While employee was walking to haul truck for start of shift, EE slipped and fell injury EE's knee and ribs.

The technician was using a 24mm wrench and a 4lbs hammer trying to loosen the bolts on the hose half clamps. The technician missed the wrench striking EE's left hand between EE's thumb and index finger the technician was not wearing gloves. Technician was sent to Innercare and received stitches.

While removing G.E.T. (Ground Engaging Tools) from the loader bucket with a hammer, a piece of metal shrapnel broke off and became lodged in a maintenance technician's thigh.

While performing track wrapping on the #402 Shovel, a piece of metal sheared off from the roller assembly when it contacted the track chain. The fragment struck the maintenance technician on the forearm, causing a 2-inch laceration. The metal was removed from the arm and the employee was transported to a local medical facility to close the wound with stitches.

Employee smashed hand under piston shaft they were removing. They received stitches and restricted duty.

Employee was opening boxes to receive them. EE pulled one of the boxes from the cart at the bottom. As EE was trying to get it out from the bottom the box on top fell over and hit EE in the head. The employee complained of a headache about an hour later but said other than that felt fine. On 4/25/2024 EE wanted to be checked out, so Safety took EE to the clinic.

Mechanic needed help on the new CMV strip tank, so the employee offered to help, as they were setting the 3'' stainless steel pipe in place to bolt it down, the pipe moved and got the operator's left thumb in a pinch point and smashed it between the 3'' stainless steel pipe that they were setting in place and an existing pipe that was there on the way.

At 10:00PM on 7/15/2023 employee was helping install a transmission oil cooler line on the haul truck at the fuel island. Employee climbed up on the right side of the truck between the engine and the frame to help align the hose. EE was in an awkward position when EE felt a sharp pain on the right lower back. EE felt sore due to this incident.

Employee was grabbing EE's gear out of the car to start EE's shift, when EE stepped on a rock and rolled EE's ankle. The pain was intolerable and wanted to go to the ER. Safety Coordinator met EE there where they took x-rays and it showed EE had a fractured ankle on right leg.

Leadman was grabbing a tow strap out of the back of the shifter truck when it had caught on something as the employee turned around and felt small pain in right shoulder. Didn't think anything of it but went to the clinic later on to get checked out. On August 4th, 2022 employee was prescribed pain meds. So it went from a First Aid to a Medical Treatment.

Operator got off the blade 604 for lunch. When getting back up on the blade EE felt pain in EE's back. Got checked out by MERT and was ok. Next day MERT checked EE out just to make sure and was still ok. So there was nothing to report. On July 11th, 2022, employee went to the clinic on EE's own was prescribed meds and put on restricted duty.

Employee was going up the 401 shovel ladder when EE heard a pop in EE's right knee. *Reason this is was not entered sooner is because at first EE was released to go back to work but now EE's condition has gotten worse so now EE is on restricted duty.

At approximately 8:40 am, Monday August 9th, 2021, a 789 haul truck (Unit#522) approached the PAD 4 ROM rock stockpile used to feed the Pad 4 crusher. While positioning the haul truck to dump in reverse, the haul truck operator stopped at the dump edge preparing to dump when the dump floor gave away causing the haul truck to tip.

Lube technician lost footing while dismounting a D6 dozer.

Employee was finishing up grinding some brackets for a project when EE felt some debris go into EE's left eye. EE tried flushing it but didn't help so EE came to safety to have it flushed as well but was still irritating EE. Safety took EE to the clinic.

Employee was unloading used G.E.T from the rear of the bumper of 735 service truck, and smashed and cut EE's left index finger between the G.E.T and the edge of the metal bin. MERT members cleaned the wound and wrapped EE's finger, they determined to have a Medical Professional look at EE's wound. Employee ended up receiving stitches.

While sanitizing hands in truck, the wind caused the door to close quicker than anticipated. Index finger got caught and caused laceration. Had to get stitches.

While changing/flipping cutting edges on dozer 605, employee was adjusting cutting edges with prybars on floor. While attempting to turn, prybar slipped and cutting edge dropped down catching finger tip between cutting edge and cement. Was taken into hospital and received stitches.

Contractor was preparing to make a cut on a pipe, as contractor picked up the saw with right hand contractor's left pinky made contact with the de-energized blade.

Two employees were handling a filter press plate when one employee lost grip and the plate injured the right index finger of the other employee.

