Struck by flying object

While completing maintenance on conveyor a piece of metal flew off hitting EE in the arm. EE fell and injured left arm and shoulder

Fall from ladders

Employee was performing repairs on crusher moving from a ladder to the crusher. EE slipped and fell on back. Possible bone fractures in back.

The crew was working on the crusher - employee was on a platform and slipped onto a belt below and hurt knee. It was first thing in the morning, with a layer of ""dew"" on the platform.
