Mc Clane Canyon Mine Coal

employee was loading belt structure into scoop bucket, rigid structure rail fell on right foot. Weight of rail estimated at 50 lbs. Right foot was sore at shift end. Employee went to Dr. on 11/06/2010. Xray confirmed two fractured metatarsals in right foot. Employee was wearing metatarsal boots at time of injury.

Roof fall was discovered about 4000' outby working section in #3 entry crosscut #34. The fall did not affect ventilation, impede persons or affect any equipment. Fall was discovered at 12:20 pm

The employee was removing a kennedy stopping at crosscut 11, #4 entry of 7th South Panel. When he removed the panel closest to the rib, coal from the rib fell striking the employee on the neck and back. The employee suffered bruising to the neck and back.

A roof fall was discovered at 8:50 p.m. during a pre-shift. The fall was located at x/c 10 7th South panel, crosscut 4-5. The fall measured 20 feet long, 18 feet wide and 5 - 5.5 feet high. The fall did not affect ventilation, impede persons or affect any equipment.

A roof fall was discovered at 8:47 am in the 2nd East Mains, x/c #4, #1 entry. The fall measured approximately 20' long, 25' wide and 7 - 9 feet high. The area was bolted with 5' fully grouted bolts. The fall did not impede ventilation or escapeways.

A roof fall was discovered during a preshift of the 7th South panel. The fall was located at xc 8, #6 entry. The fall measured approximately 60' long, 18 to 30' wide and 7' high. The fall was located in the secondary escapeway. The escapeway was relocated around the fall.

A roof fall was discovered at 6:30 am during a weekly examination. The roof fall was located in the 2nd East Mains, x/c 34, #2 entry. The roof fall measured approximately 20' wide, 80' long and 7' high. The fall did not affect ventilation, travel of persons or involve any equipment.

A roof fall was discovered at approximately 7:40 a.m. this date. The fall occurred in the Second East Mains, x/c 35, #4 entry. The fall measured approximately 20 feet long, 20 feet wide and 8 feet high. The fall did not impede travel of persons or affect any ventilation current.

A roof fall was discovered at 10:40 am in the Second East Mains, #3 entry, X/C 34. The fall measured approximately 25' wide, 25' long and 8' high. The fall did not affect travel of persons or any ventilation current.

Two employees were drilling hole in floor with hydraulic drill. The drill stalled and the handle of the drill hit the injured employee in the left knee.

Employee was walking down along a concrete retaining wall and fell over the wall to the concrete floor below. The employee received 12 stitches in his left forearm near the elbow.

A roof fall was discovered during the weekly exam of the Main East bleeder. The fall was located at x/c 37 of the East Mains, #1 entry. The fall measured approximately 18' wide, 18' long and 9-10' high.

A continuation of a previous roof fall was discovered during a weekly exam. The fall was discovered by company and MSHA officals. The fall was located in the #4 entry x/c 36 of the East Mains. The fall measured approximately 8 ft high, 16 ft wide and 20 ft long. The fall did not impede passage of men or impair ventilation.

The injured employee was putting in a sightline in # 3 entry 3rd South, #4 x/c. A piece of roof 5' long, 3' wide and 0 - 7" thick fell from between the roof bolts and struck him on the hard hat.

A roof fall was discovered during a pre-shift of the East Mains. The roof fall was located in #36 x/c, between #3 and #2 entry. The fall measured appproximately 15 ft. wide, 18 ft. long and 8 ft. high.

A roof fall was discovered in the East Main entries crosscut 67, between entries 6 and 7. The roof fall measured approx. 18 feet wide, 80 feet long and 8-12 feet deep.

A roof fall was discovered in the Main East entry crosscut 36, entry 2. The fall measures approximately 7 ft high, 18 ft wide and 18 ft long. The entry was initally supported with 5 foot fully grouted resin bolts. The fall did not impede passage of men, or impair ventilation.

An explosion occurred underground in the area of crosscut 24-25, entry #1, near the underground compressor. The explosion was believed to have been caused by a low voltage signal coming from the power company causing the main mine fan to go off, but not the power to the underground areas of the mine. No one was on the mine property when the explosion occurred.