Employee was installing a radio antenna on a haul truck when employee began to feel symptoms of heat illness.

Employee exhibited signs of heat exhaustion while welding.

Driver entered cab of semi-truck to retrieve nuts and bolts for a repair. Driver lost grip and fell backwards from the second step landing on the concrete below.

Changing hose on pump after waiting for engine to cool. Hot oil splashed on right forearm, neck and down back.

Employee was pulling drip lines from the rail on the kabota and the lines got stuck which caused a sudden stop. It pulled EE's left shoulder back and immediately caused pain in the left shoulder. Was taken to clinic to be evaluated.

Employee felt back pain while putting on boots at the start of shift.

Slipped off of the running board of light vehicle. Grabbed the steering wheel to break fall and hurt shoulder. Was seen and has a right shoulder strain, was prescribed muscle spasm medication.

While repositioning a strobe light on a light vehicle, employee missed step on the running board and awkwardly put weight on left knee feeling a pop and pain. Employee reported the incident to supervisor the following shift. Employee was seen by the safety group and referred to the clinic for further evaluation. Was placed in a knee brace.

Employee emptied the mop bucket into the sink when EE felt a sharp pain in lower back. It continued to move up EE's spine.

Employee was welding on HT 515 when a foreign object landed in their eye. EE was brought to the Safety Department and tried to flush it out. The foreign object was still imbedded in their eye. Took them to Clinicas and the object was imbedded 70% in their eye.

Started on the dozer but moved to the blade for a specific project. When done, as EE was getting off right knee buckled. Shortly after it started swelling, so EE sought medical attention from Safety.

The employee picked up an 8-inch pipe causing sharp pain in right elbow.

The operator on 14M road grader, while doing some floor work in BC6 pit, struck a partially embedded rock causing a straining lower back.

While filling the drill with water, water hose blew out from top of the tank and as the operator reached over to grab the hose, ee slipped causing the right knee to twist causing a sharp pain.

At the beginning of the shift, the employee was walking performing a pre-inspection on HT 517 at the fuel island. As employee was walking, the employee lost footing and fell on left hand.

While working on shovel, employee got foreign object/ debris in left eye that was flushed out

Haul truck operator was dumping load, bed slammed down jarring employee causing back pain

EE was working on 16M motor grader. EE climbed out from under the machine and stood up. EE's right foot rolled off a rock under soft dirt, causing knee to pop. Pain continued next day, then was reported to supervisor.

Employee received lumbar strain while operating haul truck.

Employee made a quick turn of EE's body causing back strain.

Temporary contracted employee slipped on the second step while dismounting water wagon causing back sprain.

Shrapnel entered employee's chest while working on a loader.

Dehydration, High Heat (121 Degrees) and physical exertion.

Dozer operator sustained back pain while conducting pre-shift inspection.

While operating haul truck, operator received lumbar strain.

Employee felt pain in left wrist while operating shovel.

Employee was using a Stanley knife cutting materials and cut his 3 fingers on the left hand. (Pointer, Middle and Ring). Employee was taken to the hospital and got 13 total stitches.

Maintenance welder sustained chemical burn to lower body.

Employee received lower extremity pain after tripping down 2-3 stair steps.

Contracted temporary employee sprained back while lifting sample bags.

Employee was working to install tires when the tool he was using slipped and struck his right hand.

Operator bumped left knee on dozer hand rail.

Employee was cutting a band off of a roll of liner. He had his left hand on the roll to steady himself and accidentally sliced his hand instead of just the banding.

Employee stepped on a rock and sprained his ankle.

Employee felt a pop in his lower back while attempting to lift welding materials.

Contractor was driving a water truck, struck a rock which caused equipment to jar and employee reported pain in his back.

Operator complain of pain in his wrist while operating shovel and loader.

Laboratory Employee cut his right hand while using an Erlenmeyer flask.

Employee received a rib strain while operating a vice.

Employee received a finger fracture while working on equipment.

Unplanned detonation of explosives within a blast hole that did not ignite during normal blasting procedures.

While performing pre-inspection of equipment, a gust of wind blew and a small particle got in the left eye.

Contract employee received a laceration on right leg while operating a skill saw.

Employee was trying to lock the plow bolt into the lip saver with his hand while mechanic on top was trying to remove the nut with the impact. The bolt turned and cut his left hand middle finger. Employee was wearing gloves.

