As the employee was changing screens for the Extec plant he twisted the wrong way and strained his back.

While employee was shutting off a disconnect at BC-0, located near the head pulley, the box arched, blew open the door striking him in the chin.
Employeee was carrying a shaft. As he turned around he smashed his finger between the shaft and a steel rack.
While tightening a bolt on a take up pulley, the employee's wrench slipped and he fell back striking his left shoulder on the man basket cage.
EE WAS HOLDING THE HEAD OF A BOLT WITH A GLOVE ON HIS HAND WHILE TIGHTENING IT WITH A 3/4" AIR IMPACT GUN. THE BOLT SPUN AND CUT HIS LEFT MIDDLE FINGER THRU HIS GLOVE.
Inspection lid swung shut and bruised ee's back.

EE WAS PULLING TWO SCREENS AT ONCE ONTO THE SHAKER WHILE ANOTHER EE WAS PUSHING THE SCREENS TO HELP HIM, WHEN EE'S FINGER BECAME SMASHED BETWEENTHE TWO SCREENS CAUSING A LACERATION AND FRACTUR E TO HIS RIGHT MIDDLE FINGER.

SOMETHING FELL INTO SOME DUST AROUND EMPLOYEE SENDING DUST INTO THE AIR. STEEL SUDT GOT INTO EMPLOYEE'S EYE. HE WAS NOT WEARING SAFETY GLASSES. MISSED ONLY 1 DAY AS SUGGESTED BY DOCTOR.

BACKING INTO DUMP AREA BEHIND DRIVE, HE OVERBACKED ONTO A DECLINE ON THE RIGHT SIDE. FRONT WHEELS SLID SIDEWAYS AND TRUCK ROLLED ON RIGHT SIDE.
CHECKING FOR WATER LEAKS ON WET DECK, BENT UNDER PIPES TO INSPECT. WHEN STOOD UP, HAD PAIN IN LOWER BACK INTO LEFT LEG.
HIPS WENT OUT WHILE DRIVING EQUIPMENT AT WORK OVER BUMPY ROADS.
CLEARING A CHUTE FULL OF ROCK WITH A BAR, HE HITTHE ROCK WHICH GAVE WAY AND CRUSHED FINGER BETWEEN THE BAR AND CHUTE.
LIFTING STEEL PLATERS/COVERS TO GET WIRES PULLEDLOWER BACK.
REMOVING A PIECE OF BELT PULLED ON BELT, IT WAS BURIED, WHICH CAUSED STRAIN-CAUSED STRAIN TO ABDOMEN HERNIA.
SHOVELING MATERIAL, TWISTED WRONG.
USE OF COMPUTER FOR PAST 16 MOS CAUSING PAIN IN ELBOWS DIAGNOSIS WAS FOR TREATMENT FOR A PERIOEDOF TIME & EVAULATION OF INJURY. 9/18/00SURGERY TOOD PLACE FOR ELBOW-CARPAL TUNNEN
HAMMERING ON ANVILS WHEN EITHER ANVIL OR HAMMER CHIPPED OFF AND WENT INTO HIS ARM.