Employee was standing beside a coal rib watching the miner cable when a piece of the coal rib measuring 3'x3'x4-6" fell striking him on the back.

Employee claims that he was shoveling the collection belt and felt a pain in his lower back. Strained lower back muscles.

Employee was driving a shuttle car. After dumping his load of coal and on his way back to the face the shuttle car hit a bump or hole in the mine floor. The jolt caused the employee to hit his head on the canopy of the shuttle car.

A roof fall was discovered on 2-16-2005 at 10:05 am in the 3rd North Mains in #5 entry, between crosscut 11 and 12. The fall measured 16' wide, 80' long and 8' high. The fall was reported to the Delta field office at approximately 11 a.m. on the same day.

Employee was stripping outer jacket of a miner cable using a utility knife. Knife slipped and cut left wrist

Employee had loaded his scoop bucket with a load of 8" timber that were 10' long. While hauling them into the section, part of the load fell out of the bucket. The employee stopped to reload the fallen timbers into the bucket. When he picked up one of the timbers he felt a sharp pain in his groin.

EE WAS TRYING TO LOOSEN AN ELECTRIC MOTOR FROM ITS BASE WITH A PRY BAR WHEN THE BAR SLIPPED AND THE EE FELL AND HIT HIS HEAD ON THE GROUND. 12/13/04: IT HAS NOW BEEN DETERMINED THAT EE ALSO INJURED HIS LOWER BACK, CAUSING A HERNIATED DISK.

Employee was going to add airline to the 3" airline. He shut off the compressor and 20-25 minutes later he pulled the locking pin from the airline coupler and the end of the airline blew off causing his injuries.

THE DIESEL TRK THE INJ EE WAS DRIVING DIED. THE EE SET THE EMERGENCY BRAKE & GOT OUT TO LOOK UNDER THE TRUCK. WHILE LOOKING UNDER THE TRUCK FROM THE SIDE, THE EMERGENCY BRAKE FAILED AND THE DOOR CAUGHT HIM AND DRAGGED HIM FOR APPROX 4 FT.

While hanging up a miner cable the employee felt a pain in his lower back.

Employee was unloading steel waterline from a diesel scoop and picked up one end and put it on his shoulder and started to drag it off. The other employee did not get the other end and the other end fell to the ground, causing the first employee to injure his shoulder.

A roof fall was discovered in crosscut 39 of the east mains between #1 and 2 entrys. The fall measured 25 feet long 15 feet wide and 4 feet high. 5 roof bolts were pulled out as a result of the fall and 5 bolts were rendered useless.

EE WAS USING A HYDRAULIC CHAIN SAW TO CUT A MINE TIMBER FOR A BREAKER ROW. HIS HAND SLIPPED FROM THE SAW AND HIT THE BLADE OF THE RUNNING SAW.

A ROOF FALL WAS DISCOVERED AT 6:32 AM DURING THE PRE-SHIFT EXAM. THE FALL WAS INT HE #2 ENTRY, #2XC IN THE FIRST RIGHT SECTION. IT MEASURED 18'20'X8-9'.

A ROOF FALL WAS DISCOVERED AT 10 00 AM IN THE 1ST RIGHT SECTION AT THE #1XC !4 ENTRY. THE FALL MEASURED 18'X18'X8-9'.

A ROOF FALL OCCURRED ON THE 001-0 1ST RIGHT SECTION. THE FALL WAS IN THE #3 ENTRY #3 CROSSCUT & MEASURED 20' X 20' X 8'. A SLICK INSIDED SLIP WAS DISCOVERED IN THE CAVITY.

EE WAS UNLOADING RIDIG BELT FRAMING FROM A TRAILER WHEN HE MASHED HIS RIGHT RING FINGER BETWEEN THE FRAMING & THE TRAILER.

EE WAS IN THE PROCESS OF ADDING A DRILL STEEL, HE LOWERED THE DRILL POT& THE ADDITIONAL DRILL STEEL HUNG UNDERNEATH THE POT MASHING HIS RIGHT WRIST AGAINST THE PANIC BAR.