Employee complained of lower abdomen/back pain while cleaning up material in the pit.

Employee was operating Kubota 174 and was exiting the leach pad via main truck ramp for break. As the employee descended the ramp, the Kubota lost control as the dynamic braking engaged too quickly on wet haul truck road overturning the Kubota on the driver side.

Employee was loading blast hole samples to a flatbed truck when he experienced a sharp pain in his left shoulder. Did not become reportable till he went to the doctor 5/1/14.

Employee was driving a van, dropping off haul truck drivers during their first break. He parked the van in the ditch at the ready line. He got the wheel chocks from behind the seat and proceeded to exit the van. As he stepped out, his left foot came down on a rock that was laying in the ditch. His foot rolled off the rock sideways twisting his ankle resulting in a sprain.

Employee was walking along the side of truck 517 doing his pre-inspection when he felt a pop on his left knee. He continued his inspection to finish his inspection sheet. He climbed into his truck and went to work the first part of his shift. After his first break, he notified his supervisor at 11:45 am that he hurt his left knee while doing his truck inspection.

It was a heavy day of blast hole sample pickup. EE was working alone and made three separate pickups with the flat bed and delivering to the lab during the day. The pain started after the second load was collected. Pain was experienced along with some sensation/pain on wrist and index finger. The reason 7000-1 was just reported, EE went on restriction on 6/13/14.

Employee slipped while installing HP filter on a shovel.

Employee was checking a hydraulic leak and slipped in the park ditch as he was going under the truck. His left rib cage area contacted the truck. Employee stated he was OK when he reported this to his supervisor. The next shift day, he indicated he was sore and would like to be checked out.

Loader operator received a scalp laceration when the loader was digging some hard material, causing the loader to jolt slightly, hitting his head on the sun visor.

Employees received a knee sprain while boarding a fuel truck

While the Operator was climbing ladder to the haul truck he felt a tearing in his arm.

The employee was walking down the steps and slipped off the bottom step and rolled her right ankle.

Employee received a thumb laceration on left thumb while replacing ripper shanks on a dozer.

Employee was climbing out of rear of service truck 702. When he stepped down from the truck bed he twisted his left knee.

Employee was using a whipping motion to put 2 foot spacing between drip lines when he felt a minor pain in his right shoulder.

Employee was removing a J-bolt retainer and bucket lip protector with an air hammer and chisel when struck in lower leg by a metal fragment.

Employee was changing a drill bit when his left hand got caught between the deck wrench and the bit causing a contusion/abrasion on his left hand.

Employee reported he might have pulled a muscle in his arm getting a sample on the drill.

Employee was using a mechanical pipe snake and sprained left wrist.

Employee fell and injured back while inspecting the haul truck.

Employee was cutting a plate with a track torch when plate fell on foot causing a laceration.

Employee was attempting to shorten the pull down chain on a blast hole drill. He struck the chain link with a hammer, dislodging a piece of metal which struck him in the abdomen.

Employee was tightening lugs on a motor when he felt a sharp pain in his right shoulder.

Laceration to 2nd finger left hand received when employee was handling a filter while working on the shovel.

Reaching to place a wooden block and overextended right arm.

Driller lost his balance over a depression in the ground and fell scraping his knee and shin on the left leg.

Piece of pipe supported by a rock became dislodged while tightening flange, falling on employee's finger and crushing the end of left pinky finger, requiring sutures.

Employee was removing a bent "tooth" on a dozer blade by using a hammer. Upon metal-to-metal contact with the hammer and tooth, a piece of shrapnel embedded above employee's left knee.

While performing pre-op check, employee was extending the boom when a small foreign object fell off boom, hit forehead, and behind safety glasses into employee's left eye.

Employee was walking over uneven terrain

Employee on blast crew helping load shot. He had just attached the delivery tube to the auger on bulk truck. Employee stepped backward onto a rock and twisted his ankle.

Employee was attempting to free a drill steel. The employee exerted considerable force expecting resistance. When the steel freed easily, the employee tumbled backward straining his shoulder.

Employee was struck by a piece of metal that was dislodged when a mechanic struck a drill deck bushing while attempting to free it from the drill steel.

Employee slipped while getting off haul truck, and landed on his back.

Loose ground gave beneath the employee's foot while he was standing on a berm. The employee rolled several feet.

