Employee was loading bolt resin into the bucket of the underground scoop and strained wrist. IE was put on restricted duty on 10/8/2025 for this injury.

IE was attempting to hang a 6 ton come along to the roof, IE was standing on a 6ft ladder, when the ladder slipped out from under employee. Employee fell breaking left leg and left collarbone.

Employee was on a ladder helping another employee take down a come-along. The come-along slipped and fell while they were taking it down. The employee caught the heavy come-along with left hand. The weight made their left shoulder pop. The employee had surgery on 10/15/2025

IE was setting a 9ft bolt and the long wrench broken at the chuck, the head of the wrench cut IE left arm. The IE laceration needed 12 stitches.

IE was moving ASV forks by hand with another employee, the forks slid together smashing IE left middle finger in-between the forks. IE received 15 stitches for laceration repair.

IE was hanging the miner cable. When IE lifted the miner cable EE felt a weird feeling in EE's lower back and down EE's right leg resulting in restricted duty.

IE was hauling pan line with hauler. IE went to adjust the sections of pan line IE was hauling, the top section of pan line slid off the bottom section. The pan line smashed both ankles and right leg, causing a fracture to the right tib/fib, left and right ankles. IE requires surgery to correct the factures.

IE was loading belt structure, when going to throw the structure IE right shoulder took all the weight. IE continued to work with no restrictions and was taken for evaluation on 11/7/2024 as well as 2/27/25 with no restrictions. IE had follow up evaluation on 4/28/25 where EE is placed on restricted duty.

IE was traveling down the #2 entry when IE contacted a short pan sticking out of the rib with IE's right arm just above the elbow on the inside that required 13 stitches to close the wound.

In the #1 entry of B-Access an unplanned roof fall occurred above and anchorage of the 7' primary roof supports, the fall measured 8'x 22' long x 18' wide.

IE was washing shields, as the IE stepped between the shield toes on top of the relay bar IE's left foot became stuck. IE lost their balance falling backwards causing IE to fracture both the Tibia/Fibula. IE will require surgery to correct the fractures.

IE was building a crib and was above the ground approximately 3 feet when the crib fell over. When the crib blocks fell approximately 3 or more cribs stuck the employee in various places on EE's body. One caused a fracture to T1 vertebrae.

IE was using a pipe wrench to loosen a fitting in the prep-plant when IE felt a pain in abdomen. The employee continued to work, took the employee back in for further evaluation on 01/15/2024 where it was determined it would take surgery to correct the hernia. IE is going in for surgery to correct the hernia on 01/18/2024.

A non-injury accident occurred on August 18th, 2023 at approximately 12:57 pm. An unplanned roof fall occurred at B-Mains belt entry between xc49 to xc50 in the number 3 entry. The roof fall measured approximately 35' long, 18' wide, and 7' high.

IE was traveling along the warehouse dock when EE lost EE's footing causing EE to fall approximately 4' from the warehouse dock to the ground causing a fracture of the bone to the right Tib/Fib. IE is consulting with an orthopedic surgeon today 07/27/2023.

IE was roof bolting when EE grabbed the wrong lever closing the jaws around EE's right hand causing a fracture to fourth metacarpal shaft fracture.

While roof bolting the IE was attempting to move the roof bolter cable out of the way while IE was installing a bacon strip. When the IE stepped back down onto the platform IE snagged the mast feed cylinder with IE's clothing causing the mast to move down pinching IE's right arm between the mast and roof bolt plate tray.

IE was attempting to install a new leg in shield 139 when a piece of debris fell into the leg pocket, the IE was removing the debris when the shield leg fell approximately 2" to 3" smashing IE's right index finger which required stitches to close the wound.

A roof fall occurred from x-cut 6 1 entry beltline to approximately 44 outby x-cut 6. The roof fall was reported because it blocked the secondary escapeway. The roof fall was not above the anchorage point.

IE was grading out of the tail-gate when a roof bolt was kicked out by the lump breaker striking the IE above the left eye causing a laceration that required stitches to close the wound. It also caused a tooth to be chipped.

IE was tramming a skid loader when the cab contacted a multi-hook raising the cab of the skid loader and hinging it back. When the cab came free from the multi-hook it slammed down smashing the IE's left foot.

IE was cutting a hose with a utility knife when the blade slipped it caused a laceration to the left thumb.

IE was lifting tubes above head when IE felt a pop in right shoulder. This accident will require surgery to correct the shoulder.

IE was attempting to cut a lifeline cone with a razor knife. The knife slipped off the life line cone and stuck into IE's left hand causing a laceration that required 4 stiches to close the wound.

During a pre-shift examination a roof fall in the intersection of X-cut 42 in the number 1 entry of 14th left longwall section MMU 006-0 was located. No miners were trapped or injured as a result of the fall.

