Employee alleges that EE was retrieving a tool from EE's truck toolbox and a strong gust of wind blew the toolbox lid shut, the lid hit a wrench causing it to fly up and cause abrasion to employee's upper lip

Cable tray slipped from hand and employee tried to catch it pulling on arm.

Employee alleges employee was using a hammer to fix a hub and the hammer slipped out of employee's hand cutting left thumb. Employee received 10 stitches.

Employee alleges they were in the process of breaking steel apart with a drill and they started feeling pain in the right shoulder. Employee started loosing time on 10/12/2018, but has returned to work on 10/16/2018.

Employee alleges EE was lighting a Furnace and it went out and came through the door burning EE's left forearm.

Employee alleges EE was using a torch to heat up a bolt and a foreign object blew into right eye.

Employee alleges was tightening a bolt with a ratchet and the ratchet slipped smashing right ring finger. Employee received stitches.

Employee alleges he was putting liners in a load hopper and it became too heavy and it tipped over on him. Causing a fracture to his left ankle.

Employee alleges he was getting out of the loader and he hit his right elbow on the pipe of the ladder he was climbing down from.

Employee alleges he was cleaning out a grizzly and he reached down to remove a small rock and then a big rock fell over and smashed his finger between the grizzly and the rock causing a laceration to his right index finger. He received 6 stitches.

Employee placed a 4x4 underneath a portable pump to be able to pick the pump up with a forklift. He signaled the operator to raise the forks. The 4x4 snapped pinching the end of his little finger causing a small fracture.

Employee alleges he was loading trucks and his hands started going numb. Employee was just diagnosed with carpal tunnel and he had surgery on 04-17-2012 and is now losing time from work.

Employee alleges he was loading stock pile signs on the truck and his upper back starting hurting.

Employee alleges he was removing bearings and he hit his left knee with the hammer handle causing a laceration to his left knee.

Employee alleges he was lifting a wheel hub and he cut his arm on a sharp metal object receiving stitches.

Employee alleges he was using a torch to cut steel and it went down into his glove causing 2nd degree burn to his left hand and wrist.

Employee alleges his ankle got caught between two steel beams and he fell.

Employee alleges he was changing screens and he stepped on chute and lost his footing and slipped on chute door cutting his leg. He received stitches.

Employee alleges he was drilling holes in steel plate and he heard something pop down by his belt.

Employee alleges he has been doing a repetitive motion operating a loader and he is starting to feel pain in both wrist and his fingers. Employee had surgery on 06-14-07 and started loosing time. Late report due to insurance company was investigating and was just notified by the insurance company.

Employee alleges he was drilling holes in beam and his glove got caught in drill, causing him to receive a broken left thumb.

Employee alleges he was pulling fuel hose down hill and a rock rolled down and landed on his right foot fracturing his little toe.

EMPLOYEE ACCIDENT WAS DUE TO CARPAL TUNNEL RESULTING FROM INJURIES HE HAD ON HIS FINGER.

Employee alleges he was lining a piece of steel in the saw as he closed the clamping vice the end of his fingers got caught in the vice, smashing his fingers.

EE ALLEGES HE WAS HOLDING A PUNCH FOR A CO-WORKER AND THE CO-WORKER SWUNG THE HAMMER AND MISSED THE PUNCH AND HIT HIS LITTLE PINKY FINGER ON HIS RIGHT HAND.

CONTRACTED BILATERAL CARPAL TUNNEL SYNDROME FROM 15 YEARS OF TYPING AND USING THE COMPUTER MOUSE AND THE LIKE. SURGERY WAS REQUIRED AND WAS COMPLETED ON 9-19-03.

EE ALLEGES HE WAS PULLING BROKEN DRILL STEEL OUT OF MACHINE AND HE DROPPED IT ON HIS RIGHT FOOT.

EE ALLEGES WHILE REMOVING EXISTING SPACERS ON SIDE OF IMPACT CRUSHER, THE SET OF SPACERS SLIPPED OUT OF HIS HANDS SMASHING HIS THUMB.

EE ALLEGES AFTER GRINDING OPERATION, HE TOOK HIS SLEEVE ON HIS JACKET AND WIPED HIS EYEBROW AND HE FELT SOMETHING IN HIS EYE.

EMPLOYEE ALLEGES HE WAS PUTTING CUTTING EDGE BACK ON DOZER AND AS HE WAS HITTING DOZER WITH A HAMMER A PIECE BROKE OFF AND WENT INTO HIS RIGHT LEG.

EE ALLEGES HE WAS HELPING DRILLER INSTALL 3 RODS AND COUPLINGS ON ROCK DRILL. THE OTHER EE MOVED FEED CONTROL LEVER AND CAUGHT INJURED'S HAND BETWEEN THE COUPLING AND THE ROD ARMS.

EMPLOYEE ALLEGES HE WAS LIFTING BLOCKS TO PUT UNDER PONTOONS, THE CHAIN CAME LOOSE AND HIT HIM ON HIS LEFT HAND.

EE ALLEGES HE WAS CLEANING OFF SAAND FROM SANDBLASTING WHEN HE PUT WEIGHT ON BLOCKS THEY FELL OVER AND HE FELL TO THE GROUND.
