Operator rubbed EE's eye and got a small piece of metal in EE's eye. Next day eye was swollen and doctor had to grind a piece of metal out of the operators eye.

Operator set down a grinder while cutting disk was still spinning, disk contacted the conveyor belt and jumped to the side. The spinning disk contacted the back of the operators hand and caused a laceration to the hand. Laceration required stiches to close.

Employees were performing maintenance on a crusher screen. One employee inadvertently struck partners finger with a hammer while installing screen clamp bolts. The injured employee has an open fracture of the left middle distal phalange.

Carrying ANFO to drill holes from pallet & stemming holes. Bags of ANFO weighed 50 lbs each. Tenderness felt @ 11:00 AM (described as muscle ache). Incident did not get reported until 10/12/22 (when medical evaluation was sought).

Striking a piece of metal with a hammer, small shard of steel flew off and struck the miner in the knee. Operator needed surgery to remove the piece of steel and four stitches to close the wound.

Employee was assisting to remove cutting edge bolts from a loader bucket that was properly blocked & locked out. Employee failed to use PPE. Since injury we have retrained employee as well as gave additional task training. Employee got EE's finger cut which required five stitches. No long term impairment.
Operator was struck in the hand while releasing the handle on a ratchet strap. Impact from the handle fractured Operator's thumb.
