Employee alleges EE was climbing on conveyor and EE grabbed the rail and crushed EE's left pinky finger.

Employee alleges they were cutting with a cutting torch and the slag ignited solvent nearby causing second degree burns to their forearms and neck.

Employee alleges EE was cutting top of belt on a conveyor with a knife and the knife slipped and cut EE's left forearm/wrist.

Employee alleges was walking from scale house to employee's work truck and heard a pop in right foot. Employee was diagnosed with a fracture to the right foot.

Employee slipped on ice near service truck. Made an attempt to stop fall by reaching for the grab handle on service truck and heard a ""pop"" in shoulder as employee fell holding on to the handle.

Employee alleges EE was putting some rock breakers on and EE started feeling bad and EE passed out from heat exhaustion.

Employee alleges employee was getting ready to install a plate on a service truck and it fell and cut left pinky finger.

Employee was using a torch to make holes in the fog washer and a foreign object blew into EE''s right eye. The Dr. had to use a needle to move the foreign object out of EE's eye by the cornea. Late reporting due to communication error.

Employee alleges EE was removing a guard from a water pump the guard behind EE fell on EE's left foot. EE received a fracture to EE's foot.

Employee alleges they were grinding some material off of a table and a piece of metal blew into their left eye.

Employee was using cutting torch and a piece of hot metal slag fell down inside employee's boot causing a burn on employee's lower leg

Employee alleges EE was removing a rock on crusher and it slipped and cut EE's middle finger on EE's left hand

Employee alleges after feeling discomfort in abdomen area EE went to a physician. Dr. scheduled EE to see a surgeon. Employee recently had a hernia operation not job related. Surgery was scheduled for 08/31/20 and employee started losing time then.

Employee alleges they were removing a skid steer bucket pin and a finger got jammed in between the bucket and the tire. Employee smashed the left index finger and received 4 stitches.

Employee alleges was prying a rock from a gearbox and it hit employee in right eye and nose knocking off the safety glasses and causing right eye and nose to be swollen.

Employee alleges they were hitting the end of the rotar plate with a sledge hammer and they hit their left thumb. Employee has a fracture to the left thumb.

Employee alleges ee was replacing bolts and left thumb went under the bolts pinching ee's left thumb.

EE alleges EE was climbing up steps to get on a machine and EE twisted self and EE started feeling a sharp pain in EE's lower back.

Employee alleges was cutting a zip tie and the utility knife slipped and cut EE's left hand. EE received stitches.

Employee alleges EE was operating a loader and EE hit a hole in the roadway and EE jammed EE's neck and shoulders.

Employee alleges he was lifting and pulling rocks out of the crusher and some rocks fell on him hurting his neck, left shoulder and hip. Employee just started losing time on 7/28/14.

Employee alleges he was stepping off of a barge ramp with his left leg and he felt pain in his left knee and leg.

EMPLOYEE ALLEGES HE WAS REMOVING A SUCTION HOSE FROM BUCKET AND TRIPPED ON A ROCK AND FELL.

OPERATING 992G ENDLOADER FEEDING PRIMARY CRUSHER HE RAN OVER ROUGH TERRAIN, JAMMING HIS SHOULDER INTO HIS NECK CAUSING SHOULDER STRAIN.

EMPLOYEE ALLEGES HE WAS PULLING ELECTRIC CABLES AND HE FELT PAIN IN HIS LOWER BACK.

WALKING ON BUMPER OF 992 LOADER, TRIPPED AND FELL TO GROUND. BROKEN ARM, CONTUSIONS TO FACE.

EMPLOYEE ALLEGES HIS FOOT SLIPPED WHILE CARRYING SCREENS.

EMPLOYEE ALLEGES AS HE WENT TO FREE CLOGGED ROCK FROM THE CONVEYOR HE STEPPED UP ON THE CONVEYOR USING HIS HANDS ON THE FRAME OF THE CHUTE AND HE FELT PAIN IN HIS LEFT SHOULDER.

EMPLOYEE ALLEGES HE WAS GETTING OUT OF HIS TRUCK AND HE STEPPED ON A LADDER AND IT MOVED AND HE SLIPPED OFF OF THE LADDER.

EMPLOYEE ALLEGES HE WAS FEEDING THE IMPACTOR AND AS HE WAS TRYING TO KEEP UP WITH THE SECONDARY, HE HIT HOLES AND JAMMED HIS RIGHT SHOULDER.

EE ALLEGES HE WAS TAKING BOLTS LOOSE AND HE DROPPED THE BOLTS AND CUT HIS FINGER.

EMPLOYEE ALLEGES HE WAS ADJUSTING BELT ON CONVEYOR WHEN THE CLAMP CAME LOOSE AND HIT HIM IN THE FACE CAUSING HIM TO FALL BACKWARDS ONTO THE CONVEYOR.

EH EE ALLEGES HE WAS STEPPING OFF A SAMPLE PILE OF ROCK AND HIS FOOT SLIPPED CAUSING HIS KNEE TO TWIST AND TURN OUTWARDS.

EE WAS TIGHTENING BOLTS ON LINER AND THE WRENCH SLIPPED OFF OF BOLT & HIS ELBOW HIT HAMMER HEAD LAYING ON CHUTE.

EE ALLEGES HE WAS FUELING LOADER. HE THOUGHT IT WAS FINISHED. HE PULLED OUT THE PUMP AND IT WAS STILL RUNNING AND FUEL SPLASHED OUT IN HIS FACE.
