Piece of metal got into employees' eye sometime during shift, unknown how it happened.

Employee was cutting flashing with a knife, knife slipped and cut EE's leg. Took several stiches to close the laceration.

Employee pinched EE's finger between two metal stands while moving the stands.

Operator rubbed EE's eye and got a small piece of metal in EE's eye. Next day eye was swollen and doctor had to grind a piece of metal out of the operators eye.

Operator set down a grinder while cutting disk was still spinning, disk contacted the conveyor belt and jumped to the side. The spinning disk contacted the back of the operators hand and caused a laceration to the hand. Laceration required stiches to close.

Employee tripped over a pallet, fell to ground and caused a contusion to right arm.

Employee was striking a steel pin when a metal shard broke off and embedded in EE's leg. After metal shard was removed, a stitch was required to close the laceration.

Operator fell down a flight of steps and broke left arm.

EE was installing a tire on equipment when their middle finger got caught between the snap wire and the rim flange causing an injury and cut on the right middle finger.

Employee fell over a ladder and landed on shoulder and back.

Employees were performing maintenance on a crusher screen. One employee inadvertently struck partners finger with a hammer while installing screen clamp bolts. The injured employee has an open fracture of the left middle distal phalange.

Using a grinder to remove a bracket and at some point got a piece of steel in their eye.

Operator was stepping down from catwalk to another, stepped wrong and twisted knee and fell onto catwalk.

Using air nozzle to blow away debris, small piece got into left eye.

Holding cutting edge bolt in place, impact wrench caused bolt to spin and cut index finger on employee.

Carrying ANFO to drill holes from pallet & stemming holes. Bags of ANFO weighed 50 lbs each. Tenderness felt @ 11:00 AM (described as muscle ache). Incident did not get reported until 10/12/22 (when medical evaluation was sought).

Striking a piece of metal with a hammer, small shard of steel flew off and struck the miner in the knee. Operator needed surgery to remove the piece of steel and four stitches to close the wound.

After standing on their feet all day, operator felt tightness in their back and numbness in their legs.

Operator was running to shut down a conveyor and felt a pop in lower back.

Laceration to left wrist

Metal guard fell and struck operator in the head, causing a laceration needing staples to close.

Operator was struck in the hand while releasing the handle on a ratchet strap. Impact from the handle fractured Operator's thumb.

Employee was walking around truck when left foot broke through the ice. Employee's right foot slipped on the ice and employee fell to the ground breaking lower leg.

Employee smashed a finger while moving blocking.
