Employee alleges was walking from scale house to employee's work truck and heard a pop in right foot. Employee was diagnosed with a fracture to the right foot.
Employee was pulling wire by hand and heard a pop in their left wrist. Injury was reported to employer 2/7/25. Injury is musculoskeletal, muscle strain and tendon tear. Injury occurred in AM. Injury initially recorded as an OSHA injury, error identified 7/2/25 and reported to MSHA.

Employee was using a torch to make holes in the fog washer and a foreign object blew into EE''s right eye. The Dr. had to use a needle to move the foreign object out of EE's eye by the cornea. Late reporting due to communication error.

Waited 1.5 days to notify management at which point EE was sent for medical on 12/15, foreign object removed from eye. Employee was wearing prescription glasses during grinding instead of Company supplied goggles.

Employee was doing maintenance at the plant, removing a section of catwalk. While doing so, the catwalk fell hitting EE's wrist pushing it into the hood of the scalping screen causing laceration on the outside of EE's rt wrist.

Employee was belt splicing when EE reached for the skiver tool by twisting backward to the right, reaching with EE's right hand/arm. Once EE had hold of tool EE realized it was hung on a piece of belting, EE started pulling and jerking to free up the tool when EE heard pop from shoulder and felt pain.
