Employee was working in open conveyor area at the time. Was welding 2 brackets on airline but was not wearing respirator under welders mask. Injury is still undetermined. After investigating work area, the injury is still undetermined, and if other safety measures are required we will investigate further and take proper steps to correct.
Employee was coming out of the bathroom and stepped wrong on steps and sprained ankle. Note: At first it was thought to be sprained, several days later learned it was fractured.

WENT TO SEE WHY #3 CONVEYOR HAD STOPPED. THE CONVEYOR WAS NOT RUNNING AND APPARENTLY V-BELT WAS SLIPPING ON PULLEY. HE PUT LEFT HAND ON V-BELT AND IT PULLED HIS LEFT HAND INTO PULLEY CAUSING INJURY.

CLIMB LADDER TO CHECK HOW MUCH MATERIAL WAS IN DUMP TRUCK. WHEN EMPLOYEE STEPPED DOWN HE MOMENTARILY SLIPPED. HE REACHED UP WITH RIGHT HAND AND GRABBED A HOLD OF GUARD WHICH CAUSED LACERATION TO THE PALM OF RIGHT HAND BELOW THUMB.

SUBJECT WAS HAND SELECTING BLDG STONE. HE BENT DOWN TO PICK UP PIECE OF ROCK. HE SAYS THAT HE HIT HIS RIGHT WRIST AGAINST SHARP CORNER OF ROCK. IT PUNCTURED SKIN & CAUSED HIS HAND TO SWELL.
