Showing 50 of 96Employee got into bed of service truck to ride to office. EE stated EE was sitting with EE's back to tailgate, tailgate popped open allowing EE to fall backwards onto ground, cutting back of head which required 2 stitches.
Employee was outside under feeder cutting bolt heads off. Employee stated wasn't feeling well and started coughing, stopped cutting and sat on tailgate of truck till started feeling better. Next day came in still coughing sent to ER.
Large rock was on rock breaker hydraulic line, went to roll rock off line when it rolled catching thumb between rock and frame of breaker causing a small laceration.
Injured was helping replace return roller on conveyor using a come-along when hook slipped and hit employee in face/eye causing an abrasion to eye.
Injured was removing dirt off a R60 Hauler with Compressed air, piece of debris went under safety glasses and into their eye
Employee was greasing the loader first thing in morning when EE slipped and fell.
This was a medical occurrence only. Customer truck driver was backed into stockpile waiting to get loaded when EE started having chest pains. Called 911 they took to Pulaski hospital where they found EE's blood pressure was elevated.
Was removing conveyor belt off welding truck, hit hand against vise, next day hand started hurting. Went to DR took x-ray, would let EE know. They called July 5 and said EE did have piece of metal. They removed it July 6, no stitches, no lost time.
This is not an accident or injury. Employee was operating a loader when EE needed to use the restroom, EE went to stand and could not feel EE's leg, ambulance took to hospital where they found EE had low potassium, EE was released and told to return to work Monday morning.
Employee was picking up to 2 - 2.5 gallons of DEF when EE stated EE twisted back causing pain.
Employee was cleaning primary feeder out for repair work when they ran a finger across a sharp metal burr cutting finger that required stitches.
Employee was welding in the box chute off #1 screen when a piece of hot slag fell on EE's pant leg and caught pants on fire causing a burn to lower leg.
Removing a brass bushing off main shaft of crusher, bushing had oil on it. Another co-worker was helping lift bushing off but hand slipped causing bushing to fall and caught employee's fingers under bushing causing bruising to end of index, middle and ring fingers on right hand.
Employee was opening our entrance gates, the gate has a wheel on it to where you roll gate open, wheel got turned employee kicked wheel with foot when EE heard and felt a pop in knee causing a strain to knee area.
Going to work on crusher went to use auto crane on F750 service truck when cable bird nested on cable wheel. Pulled all the cable out to rewind on wheel, as they were reeling cable in cable caught employee's glove and pulled it in between crane structure and cable wheel causing injury to fingers
Employee was using digging bar where rock was lodged in chute at Primary plant. Rock dislodged, hit digging bar and smashed employee's left pinky finger. Employee has to wear finger splint until cleared by Doctor.
Employee was walking down a slope behind our electrical building when ee stepped on wet leaves and mud, ee slipped and fell landing on left elbow shoulder area causing chronic irregularity of the clavicle from old injury ee had before EE came to work for us.
Employee was performing maintenance on #2 screen. Using a pry bar on counter weight when bar slipped. Felt the right shoulder pop. Employee is the superintendent for the welding team. At the time of the accident they had no lost time or restrictions. Later MRI was done and found a torn rotator cuff. Surgery on November 28, 2018.
Customer came into Quarry to get load of stone. Driver backed truck up next to material, loader operator went to load truck, after they loaded truck they went to motion to driver that they were loaded and noticed that drivers head was laid back against cab of truck. Loader operator went to check on driver found them unresponsive, called 911, ambulance took driver to hospital.
Employee was climbing down ladder of 988F loader when employee missed ladder with employee's left hand putting all employee's weight on employee's right arm. Employee has a right bicep tear (employee weights 500 plus)
Employee was exiting loader, when ee reached the 3rd rung hands slipped on handrails causing ee to fall to ground. At the time it was raining hard and handrails were wet. Took employee to ER to get checked out and they found nothing other then some bruising. Dr's excuse said to return in 2 days but employee was not scheduled to work except for half a day the following day.
