Employee was using a sledge hammer to try to free the wear plate on a D10 dozer blade. A metal fragment broke off the plate and struck the employee in the leg.

The contract truck driver working for STM reported slipping off the last step of the truck and falling to the ground when in the process of trimming the load. EE reportedly put hands out to catch self. EE left the site after falling and sought medical attention.

A maintenance foreman sprained right hand ring finger when EE hyper-extended it while changing a troughing roller on a conveyor. The treating physician prescribed anti-inflammatory medications and a splint for the finger.

Employee was splicing a conveyor belt when the belt sagged causing him to hold himself up by his arms while holding onto an adjacent conveyor side rail. This resulted in a strain to his right shoulder.

Employee reported on 12/07 that on 12/06 he believed he injured his lower back while performing a crusher liner change. He refused medical treatment since he wasn't sure if it was industrially related or not. On 12/10 he reported that he wanted to file a W/C claim. He indicated that the injury was from over-exertion. Provided pain meds, temp work restrictions.

As the employee was getting ready to step on the crusher he was working on, he stepped on a piece of angle iron that was tack welded on the liner. The angle liner broke off causing the employee to fall about three feet backwards onto the head of the crusher.

The employee was removing some wood blocks from the back of a service truck. When he stepped away from the truck he tripped and fell. To catch his balance, the employee leaned back towards the truck and caught his left little finger on blocks from the bed of the service truck.