An employee backed a haul truck up a ramp and struck the truck parked behind him.

employee was driving a haulage truck with an improperly adjusted seat.

Striking hammer onto bucket tooth. Mettle chipped off striking top of right hand ,piercing skin.

While building a foot bridge, a piece of metal rolled onto finger.Index finger left hand. Smashed finger brusied under finger nail.

The weight of heavy mettle plates were not placed properly allowing them to fall backward, in mettle rack, onto the employee's left hand middle finger, smashing end of finger.

Employee was greasing bulk truck and strained his back.

Employee bent forward ( stooped over ) to pick up a net with a handle when he felt a pain in his back. The net is used to retrieve objects from a wash tank and weighs less than 5 pounds.

Employee was loosening the bolts on the door of the cage over the back-up alarm on a front end loader when the dooor sprang open pinching his left index finger.

Employee was demoted on 10/21/2008. On 11/04/2008 employee filed for FMLA. Employee subsquently sought medical opinion and determination was made on 12/31/2008 that employee's condition (stress) was job related.

Employee was performing routine maintenance duties on an O&K 340 hydraulic shovel. He was walking on the rear catwalk when his foot slipped and his leg went through an opening between the catwalk and the counterweight of the shovel.

WHILE ASSISTING ON REPLACEMENT OF THE LOADER BUCKET TEETH, EE REMOVED HIS SAFETY GLASSES TO WIPE THE SWEAT FROM HIS FACE & A PIECE OF METAL BROKE OFF & LOGGED IN HIS RIGHT EYE. METAL BROKE OFF BECAUSE THE HAMMER THE WORKER WAS USING WAS STEEL ON A STEEL TOOTH. EE WENT TO THE HOSPITAL AND HAD SURGERY TO REMOVE THE METAL. HE IS EXPECTED TO MAKE A COMPLETE RECOVERY

Employee fell off shovel when exiting.

Slip and fall on inclined surface.

Employee shifted weight from right leg to left leg while kneeling and spray painting machinery.

Employee was handling wet metal grates.

Injured EE was working underneath a 16M grader installing a pump. The pump splines into the transmission and the pump was still connected to its hoses. The EE's hands slipped and the pump swung downward on the hoses and the pump struck EE in the mouth and bruised his lip and broke his right eye tooth.

Employee was repositioning a drill air supply bracket. The bracket dropped approx. four inches and pinched his hand.

Employee was working in a crouched position while fastening a hydraulic hose. He turned sideways and strained hie right knee. The employee was seen bt a doctor and told to ice the knee at night. On a follow up visit to the doctor on 12/19/07, he was placed on limited duty.

Employee was dismounting a fuel/service truck and hyperextended his left knee.

While removing 992 cab, load shifted and pressed employee between cab and trailer resulting in shoulder injury.

Employee was driving a haulage truck and hit a bump, which caused his back to wrench slightly.

Lifting /maneuvering heavy object ( machinery )

Employee alleges he struck his kneecap on a rung as he was ascending a ladder. At date of report he has been diagnosed as unable to return to work pending an MRI and any recommended treatment.

Employee's hand was caught in a pinch point when he inadvertantley placed or left it placed on a length of pine that was being pushed toward a fusion machine.

The employee was attempting to relocate a rattlesnake from outside the maintenance shop area to a more remote area of the mine. While transferring the snake from the snake pole to his hand, the employee was struck by one fang on the first knuckle of his left hand.

While in the process of closing a valve on Pipe #2 at the CIC, the employee felt his right knee buckle to the inside of his stance and a sharp pain in his knee.

The employee was pulling drip line from the spool on the leach pad when a coworker pulled on the end of the line to stop the lines from reeling out. The abrupt stop caused the employee to feel a sharp pain in his lower back.

During a power outage the employee was walking back to the plant office after checking on the emergency lights at the lab. While walking, he stepped on a small rock and turned his ankle, resulting in a mild sprain. It was very dark due to the power outage and they employee did not have a flashlight.

EE was cutting 1/2" drip line on the side slope of Leach Pad #4 to remove carbon and scale from emitters. In the process, his knife slipped off the drip line and cut his right leg.

The double deck on the Gold Plant shaker screen had become loose and rotated from the carbon discharge chute. Employee tried to reposition the upper deck back into place. After about fifteen (15) minutes, he felt some pain in his groin area. Employee was able to complete his shift. He was seen by a physician's assistant the next day, and was diagnosed with a hernia.