Employee was changing out motor on #1 Conveyor. EE was using a pry bar to help lift motor when bar slipped letting motor fall on tip of finger causing fingernail to pop off and cutting tip of finger.
Employee was greasing rock hammer at jaw and slipped backwards, tried to catch self with right hand, landed on rock cutting right thumb that required stitches
Employee was working on AC unit for a 988F loader. EE was inside the cab cleaning dust out of the vent when EE cut finger requiring stitches.
Employee was carrying a chain over shoulder at secondary plant, ground was muddy, foot slipped causing the chain to swing around and hit employee in the mouth chipping a tooth.
Employee was changing a light bulb in the control panel at our pug mill plant. The bulb broke causing a laceration to right index finger tip. Laceration required stitches.
Employee drives an on road dump truck, EE went to get out missed the bottom step and fell to ground. EE bruised EE's hip and back area causing EE to lose time from work
On 1/28/16 we had annual refresher, EE states chest was hurting some, but did not go to Doctor. 1/29/16 EE went to work then chest still hurting so called spouse, spouse took EE to ER. We called MSHA 800 and reported. This injury was not job related, EE had inflammation of the cartilage (bone that attaches the ribs to breastbone)
Employee went to grease line for #2 conveyor when EE tripped, not sure what EE tripped over, tried to catch self and hand landed on a rock causing a laceration. Laceration required 4 stitches.
Employee operating a excavator, throwing material off bench to pit below. Employee called Foreman asked him to come to excavator. Foreman arrived employee stated he did not feel well & left arm hurting. They immediately put employee in vehicle took to hospital. In route notified me I called 800# to report. Employee required 2 stents and will have open heart for one blockage.
Employee was putting teeth on a 988 Cat Loader. He was using a hammer to knock pin clips in when he hit the tooth with hammer causing a piece of metal to chip off and enter employee's upper leg. As of now its still not decided if they will remove or leave metal in.
At 1:50pm employee called foreman to help him down from crusher platform. While cleaning up for crusher maintenance employee slipped, his foot hung between conduit and I-beam, his body weight falling against his pinned leg causing leg injury. He was taken to hospital where it was later determined to be fractured.
Employee was removing a metal strainer from a pvc line at our Sand plant, when he tried to remove the pvc line from the metal strainer he cut top of left thumb.
Employee was standing on crusher platform using a digging bar to try and dislodge a wedged rock in jaw crusher feeder when bar slipped catching his finger between the bar and handrail. This caused a laceration to tip of ring finger on right hand requiring 8 stitches.

EE was on a scaffold 4' off ground welding hooks to hold electrical cables, he went to move and a board had been moved causing him to fall. Lanyard caught him but he tit his rt hand on scaffold causing injury. He did not report to management. Co-workers asked if he needed to go to Dr, he stated he was okay. Reported 11/12/12. He has surgery for joint locked in flexion.
Employee was operating a D8 dozer with cab, he was traveling around area that was being stripped of overburden. A rock being seen by employee was misjudged for its size (part of rock was covered with dirt)the track runs over rock when dozer came down it tossed employee forward hitting forehead causing a laceration that required stitches.
Employee was working on conveyor, he was using a hammer to put a bolt throught a turnbuckle when he missed bolt and hit his left thumb causing a laceration requiring sutures.

Employee and a co-worker was using hammer to clean out a box at old pugmill, co-worker went to hit box, hammer glanced off angle iron, came back and hit employee in eye with hammer. Safety glasses broke and cut employee above eye, requiring stitches.
Employee was walking to #1 tailpulley when he stepped across about a 2 foot puddle of water, he felt a pop in his leg. Employee pulled a muscle in upper right leg, causing him to use crutches with limited walking.
EE was putting new brake pads on dump truck when he felt a strain in lower back. Employee went to doctor and was put on light duty work for 4 days.We are looking into this accident. We had a helper with him, he did not report any injury friday before he left work but came in monday morning first thing and reported back injury.
Employee was using a pocket knife, to cut tape that was holding stirring paddle, off a bucket of backing. The knife slipped, causing a laceration to his left thumb.
