A small piece of metal left over from cutting a pipe with a bandsaw was left deposited on my sweatshirt. When I took off the shirt, a piece of metal got in my eye. An eye doctor removed the piece on 5/1/2021 and I am back to normal.

Installing a heavy drive shaft on dredge. Employee set drive shaft down to reposition employee's body and the U-joint pivoted and pinched the tip of employee's finger. Fracture to the tip of left hand ring finger and contusion under fingernail.

EMPLOYEE WAS OPERATING THE DREDGE WHEN HE START-ED FEELING CHEST PAINS. HE WENT TO THE HOSPITAL TO BE LOOKED AT AND THEY SENT HIM TO OMAHA BY AMBULANCE WHERE HE DID END UP HAVING A HEART ATTACK.
