We had a roof fall measuring 4 to 5 feet thick X 19 to 20 feet wide X 70' in distance. This fall does not impede ventilation or travel-The fall will not be cleaned up-There were no injuries.

Mechanic was performing maintenance on John Deere 850c Dozer when he slipped and fell from the tracks to the ground injuring left leg.

Employee was removing gob from #4 tailpiece. The gob was coal and rock mixture, a piece of rock fell thru and rolled onto right index finger, smashing the employees finger.

He was installing curtain boards and when he was removing drill steel, he rotated the steel and the drill steel hit his forearm.

EE was cutting a zip tie and the blade came out and cut his right hand.
