Employee was exiting haul truck and missed the last step, causing EE to fall.

While doing some service work to a skid steer, employee was reaching into tight area of the machine to remove a hose and EE felt a pop in the front side of their right shoulder.

IE was operating a dozer when the blade caught a buried pinnacle and jerked the machine, causing them to cut their head on an overhead vent. IE was wearing a seatbelt.

During offsite Annual Refresher meeting, IW passed out during instruction. Was immediately taken via ambulance to hospital. Later released the same day. Low potassium and dehydration were the diagnoses. IW returned to work with no lost time.

Fall from machine

Iw was on discharge conveyor to jump start a diesel engine when the engine started and the conveyor unexpectedly started causing fall underneath of magnet and then fell about 7 feet onto a feeder pan. Plant was not processing material and no material was in the plant. Iw experienced compression fracture to lower back area vertebrae.

Struck by falling object

Company Closed 11/23-24 for holiday. Injury Reported 11/16 but IE refused medical attention until 11/17. While working on removing skidloader bucket, bucket fell and struck IE right knee causing bruising. IE immediately released to restricted light duty. Due to nature of job, no light duty work available under given restrictions.

Over-exertion in lifting objects

Closed 7/3-4 for holiday. Injury reported to mgmt on 06/29/2017. Left knee pain from lifting Telsmith Jack(~70lb) on 6/23. IE sent to USH to be treated. IE immediately released to light duty. Restrictions of NO: lifting >10lb, climbing stairs/ladders, squatting/kneeling, driving mobile equipment; limit walking 15min/hr in 8hr day. Due to nature of job, no light duty work available.

Struck by flying object

Employee was emptying 4"" stone from the bin into the stock truck. A rock bounced off the load striking the employee on the left middle finger lacerating it.

The employee was feeding a portable impactor crusher with an excavator. A liner plate broke free inside the crusher and was shattered by the rotor. A small piece was ejected through the chain and rubber curtain at the feed opening and through the windshield of the excavator striking the employee's left hand.

EE was identified as having a Standard Threshold Shift through our annual hearing test on 10/14/2014. After retest on 11/10/2014, the shift was confirmed by Audiologist on 11/11/2014. EE is a mechanic exposed to loud noises from power tools and engines. EE also reported exposure to loud noises away from work & indicated he wears hearing protection most of the time.

The employee was helping to replace the feed chute to a crusher. He was standing on top of the crusher (approx. one foot above the floor surrounding the crusher) when his foot slipped and he fell, striking his knee.
