Showing all 14A small piece of metal left over from cutting a pipe with a bandsaw was left deposited on my sweatshirt. When I took off the shirt, a piece of metal got in my eye. An eye doctor removed the piece on 5/1/2021 and I am back to normal.
Installing a heavy drive shaft on dredge. Employee set drive shaft down to reposition employee's body and the U-joint pivoted and pinched the tip of employee's finger. Fracture to the tip of left hand ring finger and contusion under fingernail.
While stepping up onto the dredge out of the row boat, employee's knee popped and caused them pain. Employee has a slight tear in the cartilage below the knee.
Employee was attempting to use three points of contact when exiting the loader and missed the hand rail causing them to balance and fell about 5' to the ground injuring right upper thigh. It is believed that they broke their femur bone.
Pit supervisor was pushing dirt, when backing up the operator lost track of where operator was at and backed over a berm causing the Caterpillar D6H Dozer to roll backwards 270 degrees landing vertical on it's blade. The operator was able to exit the machine without any injuries. The dozer caught on fire, the fire department was call and the fire was extinguished.
Employee experienced light headedness, weakness, seeing spots, and tightness in the chest while repairing a winch on the dredge. Prognosis has not been disclosed to the employer as of the time of this report.
Miner was greasing a dewatering screw when elbow locked up & miner felt a pop in the joint. Miner did not report incident to management until 6/3/16 and did not seek medical treatment until 6/7/16. Miner was referred to another doctor on 6/21/16. We currently do not know the extent or cause of the injury but the miner is still working under light restrictions.
Employees were lifting a pipe using a loader and chain. An employee was adjusting the chain and positioned his body under the loader bucket. The loader operator lowered the bucket to provide slack in the chain pinching the ground employee between the loader bucket and pipe, breaking three ribs.
In work boat tied off to the dredge, pulling on 3/4" ratchet wrench to tighten 7/8" diameter bolt. On 9/30/15 went to clinic because of low back pain.
Moving a large tool chest and needed to slide it over, pushed against it with the right shoulder and arm and strained a muscle on the right side of the body.
Employee was pumping on jackhandle when the jack kicked out & metal stand hit him in the head.
EE said he was pulling and lifting on screen retainer to slide retainer out of the way so they could replace screen.
Just moving a battery from one spot to another & felt a pop in left shoulder.