Employee states his right hand became entrapped between line and casing brush, while working brush into well casing.

In the act of leaving bolts and tools in tool box on floor, when raising back up hit left elbow on hand railing.

EE WAS GETTING THE PREG POND LEVEL. HE WAS MOVING THE WEIGHT CONNECTED TO THE TAPE MEASURE. AS HE WAS DOING THAT, HE LOST HIS FOOTING AND TWISTED HIS ANKLE. FACTORS/REASONS: POND COVER IS A SLICK LINER; EE WAS NOT USING ANTI-SKID PADS THAT ARE STUCK TO THE POND COVER. LOST BALANCE; EE WAS NOT USING ROPE TIED TO TOP OF POND.

EE WAS WORKING WITH TWO OTHER EMPLOYEES TO TROUBLE SHOOT & REPAIR A 2" SUBMERSIBLE PUMP AT THE BOTTOM OF THE "EVENT" SOLUTION POND. HE WALKED THE HDPE LINED SLOPE OF THE POND (3-1 POND) A COUP LE OF TIMES WHILE TROUBLE SHOOTING THE PUMP. WHEN FINISHED HE REPORTED A "TINGLING" SENSATION IN HIS LEFT HEEL. STRAINED ACHILLES TENDON.

EE WAS WORKING WITH CO-WORKERS TO STRING DRIP TUBE ON A "RIPPED" PAD SECTION. AFTER CUTTING THE DRIP TUBE TO LENGTH, HE SQUATTED DOWN TO PICK UP THE LOOSE ENDS OF THE TUBE TO PULL THEM INTO PO SITION. AS HE SQUATTED HE FELT HIS LEFT KNEE "LOCK UP" AND FELT A SHARP PAIN IN THE KNEE.

EE WORKED WITH LEECH PAD CREW TO STRING DRIP TUBING ON THE N. VISTA LEECH PAD. HE WAS LOADING ROLLS OF DRIP LINE ON THE HOSE REELS, SPLICING THE TUBING & CUTTING IT TO LENGTH. AFTER COMPLET EING THE WORK HE COMPLAINED OF A SORE BACK. HE COULD NOT CITE ANY SPECIFIC TIME OR INCIDENT THAT MAY HAVE CAUSED SORENESS. IT WAS AN EXCEPTIONALLY WINDY DAY.

EMPLOYEE SUFFERED A HEART ATTACK. EXACT CAUSE IS UNKNOWN. EMPLOYEE WAS IN THE SHOP BAY LEANING AGAINST HIS TOOL BOX. WHEN QUESTIONED BY HIS SUPERVISOR, HE COMPLAINED OF CHEST PAIN. HE WAS TRAN SPORTED TO PIONEER'S MEMORIAL HOSPITAL IN BRAWLEY, CA, WHERE HE WAS FOUND TO BE SUFFERING A HEART ATTACK. WAS LATER TRANSFERRED TO A HOSPITAL FOR BY PASS SURGERY.

EE HAD OPENED THE DOOR TO THE CABINET LOCATED IN THE LADIES' CHANGE ROOM. AS SHE WAS REMOVING AN ARTICLE, HER FINGER WAS CUT ON A SHARP EDGE OF THE CABINET.

EE WAS REPLACING CUTTING EDGE ON A RUBBER TIRE DOZER. AS HE LIFTED A CUTTING EDGE INTO PLACE, HIS RIGHT INDEX FINGER WAS CUT ON A SHARP EDGE OF THE MOLD BOARD.

EE USED A 6' STEP LADDER TO ACCESS THE FUEL FILTERS ON A HAUL TRUCK. WHILE DISMOUNTING THE LADDER HIS RIGHT FOOT CAUGHT ON THE LAST STEP CAUSINGHIS KNEE TO TWIST. EMPLOYEE HAD NO TREATMENT UNT IL 1/15/01 AT WHICH TIME THE DOCTOR PLACED HIM ON RESTRICTED DUTY.

EMPLOYEE WAS USING A RAZOR KNIFE TO CUT STRINGS THAT ATTACHED TO A SPLICE IN A SECTION OF DETACORD. HE HELD DETACORD WITH HIS LEFT HAND AND AS HE CUT THE STRING THE BLADE ALSO CUT HIS LEFT IND EX FINGER.