IE was attempting to change out a top middle roller on a conveyor with another employee. They were prying the roller out with a long screw driver, when the screw driver slipped it struck the injured in the right ring finger causing a laceration and fracture, the laceration required sutures to close the wound.

IE was roof bolting in second right X-Cut 1-2 when they inadvertently closed the jaws on their left hand causing a laceration on the left middle finger that required stiches to close the wound.

I.E was trying to straighten frame work on the tailgate drive with a crib block between the anti slew bracket and tailgate drive frame. I.E was holding the crib block while another employee was operating the shields pulling the pan-line back. The crib block popped out striking I.E in the face knocking I.E unconscious and causing a fracture to zygomatic bone.

At approximately 10:35pm a Roof Fall was observed by an out-by employee in B-Mains number 5 entry between xc 13 to xc 14. The Fall measured 25' in length, 16' wide, and approximately 6' to 7' in height.

IE was walking into a ditch when IE stepped on a loose rock rolling left ankle causing a fracture.

IE stepped into a broken wheel chock that EE did not observe on the mine floor causing a sprain to EE's left ankle.

A roof fall occurred at x-cut 14 between entry #2 to entry #1 in the 13th left beltline above the anchorage of the fully grouted 9' headed rebar bolts.

IE left approximately 10' of steel in the roof while checking suction placed left hand on drill head steel came out of the roof striking IE's left ring finger causing a fracture.

IE slipped exiting the hourly showers striking their head on the floor causing a laceration to the back of head the laceration required 11 staples to close the wound.

IE was washing down the slot storage when IE slipped on the ice causing IE to land on right hip and right elbow causing an injury to right shoulder. The next regularly scheduled shift was 01/20/2020 which the IE was not able to work.

IE was changing bits on the continuous miner when a piece of metal struck the employee in the neck. The object was removed by the doctor and the wound took stitches to close the laceration.

Ie was bent down moving the water hose to the continuous miner when a rock came out from behind the mesh hitting the employee in the head slamming the employee's face into the shuttle car and fracturing a tooth.

IE was examining the beltline when they stepped on a loose chunck of coal causing their left leg to raipidly slide forward causing a major tear to the left hamstring.

IE was holding the back up wrench while loosening a bolt on the can setter when EE's wrench slipped causing it to pinch left pinky finger between the wrench and ram on the ASV.

IE was struck several times by a water hose that had been turned on by a scoop bucket causing a laceration that required 5 stitches to close the wound.

While rehab bolting in the bottom of the B-Mains two cribs where required to be removed to bolt a brow, after the roof bolting resumed the top started to settle on the ATRS of the roof bolter.

A roof fall occurred in the # 2 entry of the 13th left start line between xc's 2&3. Trapping 2 roof bolters, once the roof fall was discovered by the section lead man efforts were made successfully to reach the trapped miners.

Employee was carrying a crib block up a ladder when the ladder shifted cause the employee to fall off the first rung of the ladder spraining right ankle.

Between X-cut 41 and 42 in the 13th left return MMU 002-0 a roof fall has occured. Roof Fall dimensions are 16' wide, 50' to 60' feet long, and approximately 10' feet high.

Employee was installing a washer and washer crushed employee's left thumb cutting off the tip of the thumb. Able to return to work with temporary restrictions. No lifting, carrying with left thumb.

Lowering chute into place, chute wedged and came down onto thumb resulting in amputation of tip of thumb. Originally restricted work only for 1 week. Then complications developed and employee returned home to WV for treatment. Injury changed from zero lost time to indefinite lost time.

Drilling supervisor was attempting to move a landing plate during the rig down process. After it was loaded onto the forklift they attempted to shift the plate which tipped to one side and caught the tip of their left ring finger resulting in localized cuts.

While roof bolting employee got right ring finger caught in the jaws amputating the tip of employee's finger.

While unloading belt structure off the trailer a top roller fell on employee's left index finger causing a laceration that required stitches to close the wound.

Employee was hanging 4" pipe when ee slipped on the trailer and went to catch self ee fractured a bone in left hand.

Two employees where throwing belt structure into a metal dumpster, the one employee did not see the injured employee's hand on the edge of the dumpster before throwing the belt spreader striking the injured's right pinky finger and ring finger.

While roof bolting the employee's drill steels got stuck in the roof mesh. The roof bolting partner set angle bolt causing the roof mesh to move freeing the steels from the mesh. The steels came out of the roof striking the employee's right pinky finger that was on top of the jaws causing a wound that required stitches. Right pinky finger also has a small fracture.

Employee was traveling through a man door and felt a pop in right knee below the knee cap. Employee went in for Surgery on 03/30/18 First lost day was on 04/02/18.

