Struck by flying object

Miner was hammering on a small piece of metal when a shard from that piece of metal splintered off and became embedded in the back of EE's hand.

Struck by falling object

Miner was climbing up a rock pile when a rock shifted and caused EE to fall. When EE fell, the ice auger that EE was carrying struck EE's left wrist and cut it open. Four stitches were required to close the wound.

Miner was blocking a conveyor. The conveyor shifted and pinched a wrist between the conveyor and a screen plant.

Miner was washing the upper deck of the crusher plant screen and slipped. Miner strained right quadriceps.

Struck by flying object

Miner was pounding on a pin from the track of a backhoe with a hammer when a steel shard from the pin broke off and embedded itself in right shin.

Contact with hot objects or substances

Miner was working to repair a crusher using parts cleaner. Parts cleaner and oil from the crusher got onto EE's pants. When EE used a grinder to cut steel sleeve, sparks ignited EE's pants and EE's knees and calves were burned.

Struck against stationary object

Miner was using a Tamrock drill rig and was attempting to remove the drill bit by hitting it with a hammer. Another hammer was propped against the drill bit to secure the bit. When EE struck the bit with one hammer, the other hammer slid down pushing EE's finger across the broken drill bit which lacerated the finger.

Miner was operating a backhoe equipped with a rock hammer. After several hours of operation, ee began to feel numbness in right arm and right hand.

Driller was changing the steel when he smashed the #2 finger of his right hand between the steel and a piece of stone causing a laceration needing 7 stitches to close. Worker returned to work with restrictions.

Worker was removing nuts using and air impact wrench and his right wrist became sore. He did not report the injury to management until 05/26/2009. Went to a clinic and was placed on restricted duty. No lost time.

Worker was repairing a air-powerd greaser; the piston became stuck. When he tried to free the piston the fingers of his left hand became trapped by the piston causing a laceration to the middle finger of his left hand requiring stitches to close the wound.

EE WAS GRINDING IN A REAL TIGHT SPOT AND THE GRINDER (9"" HAND HELD GRINDER) KICKED BACK AND HIT MY LEFT KNEE. SOME SWELLING AND HURTS WHEN HE BENDS THE KNEE BUT IF HE STANDS UP STRAIGHT DOES N OT HURT.

EE WAS CLIMBING DOWN THE LADDER ON A LOADER WHENHE STEPPED ON THE LAST BAR OF THE LADDER & IT GAVE WAY CAUSING HIM TO FALL & HIT THE SIDE OF THEMETAL LADDER CRACKING 2 RIBS ON THE RIGHT SIDE

EE WAS WALKING FROM FILTER ROOM PAST BATH ROOM TOWARD FRONT SERVICE DOOR BY PRESSURE WASHER, NEAR BATHROOM AND TRIPPED OVER PORTABLE STAIRS NEAR HOSE PRESS MACHINE. NOW IS EXPERIENCING SORE LO WER BACK AND RIGHT SIDE, MAY HAVE CRACKED A RIB. 2/14/00 UPDATE - SAW DR AND ALL IS O'KAY PER EE AND DR. NO CRACKED RIBS.

EE WAS RUNNING A LOADER WHILE LOOSENING DIRT IN QUARRY. WHILE DOING SO, LOADER HOOKED STONE JERKING LOADER AND EE WITH SAFETY SEAT BELT ON SIDE WAYS CAUSING EE TO HIT THE ARM REST OF HIS SEAT LEFT SIDE.
