Employee was exiting haul truck and missed the last step, causing EE to fall.

While doing some service work to a skid steer, employee was reaching into tight area of the machine to remove a hose and EE felt a pop in the front side of their right shoulder.

IE was operating a dozer when the blade caught a buried pinnacle and jerked the machine, causing them to cut their head on an overhead vent. IE was wearing a seatbelt.

During offsite Annual Refresher meeting, IW passed out during instruction. Was immediately taken via ambulance to hospital. Later released the same day. Low potassium and dehydration were the diagnoses. IW returned to work with no lost time.

Iw was on discharge conveyor to jump start a diesel engine when the engine started and the conveyor unexpectedly started causing fall underneath of magnet and then fell about 7 feet onto a feeder pan. Plant was not processing material and no material was in the plant. Iw experienced compression fracture to lower back area vertebrae.

Company Closed 11/23-24 for holiday. Injury Reported 11/16 but IE refused medical attention until 11/17. While working on removing skidloader bucket, bucket fell and struck IE right knee causing bruising. IE immediately released to restricted light duty. Due to nature of job, no light duty work available under given restrictions.

Closed 7/3-4 for holiday. Injury reported to mgmt on 06/29/2017. Left knee pain from lifting Telsmith Jack(~70lb) on 6/23. IE sent to USH to be treated. IE immediately released to light duty. Restrictions of NO: lifting >10lb, climbing stairs/ladders, squatting/kneeling, driving mobile equipment; limit walking 15min/hr in 8hr day. Due to nature of job, no light duty work available.

Employee was emptying 4"" stone from the bin into the stock truck. A rock bounced off the load striking the employee on the left middle finger lacerating it.

The employee was feeding a portable impactor crusher with an excavator. A liner plate broke free inside the crusher and was shattered by the rotor. A small piece was ejected through the chain and rubber curtain at the feed opening and through the windshield of the excavator striking the employee's left hand.

The employee was dismounting a front end loader. He missed the bottom step, and when he caught himself, he strained his right wrist.

EE was identified as having a Standard Threshold Shift through our annual hearing test on 10/14/2014. After retest on 11/10/2014, the shift was confirmed by Audiologist on 11/11/2014. EE is a mechanic exposed to loud noises from power tools and engines. EE also reported exposure to loud noises away from work & indicated he wears hearing protection most of the time.

The EE was fueling a front end loader. Upon dismounting the machine, he stepped on the fuel pump hose, and twisted his ankle.

The employee was helping to replace the feed chute to a crusher. He was standing on top of the crusher (approx. one foot above the floor surrounding the crusher) when his foot slipped and he fell, striking his knee.

Employees assigned job is emptying product bins with a Euclid dump truck. The truck is powered by a DC treit diesel engine. Mild to moderate high frequency hearing loss.

Employee was getting off truck, stepped onto uneven ground surface and twisted his left knee.

Employee was assisting the welder by handing tools and assisting in the placement of steel.

EMPLOYEE REACHED UNDER CONVEYOR TO REMOVE BOARDS TO REPAIR CONVEYOR #1. EMPLOYEE STOOD UP AND STRUCK FOREHEAD ON ELECTRICAL CONDUIT SUPPORT.

EE HAD INJURED HIS KNEE FALLING ON ICE AT HOME THIS WINTER. THE KNEE CONTINUES TO BOTHER HIM, BUT HE WAS PERMITTED TO RETURN TO WORK. HE REQUESTED OF HIS SUPERVISOR TO TAKE HIM HOME AS HIS KNEE WAS ACHING. WHEN HE STEPPED UP INTO SUPERVISOR'S PICK-UP HIS KNEE BUCKLED UNDER HIM.

EMPLOYEE WAS SHOVELING DIRT/ STONE FROM UNDER 2A CONVEYER. STRAIN TO LOWER BACK.

EMPLOYEE DID NOT LISTEN TO EXPERIENCED MINER, WENT AHEAD AND TRIED TO PULL TIRE OFF HUB OF CAT 769C HAUL TRUCK.

EMPLOYEE STEPPED DOWN FROM CATWALK TO SCREEN DECK. TURNED ANKLE.

UNDER CAT 973 LOADER SERVICING EQUIPMENT. WHEN EMPLOYEE CAME OUT FROM UNDER EQUIPMENT AND STOOD, HE TWISTED LOWER BACK.

EMPLOYEE WAS SHOVELING STONE FROM UNDER CRUSHER.

CUTTING OFF FRAYED CRANE CABLE WITH A TORCH. CABLE UNRAVELED, AND ONE STRAND CAUGHT EE IN EYE.

CLEANING OUT A 2A AGGREGATE BIN WITH A HAMMER AND CROWBAR WHEN MATERIAL BROKE LOOSE AND CAUGHT EMPLOYEE'S RIGHT HAND BETWEEN THE BIN AND THE STONE.

SHOP AREA, EMPLOYEE WORKING ON ENGINE REPLACEMENT. ASSISTED BY ANOTHER EMPLOYEE. DROPPED A CROSS MEMBER ON FOOT ABOVE HIS STEEL TOED SHOE. VEHICLE A CAT 769C HAUL TRUCK.

PRYING SHOE PIN (SPOKANE CRUSHER) WITH PRY-BAR. BAR SLIPPED AND SMASHED FINGER BETWEEN BAR AND ANVIL.

DRIVER CLOSED TRUCK DOOR, CAUGHT END OF MIDDLE FINGER IN DOOR.

EE WAS HAMMERING A BEARING INTO PLACE ON TRIPLE-SCREEN. HAMMER SLIPPED, HIT HIS HAND.

EMPLOYEE WAS IN A SQUATTING POSITION TO REPLACE A PUMP IN PIT. TASK COMPLETED, WHEN HE AROSE TO STAND, LEFT KNEE MADE A POPPING NOISE.

EMPLOYEE USING SHOP HAND GRINDER, AND ALTHOUGH WEARING SAFETY GLASSES, MATERIAL WENTINTO EYE, RESULTING IN IRRITATION (NO EMBEDMENT NOR ABRASION) THOUGH DR PLACED EMPLOYEE ON PRESCRIPTION STRE NGTH EYE DROPS FOR 3 DAYS.

EE WAS LOADING TRUCKS WITH ROCK TO SEND TO CRUSHER WITH AN LS 3400 LINK BEL BACKHOE. DROPPED ROCK ONT DUMP BED; A SMALL STONE FLEW BACK AND HIT OPERATOR IN EYE.

EE WAS ASSISTING WELDER BY LIFTING AND CLAMPING PLATES IN THE PRIMARY CRUSHER FEEDER. WHEN WELDER STOPPED TO CHANGE RODS, EE GLANCED IN DIRECTION OF WELDER WORK WOULD BEGIN AGAIN.
