Employees were removing a pipe from a classifier to the screen plant. The pipe, once unbolted, rolled off of support. Employee tried to hold pipe in place and got fingers pinched between pipe and frame.

Making preparations to repair hoist cable anchor end on dredge digger, when a support cable broke, causing the digger to fall. Which in turn, caused the EE to fall, striking his face and head.
Employee was on a trailer with muddy boots and slipped and fell, jarring his back.

Employee was on the 3rd step of a 6' ladder greasing pulleys on conveyor. The ground was soft and uneven which caused the ladder to shift. Employee fell to the ground landing on his left hip and lower rib area of his back.
Employee was lifting pipe to set a water pump when he felt pain in his groin area.
Cutting nylon rope with pocket knife, slipped and cut arm.
Employee was getting a section of pipe & catwalk together on the water. A gust of wind caught the section which was being added, ee's hand was caught in between the guard rails.
Employee was pulling clay off of conveyor. Pulled off conveyor by a large piece of clay and caught leg on conveyor frame.
EMPLOYEE WAS CLEANING AXLE HOUSING WITH BRAKE CLEANER AND LOOKED INSIDE THE HOUSING WITH A HALOGEN LIGHT. THE HEAT OF THE LIGHT IGNITED THE VAPORS, CAUSING A FLASH FIRE (BURNS TO FACE & NECK).
As dozer crested the top of travel way ramp the center of gravity pulled the front of the dozer down. Employee was thrown forward into the cage of the dozer from the seatbelt not being buckled correctly.
EMPLOYEE WAS ENTERING HIS TRUCK AS PART OF HIS NORMAL JOB DUTIES & FELT SEVERE PAIN IN THE BACK OF HIS RIGHT CALF.
Employee has complaints of carpal tunnel syndrome. She has worked in the scale house writing scale tickets by hand for 30+ years. Employee now uses a computer to complete tickets. Hand and wrist is bothering the employee.
A rock was wedged in the primary crusher. Employee was removing it with a bar. The rod was grabbed and employee's hand was slammed against the corner of the flopper, resulting in a lacerated finger.
Employee was folding a stacker when the pry bar slipped and crushed his thumb between the conveyer frame.
Employee was tightening chain on the fuel tank he was hauling. The trailer was wet causing him to slip. He caught himself but wrenched/strained his left knee while doing so.
Carpal Tunnel Syndrome to both left and right hands/wrists. Repetitive use of hands operating loader levers and writing out scale tickets. Still working, scheduled for surgery at a later date.
MINER WAS HELPING TO SET UP WASH PLANT WHEN HIS HAND CAME IN CONTACT WITH THE END OF A PIPE LACERATING HIS 1ST FINGER ON THE LEFT HAND REQUIRING STITCHES.
EMPLOYEE THOUGHT HE WAS HAVING CARPAL TUNNEL PROBLEMS. DR. HAS DIAGNOSED WITH A HERNIATED DISC IN HIS BACK CAUSING PAIN IN ARM & NECK. EE STATES THAT ON FEB. 13, 2004, HE WAS OPERATING THE FRONT END LOADER, HIT A BUMP & HIS HEAD HIT THE ROOF OF THE CAB. EMPLOYEE FEELS THIS CAUSED HERNIATED DISC.
EMPLOYEE WAS WALKING ACROSS THE PLANT YARD, SLIPPED ON ICE AND FELL, CAUSING A LOWER BACK MUSCLE STRAIN.
EE WAS PREPARING TO MOVE CONVEYOR AND WHEN HE CLIMBED OFF THE CONVEYOR HE CUT HIS THUMB ON A SHARP METAL EDGE. HE RECEIVED FOUR STITCHES TO THE RIGHT HAND THUMB.
EMPLOYEE WAS STANDING AT SUPERVISOR'S PICKUP TALKING. WHEN FINISHED SUPERVISOR SHUT PICKUP DOOR SMASHING EE'S RIGHT THUMB IN DOOR.