THE EMPLOYEE WAS OPERATING A ROOF BOLTER. AFTER HE HAD FINISHED DRILLING THE HOLE, HE BENT OVER TO PUT THE PIN IN THE HOLE AND A PIECE OF ROCK FELL FROM THE ROOF AND STRUCK HIM ON THE LEFT SIDE OF HIS HEAD. THE ROCK MEASURED APPROX 1' BY 1' BY 2".

A roof fall was discovered in the North Sub-Mains, inby crosscut 10 in the #5 entry. The fall measured approximately 6 feet high, 18 feet wide and appeared to be approximately 100 feet long.

A ROOF FALL OCCURRED FOLLOWING PILLAR EXTRACTION. ONE MRS WAS LOWERED AND THE ROOF STARTED WORKING. IT WAS RESET WHEN A FALL STARTED INBY THE MRS AND IT CONTINUED OUTBY TO THE CROSSCUT. THE FA LL MEASURED APPROXIMATELY 100 FT LONG, 18 FEET WIDE AND 10-15 FT HIGH.

EE WAS USING A PRY BAR TO REMOVE A PIECE OF ROCK FROM THE FEEDER. THE BAR WAS CAUGHT BETWEEN THE ROCK AND A FEEDER FLIGHT. CAUSING THE BAR TO STRIKE THE EE.

A ROOF FALL WAS DISCOVERED DURING THE PRE-SHIFT OF THE MINE. THE FALL WAS LOCATED IN #3 ENTRY XC-35 OF THE EAST MAINS. THE FALL MEASURED 55 FT. LONG, 20 FEET WIDE AND 10 FEET HIGH.

A ROOF FALL WAS DISCOVERED DURING A PRE-SHIFT EXAMINATION. THE FALL WAS LOCATED IN THE #3 ENTRY XC #38 IN THE EAST MAINS. THE FALL MEASURED 68 FT LONG 18 FT WIDE AND 8 FT HIGH.

EE WAS CARRYING ROOF BOLTS FROM THE BACK OF THE ROOF BOLTER, WHEN REACHING THE FRONT OF THE BOLTER, HE MISSED THE STEP & FELL APPROX 2 FT TO THE GROUND.

EE WAS HELPING TAKE A HIGH VOLTAGE COUPLER APART ON TOP OF A PICKUP. WHEN THE COUPLER CAME APART IT KNOCKED THE EE OFF THE TRUCK.

HANGING ACTUAL ROLLDOWN PART OF OVERHEAD DOOR. EE WAS STANDING AT RIGHT END OF DOOR BETWEEN MAINFRAME AND RIGHT RIB IT FELL, CAUGHT HIM UNDER DOOR, TRIED TO GET OUT, DOOR SLID AND CAUGHT HIS L EFT EAR.

EE WAS NOTIFIED BY CERTIFIED LETTER THAT EE HAD TAKEN BLACK LUNG X RAY AND THAT MINER HAD SUFFICIENT DEGREE OF BLACK LUNG TO QUALIFY AS PART 90 MINER.

EE WAS RE-INSTALLING COVER ON MINER WHEN THE COVER SLIPPED AND STRUCK HIS LEFT HAND.

EE WAS HANGING A WING CURTAIN & PLACED HIS HAND ON ROOF BOLTER. A ROCK FELL FROM THE ROOF STRIKING HIS LEFT ELBOW. ROCK MEASURED 2' X 1' X 6".

WHILE CLIMBING UP THE LADDER OF AN ENDLOADER THESTEP BROKE AND HE FELL ABOUT 5' TO THE GROUND.

EMPLOYEE WAS ROOF BOLTING AND A PIECE OF ROCK FELL FROM THE ROOF, STRUCK A ROOF BOLT WHICH FLIPPED UP AND STRUCK EMPLOYEE ABOVE HIS RIGHT EYE.

GRABBED WATER HOSE TO WATER DOWN AREA AND HURT RIGHT SHOULDER, INJURED DID NOT REPORT INJURY UNTIL 6-14-00 WHEN SHE CALLED AND SAID SHE HAD SOME DISCOMFORT. TOLD HER TO GO TO THE DOCTOR FOR SHOULDER AND BACK.