While shoveling at the 12th left belt transfer the employee was struck in the mouth by a chunk of rock or coal, the wound required stitches to close the wound.

While removing angle iron from the sewer plant a piece of angle iron swung down striking the employee in the head right below EE's hard which required stiches to close the wound.

While roof bolting employee got left thumb caught between the roof bolt and the back of the jaws causing a laceration that required stiches to close the wound.

Employee was pulling in shields when a chunk of coal was propelled by the tailgate drum of the shearing machine, contacting the employee's right middle finger causing a fracture.

Employee was making a cable splice, while crimping the leads together EE missed stomper with the single jack striking EE's left thumb. The smashed thumb required stitches to close the wound.

Employee was building a block stopping when one of the blocks fell off the wall the employee attempted to catch it smashing EE's left index finger. This created a fracture to the employee's left index finger.

While climbing the ladder of a haul truck to check the odometer, the employee's foot slipped off of the fourth rung, causing her to hit the left side of her rib cage against the ladder causing a fracture to one of her ribs.

During the process of splicing a steel cable belt on an overland conveyor the contractor employee sustained a laceration to his right knee requiring 5 stitches while using a knife to put a bevel edge on a section of 48" steel cable belt which being spliced into the conveyor

A Roof fall was discovered in B-Mains x-cut 4 intersection of the number 2 entry. The fall was discovered on a weekly exam. The fall compromised our secondary escape way, all miners were withdrawn to outby that location. Report was called in to the one eight hundred number upon discovery.

We were notified that we have a miner that has been diagnosed with miners pneumoconiosis MIIN#M96297451 which is a degree 7 occupational illness.

While pulling a leg gauge on #90 shield gauge blew out striking employee in his right hand causing a laceration that required stitches.

Employee sprained his right knee back in September, went in for surgery on 03/15/2016 to have the damage to the kneee corected.

While traveling into the section the door of the man trip portion or dog box came open both employee's in that compartment grabbed the door to close it, the one employee did not get his finger out of the way in time slamming in the door as it closed. No fracture but it did cause a laceration that required sutures to close the wound.

Employee allegedly had the headgate push into his ladder when he was taking down a piece of monorail causing him to strain his neck. Investigation is on going due to conflicting testimony by employees.

Employee was attempting to cut a rock dust hose with a ulility knife, when the knife slipped the employee cut his left thumb. Requiring stiches to close the wound.

Employee was attempting to hang up the wash down hose by flipping it up onto the sims. The hose fell turning on the valve handle knocking off the employee's airstream helmet. The hose continued to whip striking the employee in the right eye ball causing a laceration.

Employee was tightening flinger plate on chuck, raised his drill steels and closed the jaws to hold drill steel up out of the way when his partner set his bolt the drill steels fell about 2' striking employee's middle finger on his left hand, causing 2 small lacerations that required stitches and a fracture to the bone.

Employee was exiting the south door of the sample tower headed for the screen building. He slipped on some ice, employee did not fall down but did wrench his back. Employee does have degenerative disc disease, this is a health issue and should be considered as a degree six.

While building a block stopping employee went to break a 3" solid block over a metal crate, his hand slipped off the lower portion of the block causing his right hand to contact the metal crate fracturing the fourth metacarpal.

Employee was filling cement cribs when the pressurized line he was holding slid back in his hand. There was a hose clamp on the end of the hose that caught his 5th digit finger causing laceration.

Material fell out of the roof creating a roof fall. No equipment or personnel were involved. No relevant events occurred before or after the roof fall. The fall occurred at 11th left #1 entry crosscut 39.

Unplanned roof fall above the anchorage zone B-Mains xc6 between the number 2 entry and number 3 entry. The roof fall has not compromised the alternate escapeway or the primary escapeway. Primary roof support 7'point anchor bolts with resin. Breaker rows of crib will be installed on both sides of the roof fall.

While loading roof mesh onto the A.T.R.S. of the roof bolter employee got his little finger pinched, when he pulled his finger out the mesh fell on his head straining his neck.

Employee was constructing a block stopping, and moving up kitchen supplies. Employee woke up the next morning with a sore back.

Employee was using a come-along to lift up the conveyor belt, while lifting the other side employee felt a sharp pain in his lower back. The incident originally happened on 04/25/2014 he didn't seek medical attention until 05/06/2014.

Employee has been off work since 04/15/2014, on 04/29/2014 is when the injury turned from S&A to workers comp. Employee has two bulged discs in his back and is seeking medical attention. We have no incident to relate this injury too, there for this will be degree six.

Roof fall above anchorage zone B-Mains xc 9 in the #1 entry. No one injured escape way was not affected. Area around the fall was dangered off and breaker rows were set to avoid any additional fall of roof.

