Employee slipped on muddy surface near crusher after rainfall, injuring left wrist. Walked to break room, notified coworker, and was taken to Hospital. Treated and released same day; prescribed pain medication and muscle relaxer, making this a recordable injury.

Operator tripped over uneven concrete just before the time clock, and landed on knees and hands.

2 employees were removing a set of large bolts with a 3/4" impact and a backup wrench. The employee holding the backup allowed a hand to slip and had left index finger smashed against the equipment, resulting in 6 stiches. Medical care was handled quickly and without delay.
Description of how the injury occurred: While using the left hand the operator was swinging the dead blow hammer from right to left, backwards, and across EE's chest. This resulted in a glancing blow which allowed for the wrist to be hyperextended. This resulted in modified duty, and is a recordable injury.

Mechanic was using a 14oz. ball peen hammer and a punch to remove 3 stripped bolts. Employee used the hammer and punch off and on for approximately 1 hour before the bolts were removed.

Employee was coming down from a step ladder, and struck his elbow on the lip of a tank next to the ladder. Symptoms from the injury did not appear until approximately 48 hours. Diagnosis was soft tissue contusion with no fracture. Prescriptions were written for pain and swelling and the employee was placed on restricted duty.

The employee slipped on a wet floor, landing on his shoulder and back. The employee experienced soreness in his shoulder and upper back. Medical evaluation only on 7/13. Released for work with restrictions 7/16. Restrictions lifted 7/18. 1 day restricted duty, 7/18. Not scheduled to work 7/14-17. Prescription for pain meds not filled.

Employee received a laceration to his right hand while driving an electrical grounding rod into the ground with a ground rod driver tool. The employee received 5 stitches in his right hand at a local emergency room.

Employees foot slipped while pulling up on a centrifuge to disassemble for cleaning. Foot hit the toeboard, jarring his right lower back/hip. Reported to his supervisor 2/7 as "report only". On 2/8, the employee reported back pain, so we took him to the doctor for evaluation. No drugs prescribed. Dr. recommended light duty on 2/8/07. Released to return to full duty 2/13/07.

EE WAS FEEDING BACK LOTS OF EW & DUST COLLECTOR WAS NOT WORKING. PARTICLES OF EW DUST GOT INTO HIS EYES WHILE EW WAS BEING FED INTO BAG DUMP. HESTATED HIS EYES FELT SWOLLEN & HAD DIRT IN THEM.

EMPLOYEE WAS PACKING 50 LB BAGS OF PRODUCT. THE LOT WAS SET INCORRECTLY, SHE BELIEVES SHE HURT HER SHOULDER WHILE LIFTING THE BAGS ON TO THE TOP OF THE PALLET.
WHILE EE WAS CLEANING THE #4 ROLLER ON THE EAST SIDE IN THE PLANT FACILITY, HE SLIPPED OFF THE CLEANING PLATFORM. HE INDICATED THE PLATFORM TETERS WITH THE HANDRAILS INSTALLED. HE FELL AND SCR ATCHED HIS CHIN, HURT BOTH WRISTS AND HAD PAIN IN THE CHEST AREA ALSO.
EMPLOYEE ALLEGES THAT HE WAS PACKING MATERIAL AND FELT A PULL IN HIS BACK WHEN TURNING TO PUT THE MATERIAL (BAG) ONTO A PALLET. HE SAID THAT HE THOUGHT IT WAS NOTHING, BUT HAS BEEN GETTING PR OGRESSIVELY WORSE.
