Employee utilized leverage to free a stuck part when the applied force caused the tooling to come into contact with their face.

Employee alleges ee was aligning the pulleys and the bolts up and ee was pushing forward on the bearings with left arm and ee heard shoulder pop. Late Reporting due to employee just had surgery on 02/28/17 and ee is now losing time from work.

Employee alleges EE was tightening liner bolts and the wrench hit EE's gloves cutting EE's right palm. EE received stitches.

Employee alleges was swinging a hammer to bust up some rock and EE hurt left upper arm. The employee didn't loose anytime. EE had surgery on 06/13/2016 and EE started loosing time.

He was installing new hydraulic control levers on a loader and while reconnecting the electrical wiring had to push hard to connect wiring harness. He heard a pop in his left hand at the base of his index finger. His hand was swollen and hurting the next morning.

Employee alleges he was climbing up a ladder and his foot slipped and he hurt his shoulder.

Employee alleges he was reaching up to put a roller under the conveyor belt and he felt pain on the right side of his groin area. Causing a strain to his groin.

Employee alleges he was climbing down from haul truck and slipped on ice, landing on his left heel and buttocks, causing lower back pain. Employee just treated with chiropractor on 01-07-08.

Employee alleges he was walking down wet ramp and slipped on wet area hurting his left shoulder. Employee had surgery on 06-13-07 and started losing time as of that date.

Employee alleges he was backing up dump truck and he flipped it over on its side.

EMPLOYEE ALLEGES HE HAS BEEN OPERATING A PIT LOADER FOR 33 YEARS, THE STEERING WHEELS AND CONTROLS HAVE CAUSED PAIN AND NUMBNESS TO BOTH HANDS & WRISTS.

EMPLOYEE ALLEGES SOMETHING FLEW INTO HIS FACE AND EYES WHILE CARRYING TOOLS UPSTAIRS OF FINISHING PLANT.
