Caught in, under or between a moving and a stationary object

Supervisor was in the loader picking up on the fifth wheel base to take pressure off of the shaft to remove the tail pulley stand base. Employee figured they could use the 1/4 bolt on the one side of the shaft for leverage to remove the shaft from the base. As they picked up with the loader to remove the pressure it smashed employee's finger between the bolt head and stand base.

Employee lifted a 3 gal. jug of oil up onto loader steps & strained his shoulder.

Employee has complaints of carpal tunnel syndrome. She has worked in the scale house writing scale tickets by hand for 30+ years. Employee now uses a computer to complete tickets. Hand and wrist is bothering the employee.

Carpal Tunnel Syndrome to both left and right hands/wrists. Repetitive use of hands operating loader levers and writing out scale tickets. Still working, scheduled for surgery at a later date.

Struck against a moving object

MINER WAS HELPING TO SET UP WASH PLANT WHEN HIS HAND CAME IN CONTACT WITH THE END OF A PIPE LACERATING HIS 1ST FINGER ON THE LEFT HAND REQUIRING STITCHES.
