Was changing cone liners on crusher. Had fingers in wrong spot. Crush injury to fingers between two pieces of metal.

Employee was doing repair on crusher - was swinging sledgehammer when he missed his target and hit his finger against metal surface.

Employee was adjusting scraper on mud machine with a wrench - his hand slipped off the wrench and his elbow hit metal on the catwalk.

Employee was changing a bearing; was holding chisel in right hand when thumb got hit between the hammer and bearing.

During snowstorm, employee was walking across yard to get a shovel, slipped and fell on his back.

EE was climbing down on the ladder from his rock truck and lost his footing and fell to the ground.

Employee filled sandblasting machine and was bringing it back to shop when he slipped and fell. The machine landed on his foot.

Employee was in generator bldg changing an air filter and some debris got in his eye.

Employee was cleaning scale when he started to feel dizzy and light headed. His supervisor noticed his color was bad, and called an ambulance. He suffered a heart attack.

Employee stepped down off a ladder onto a loose rock and twisted ankle.

Employee was moving a concrete block when he pinched his finger against the block and a piece of steel, lacerating finger tip and nail.

Employee was standing on a ladder to move a hopper that was being placed on the crusher, and felt his left knee give out.

HE WAS MOWING LAWN AND COLLAPSED.

WAS HIT ON SIDE OF HEAD WITH ROCK FROM CONVEYER.
