Miner was stacking stone on a pallet when a loader passing by flipped a stone striking the miner in the left knee.
Miner was hammering on a small piece of metal when a shard from that piece of metal splintered off and became embedded in the back of EE's hand.

As EE disconnected the filling hose, the pin in either the filling nozzle or the forklift propane tank stuck open allowing propane to escape under pressure. The propane hit the back of the miner's hands and caused freeze burns resulting in skin loss on the back of the right and left hands.