EE HAD CHECKED THE HYDRAULIC OIL ON A LOADER. WHEN STEPPING TO THE GROUND WHILE DISMOUNTUIBNG, HE STEPPED ON A ROCK TWISTING HIS RIGHT ANKLE.

EE HAD BEEN ASSIGNED TO OPERATE A HAUL TRUCK. AFTER CONDUVCTING A PRE-OPERATION CHECK ON THE TRUCK, HE LIFTED HIS RIGHT LEG TO PLACE IT ON THE STEP OF THE TRUCK, A LADDER. AT THIS TIME HE ALLE GES HE FELT SUDDEN PAIN IN HIS LOWER BACK.

EE WAS CLEANING LIME BUILDUP OFF THE SLAKER TRUCK WITH A SHOVEL. DURING THE PROCESS LIME GOT ON THE SKIN BELOW BOTH EYES CAUSING IRRITATION. FIRST AID WAS PROVIDED ON SITE. EE CONTINUED TO HAV E IRRITATION & WENT TO EYE DOCTOR. MEDICATION WAS PROVIDED & EE WAS TAKEN OFF WORK FOR ONE DAY.

EE WAS LIFTING A FLOOR JACK. WHILE LIFTING THE JACK SLIPPED OUT OF HIS HAND. HE REACHED TO CATCH THE JACK AND FELT A SHARP PAIN INHIS RIGHT LOWER BACK.

EE WAS CUTTING THE STRAPS FROM DRIPPER LINES WITH A RAZOR KNIFE. HE CUT TOWARD HIS LEFT ARM, CUTTING HIS LEFT FOREARM.

EMPLOYEE WAS DRIVING AN A.T.V MULE. A CREW CAB PICK-UP WAS FOLLOWING. WHEN THE EMPLOYEE SLOWED THE MULE, THE PICK-UP BRAKED BUT COULD NOT STOP IN TIME BECAUSE OF WET CONDITIONS AFTER THE RAIN. THE CREW CAB PICK UP STRUCK THE REAR OF THE A.T.V.

EMPLOYEE WAS ASSIGNED TO PICK UP TRASH. SHE KNEELED DOWN TO TIE A TRASH BAG AND LOST HER BALANCE. SHE REACHED OUT WITH HER RIGHT HAND TO BREAK HER FALL AND FELT PAIN IN THE RIGHT WRIST JOINT .

EE WAS DISMOUNTING A HAUL TRUCK. AS HE STEPPED ON THE GROUND HE FELT A POP IN HIS LOWER BACK. HE FELT NO PAIN AT THE TIME, SO HE FAILED TO REPORT IT TO HIS SUPERVISOR. AFTER HE ARRIVED HOME, H IS BACK STARTED HURTING. HE REPORTED THE INCIDENT TO HIS SUPERVISOR WHEN HE REPORTED TO WORK HIS NEXT SHIFT.

EE AND A COWORKER HAD PARKED THE BASKET TRUCK AT THE JOB SITE. ONE EE WAS AT THE CONTROLS AND THE INJURED EE WAS STANDING BESIDE THE TRUCK IN THE PATH OF THE OUTRIGGER WHILE IT WAS BEING LOWER ED. HIS FOOT WAS CAUGHT UNDER THE OUTRIGGER, RESULTING IN A BROKEN BONE IN HIS RIGHT FOOT.

EMPLOYEE WAS WASHING A HAUL TRUCK ON THE WASH RACK. HE SLIPPED IN WATER AND LOST HIS BALANCE. HIS LEGS WENT OUT FROM UNDER HIM IN A SPLITTING MOTION. HIS RIGHT LEG TOOK MOST OF HIS WEIGHT PUTT ING PRESSURE AND TWISTING HIS RIGHT KNEE. HE BEGAN TO FEEL PAIN AND SWELLING IN HIS RIGHT KNEE.

EE ALLEGES THAT AFTER DUMPING MATERIAL FROM HIS TRUCK ON THE LEACH PAD, HE LOWERED THE TRUCK BED AND WHEN THE BED CAME INTO CONTACT WITH THE TRUCK FRAME, THE EE FELT PAIN IN HIS LOWER BACK.

EMPLOYEE WAS CUTTING DRIP TUBING WITH A RAZOR KNIFE. THE KNIFE SLIPPED, CUTTING THE EMLOYEE'S LEFT INDEX FINGER. THE INJURY REQUIRED 5 STITCHES.
