. On Oct 6th he worked 6 1/2 hours. He has not worked since Oct 6th to date.
2 EEs & Foreman were making a splice in #1 Belt. They were using two 1 1/2 tons chain hoists to pull belt back together. 1 man on each side of belt with 1 man operating each hoist. Belt was almost back together when EE stopped ratcheting the hoist & said he'd done something to his rt shoulder. This happened 7/12/10, he worked thru 7/24/10, went to dr & didn't come back to work.
Location at the end of the track. Driving a rubber tired four wheeler and the corner chain was hanging down, ran into the chain. The hook caught the inside of his left leg. The hook caused a gash in his left leg.

Two breaks inby spad 4771, a roof fall was found. EE & mine inspector found the fall. The size of the fall was 20 ft by 20 ft long and 6 to 8 feet high. The fall will not be cleaned. The fall is timbered and dangered off.
Location Break #12, #14 belt; Crew moving section. Putting the power on the roof bolter machine. Cat head on the power center blew up and caught fire.
EE states they were moving equipment from one part of mine to another & were getting ready to move #2 bolter machine when EE plugged up CAT head and put breaker in @ Break 12 #4 Belt. While putting in breaker it blew up and burnt back of both hands. Co-worker brought him outside and drove himself to hospital.
The roof fall occured at the 1st of left hand rooms number 6 belt, 3 crosscuts to the left of spad 4244. The length of the fall was 60""long, 18' wide, 5 1/2"" high. The passage of the men were not eggected. No injuries occured from this fall. The fall will not be cleaned up. The time of the roof fall was 10:20 pm Person reported the fall at 10:50 pm ...fall have been supported
A roof fall occurred at outby spad #1499 and was discovered at 1:15 am. The x-cut fell in at #5 entry through break over to start of #6 entry. The length of the fall was 6 x 18 x 6. The fall will not affect the passage of men. No injuries occurred. All access to the fall was timbered and barricaded.
Location in Mains. EE was changing rearend in a rubber tired mantrip. The mantrip fell of of blocks, landing on right leg. He finished out shift before going to the Montgromery General Hospital ER.

Injured employee was helping the rest of the crew wind belt w/belt winder when belt winder slid out of scoop bucket - striking his right foot.
EE was bolting top and dropped the head of the pinner down to remove the drill steel, when the drill steel came back and struck the EE on the right index finger. The finger was broken in two places.
Unhooking the coupling from a flat car, it did not disengage, causing flat car to hit his leg between right calf and knee area.
This is for an unintentional on 1st left panel off of #5 belt #3 entry spad 3018. Does not affect travel or air. Approximately 60-70 feet long, 18 feet wide. Above anchor. Fall will not be cleaned up. Fall is timbered and dangered off.
Location: Mains. Employee was welding on the lug of a miner, chipping away from the weld. A piece of metal flew up and went into his left eye. He stated he did have his glasses on.
Cut right leg making belt splice in Mains Section.
Section Mains, EE was putting rear end in a rubber tired 4 wheeler. The rear end shifted back a cut off the first join of the 4th digit on right hand.

Traveling on a rubber tire mantrip at the end of the shift. The mantrip cut close to the rib to let the 1st shift go by. The rib caught the toe of his boot twisting his right knee, right ankle and right foot.
Driving 4 wheeler, hit a rock. The steering wheel whipped around hitting hand.
Employee was attempting to open airlock doors; doors suddenly shut catching his wrist in them. Unknown injury to right wrist. 11/2/07 went to hospital: chiped bone right wrist.
Muddy conditions. Stepping over a coupling on a flat car, EE tripped on the coupling. His right ankle hit the track.
001 section cross cut between 1 & 2 entry 2 breaks inby spad # 4146 # 2 entry. The fall fell between #1 & #2 entry last open break. Length Approx 100' x Width 19' x Height 4-5 foot. The fall will be cleaned up.
#5 Entry intersection. Hauling coal on the shuttle car from the miner to the belt. He hit a large rut or hole in the roadway. Knocked the power on shuttle car. He hurt his neck, back, hips. Could not feel his foot and right leg. They were numb.
#2 Belt #6 Entry, Spad No. 5554 #5 Entry Roof Fall. Intersection fell straight across in #6 entry. Length of fall 25 ft, Width of fall 20 ft, Height of fall 4 ft.
EE was rock dusting Friday, Aug 31, 2007. He injured his left shoulder draggng the rock dust hoses. He finished his shift. After Labor Day weekend, he returned to work Tuesday. His shoulder still hurt. He did not work Thurs or Friday. When he reported Monday, he reported incident & sent to Dr. He was told he had a pulled muscle in shoulder & returned to work 9/11
Mantrip struck in tram with no breaks It worked fine for the first 6 hours then lost it's breaks. injured left shoulder, right knee, low back.
Mains Section. Setting timbers in Mains when he turned and pulled his left hip.
#3 entry mains section, walking throught crosscut he hit head on brow.
Skining the belt in preparation of a belt splice. Utility knife passed through belt & cut left hand.
Going to do belt splice on #6 belt; grabbed top roller underneath belt, pulled on roller & pulled muscle on right shoulder.
Loading 4' straps, band broke, striking left hand and cutting it. Caused swelling of the left wrist.
Drilling into the top, a piece of rock fell on top of EE's left foot.
Miner boom contacted shuttle car, lifting it up and then the shuttle car slammed to the mine floor. It occurred in sub mains, break 27, approximant location.
Miner cable was in roadway for buggy. EE bent down to pick up cable and he heard a pop in his chest and hurt in his chest. He went on with is shift.
EE was running shuttle car. There was a metal roof support strap hanging down and it came inside the shuttle car. It cut the right ear of him and just about cut the ear, He drove himself to montgomery gen. emergency. He had 7 stitches and gave him antibotic cream. Check up following Mon. Before shift and was ok to go to work. shuttle car.
Roof fall length 130' 2 and height 5-6 ft #4 belt 5 Rt area room off of #1 entry. 1 brake inby sped #'s 13 74, 1375, 1388. no person involved no equipment involved. all egress timbered off. fall will not cleaned up.