EE WAS USING A PRYBAR ON A SAND FLUME WHEN THE BAR SLIPPED AND PINNED HIS FINGER BETWEEN THE PRYBAR AND THE FLUME. HE RECEIVED MULTIPLE FRACTURES TO THE MIDDLE FINGER ON HIS RIGHT HAND ABOVE T HE FIRST KNUCKLE.
EE WAS HAMMERING ON A BEARING WITH A SLEDGEHAMMER, MISSED THE BEARING, GETTING FINGER PINCHED BETWEEN THE SLEDGE HAMMER AND THE BEARING.
DESCENDING LADDER FROM PLANT, HIS FEET SLIPPED. HE GRABBED LADDER RUNG WITH LEFT HAND. THE JERKING MOTION CAUSED A TEAR IN LEFT ROTATOR CUFF, DIAGNOSED ON 6/21/02.
WHILE UTILIZING A 3/4" RATCHET CUM-A-LONG TO MOVE A 12" DIAMETER, 15' LONG PIPE, EE INCURRED AN UMBILICAL HERNIATION. THE INJURY WAS REALIZED THE FOLLOWING DAY. WAS REPORTED THE FOLLOWING WORK DAYS 11-26-01. EE HAS NOT LOST TIME YET BUT IS DUE TO HAVE A SURGICAL PROCEDURE ON 12-6-01 THAT MAY RESULT IN LOST TIME FROM WORK.
WHILE PUSHING MUD DOWN A CHUTE WITH A SCRAPER, EE STRUCK FINGER ON A PIECE OF STEEL PROTRUDING FROM CHUTE CAUSING MINOR LACERATION TO FIFTH FINGER ON LEFT HAND WHICH REQUIRE BANDAGING BY DR AN D FOLLOW UP VISIT THE FOLLOWING MORNING.
MOVING 50' TRANSFER CONVER WITH A LOADER, BOTTOM OF CONVEYOR RESTING ON LIP OF LOADER BUCKET, EMPLOYEE UNHOOKED CHAINS HOLDING CONVEYOR TO BUCKET WHEN IT STOPPED. CONVEYOR SLIPPED OFF OF BUCKE T AND FELL TO GROUND LANDING ON EMPLOYEE'S LEFT FOOT FRACTURING IT IN TWO PLACES.
WHILE MOVING ROCK AROUND (ROUGHLY 12" DIAMETER) WITH SKID LOADER, ONE ROCK ROLLED BACK INTO CAB STRIKING LEFT LEG CAUSING LACERATION TO UPPER SHIN ON LEFT REQUIRING STITCHES.
CLAIMANT WAS HOLDING A PUNCH SO ANOTHER EE COULD STRIKE IT WITH A SLEDGE HAMMER TO KNOCK OUT TRACK PINS ON A BACKHOE. EMPLOYEE MISSED PIN WITH SLEDGE AND STRUCK CLAIMANTS RIGHT WRIST, CAUSING A FRACTURE.
EE WAS GRABBING FUEL NOZZLE TO FUEL UP DJB. HE WAS STANDING ON ICE, SLIPPED AND STUCK OUT LEFT ARM TO BRACE FALL. HE STRAINED LEFT SHOULDER IN FALL.
EE WAS PREPARING TO USE CUTTING TORCH, THE TORCH FLASHED BACK AND ACETYLENE HOSE BLEW UP CAUSING BURNS TO FACE.
THE CREW HAD BEEN CLEANING THE GENERATOR THAT SUPPLIES THE PLANT; WHEN THEY STARTED IT, IT SHORTED OUT CAUSING SMALL BLOWUP. EMPLOYEE WAS STANDING IN BACK END OF GENERATOR TRAILER WHEN THIS O CCURRED, AND IT CAUSED HIM TO LOSE HIS BALANCE AND FALL OUT OF TRAILER, BREAKING LEFT ANKLE.
REPAIRING AN AIR BRAKE LINE, WHILE CUTTING LINE WITH KNIFE, EMPLOYEE SLIPPED AND CUT LEFT THUMB, REQUIRING SIX STITCHES.
