Employee was cleaning conveyor close to the head pulley and snubber idler. EE failed to follow Lock Out / Tag Out procedures and inadvertently got hand caught between the head pulley and snubber.

EMPLOYEE WAS CARRYING 90LB BAGS OF SAND AND FELT SPASMS IN LOWER BACK.
A MANHOLE COVER ON TRUCK SCALE HAD A CORRODED SUPPORT BLOCK WHICH GAVE AS EE WALKED ON IT. EMPLOYEE FELL, HIT HIS KNEE AND SCRAPED HIS SHINS AND CAUGHT HIMSELF WITH HIS HANDS. THIS RESULTED IN MINOR CONTUSTIONS/SCRAPES. DR. PRESCRIBED CELEBREX & VICODIN AND EMPLOYEE WAS PUT ON MODIFIED DUTY. FOLLOW UP ON 7/19/2004, DR ORDERED EE OFF FROM WORK.
MINER REPORTED PAIN IN KNEES DUE TO YEARS OF REPETITIVE MOTION, WALKING UP STAIRS AND LADDERS, ETC. DOCTOR'S EVALUATION CALLED FOR ORTHROSCOPE SURGERY IN BOTH KNEES TO REPAIR DAMAGE FROM REPETIVE MOTION STRESS ON KNEE JOINTS/CARTILEDGE.

EMPLOYEE WAS WALKING UP STEPS TO PACK HOUSE AND HE TRIPPED ON TOP STEP, LANDING ON HIS RIGHT HAND/WRIST. THE STEPS, WHICH WERE NOT UNIFORMLY SPACED, WERE FIXED IMMEDIATELY.
SORTING PALLETS. PALLET SLIPPED AND HIT EMPLOYEE ON HIS LEFT KNEE.
EE WAS CLIMBING UP AND DOWN THE LADDER ON THE LOADER AND WHILE OPERATING THE LOADER REPETITIVE BOUNCING WHEN HIS BACK STARTED TO HURT.