Injured employee and a co-worker were changing out batteries on the scoop. While going to get another set of batteries, the co-worker on the scoop steered the scoop and it pivoted and pinned the injured employee between the scoop and battery.
EE walking backward rock dusting-stepped in hole where floor cut unevenly. Hurt arm and shoulder, when he fell backwards hit scoop before landing on mine floor. Contusions to shoulder and forearm worked 14th-15th pain worse swollen not on shoulder wnt to dr. Hospital refer him to dr. on 20th. He is to do MRI on 9-21-06.
The employee was on a 4' step ladder. When he went to get more bobond he slipped off of the ladder & landed on the mine floor.
While bolting top, employee was drilling the top with a 4'drill steel. The drill steel bowed in the middle and spun around hitting the EE in the right arm.
EE was loading belt structure on scoop, when he felt and heard a pop and pull in his right shoulder.
Spot bolting the top with a single head bolter when his left hand was caught between the jack & the arm of the bolter. The end of the left index finger was pinched off.
While he was walking through a curtain a piece of rock fell from the roof and cut his left hand. The rock was about the size of a plate. He received a cut between his thumb and forefinger.
TORSO STRAINED FROM REAL BAD JAR WHILE RUNNING #10 BUGGY. HE KEPT RUNNING FOR AWHILE BUT COULD NOT HOLD THE BRAKE AND SO HE GOT OFF BUGGY WENT TO HOSPITAL. DROVE HIMSELF. REPORTED TO HIS BOSS.
EMPLOYEE WAS BOLTING UP STRAP & GOT HIS RT. INDEX FINGER CUT BY A PIECE OF ROCK THAT FELL HITTING HIS HAND.
Employee was lifting a smaller toolbox out of a bigger box and felt pain and the right arm went numb.
TRAVELING OUT OF MINE ON MAN TRIP. THE MAN TRIP DID NOT STOP & TRAVELED THROUGH THE CLOSED DOORS. INJURED EMPLOYEE WAS RIDING IN THE FRONT SEAT. THE RIGHT SIDE DOOR STRUCK THE EE'S UPPER RIGHT ARM.
Victim was pulling high line cable, when went around a corner the cable slipped out of his hands hitting his right leg, turning his foot. Employee fell backwards.
ROOF FALL IN #3 ENTRY (TRACK) ON SUB-MAINS SECTION. APPROX. 80'X 20'X 10', 0-10' IN CENTER. ALL ACCESS HAS BEEN TIMBERED & CRIBBED OFF. THE AREA HAD 4'FULLY GROUTED & 6'TENSION REBAR BOLTS IN IT. AS CLEANING PROGRESSED 8' & 12'SUPERBOLTS INSTALLED & HEADERS WITH CRIBS SET, TOP HEADER CRISSCROSSED & CRIBS SET ON TOP.

The injured employee pulled roof bolter up then swung head of bolter out to put up outside bolts. swung head back in, drilled hole, put bolt & glue in hole. Employee set the head of the bolt in the chuck, hit the rotation control to set bolt down in head. Employee's glove had went in hole with bolt, finger was twisted with bolt.
Putting 6"" water line together. When trying to align the pipe, another employee hit the other end of pipe catching the injured employee's finger.
Employee was on knees mixing B-Bond. Safety glasses were fogged up. He removed safety glasses to let dry & continued mixing up B-Bond and the B-Bond splashed into his face and into right eye. He went to boss and boss flushed out eye with water. Employee finished shift and completed job of building a stopping. Next day eye was swelled shut. EE went to Dr.
Rock fall 4.5' to 5' thick, 20' wide, 50' long. #7 entry 1 break inby spad 5460. Will not be cleaned up. #8 entry will be used as return.
Pulling S/car with 20' chain (1/2""). Chain broke. 14 links & hook hit employee on right hand, right leg, and groin area. He was standing app. 30' away when chain broke. Employee returned to work the next day. Bruises.

Roof fall at spad #1332 in #1 entry return. Approx 6' thick x 20'. All access has been timbered off & return has been routed around the fall. Area was bolted with 4' resin bolts (fully grouted) with some 6' tension rebar bolts spotted.
BROKEN FLYBOARD CAME THROUGH DECK UNDER CANOPY AND HIT NOSE.
Victim finished bolting (corner bolt). He reached over to let canopy down but hit swing lever by mistake. Bolter moved left hitting his left knee - hyper-extending his knee.
Roadway rough; employee jarred his head into canopy and strained his lower back.
Lifting jack cylinder when employee felt a burning sensation in his lower stomach.
