WAS HIT ON SIDE OF HEAD WITH ROCK FROM CONVEYER.

EMPLOYEE WAS WELDING, DROPPED A CHUNK OF METAL ON HIS FOOT, AND BROKE TWO BONES IN HIS FOOT.