Setting a motor base into place with a come along, the hook on the come along sprung loose cause the employee to fall face first into a 5 gallon bucket causing loss of consciousness and a laceration that required stitches to close.

While lifting a 4'x8'x1/2" plate of steel into position in the prep plant elevator chute, the plate came dislodged from the chain and hoisting equipment striking the employee in the left foot. Two fractured bones in his left foot.

Employee was sliding a 55 gallon drum of oil out of the batch to be lifted out of the rack. The employee then felt a sharp pain in the lower back. Employee has taken personal leave to seek physical therapy on 02/05/14 for this injury.

While building a block stopping employee was standing on a ladder stacking blocks on the stopping wall when he lost his balance and fell approximately 4 feet causing 2 fractures in his left ankle.

Employee was chipping clay out of a chute when he got his left pinky finger caught between the axe and the chute, causing a laceration to his pinky which required 3 sutures to close the wound.

When dismounting fork lift employee stepped on a board twisting his left knee.

While unloading belt structure off of the trailer, employee's glove got caught pinching his hand between two pieces of structure. No fracture but employee does have some restrictions through November 1st.

Employee jammed his neck when getting into his hauler.

Employee stated that his shoulder had been bothering him for some time. After review from a physician it was determined that employee would need surgery to correct his shoulder. This will be degree six.

While cutting bands that hold the roof mesh together employee got a laceration to his left forearm, that required stitches to close the wound.

While trying to unplug a 6" drain pipe for the washbay employee suffered a hernia. They were using a 1" hose as a snake to flush out the 6" line. The accident happened on 06/10/2013, he went in for surgery on 07/09/2013.

Employee while taking down monorail chain slipped loose on the other end causing the monorail to swing down pinching his left thumb between the two pieces of monorail, requiring 5 stitches to his left thumb nail bed. Employee returned to work his next scheduled shift on light duty.

Original back injury occurred back in 2007, then in may of 2011 we took this employee back into the ER for back spasms, employee felt pain on the 18th of May but at that time he didn't want to see the doctor. We took him to the ER on 05/20/2013 where he was placed on light duty until 05/27/2013. Note this employee didn't have anything that realated to a specific incident.

Took employee in to have his right wrist looked at after a tire for the road grader popped off the rim while airing it up. The tire was in a protective rack and chained off. At that time the Doctor asked for a follow up exam, took employee back in on 05/29/2013 where he discovered a fracture.

Due to Shoveling under CNV 2 repeated motion to an old injury. Right Shoulder. Went to orthopedic on 04/26/2013 where he was placed with some restrictions 40 hour work week and lifting restrictions. He is still performing his normal duties and has not lost any work days.

Trying to get a pin into place on a Continuous Miner had another employee swing the double jack to get the pin in, he had his left finger in the back of the pin hole with one swing the pin went clear to the back of the pin hole causing it to slam into his finger, causing a fracture to the left index finger.

Employee was installing a roof bolt when he went to adjust the roof bolt plate he got his finger caught between the mine roof and the roof bolt plate.

Carrying a bucket of tuff stuff from kithen to shuttle car, slipped on slick floor spraining right knee.

While loading cleaning supplies from the warehouse to a pickup, while walking felt something pop in her foot. After she left work she went to see her physician were it was discovered that she had a broken bone in her right foot.

Hauler door swinging shut went to catch it and punched the door unintentionally in an attempt to catch it. The accident happened on 10/12/2012 took EE to the doctor on 10/23/2012 after he complained of his hand still hurting where they found a fracture of the right pinky finger.

Carrying bucket of tuff stuff from kitchen to shuttle car, slipped on slick floor spraining right knee.

While walking employee slipped on wet metal and fell into the base for the head roller.

Removing scraper box from head of BC-107 belt conveyor when the box slipped pinching the employee's right hand, which gave him a pressure laceration to his right middle finger.

No Accident time determined. Employee allegedly strained his right knee while pulling a 14/3 electrical cable. This alleged accident was not reported to his supervisor as required. During investigation of alleged accident, his co-worker was unaware of any incident by employee. Operator concluded no incident occurred. Employer made aware of alleged incident on 6/29/12.

Loading cable bolts from a Wagner scoop to the roof bolter the employee strained his back. This employee originally injured his back in January of 2012 did not seek medical attention until June 19th of this year that is when it became a reportable accident.

Roof Mesh standing up against rib started to fall over, the employee allegedly tryed to stop the mesh from falling by putting his shoulder up against it. We do not believe this to be a work related injury , and is under investigation by Rock Wood Casualty.

Pulling lift cylinders down to the roadway from beltline to roadway.

Shoveling the off side of BC-103 belt and twisted right knee

Packing an acetylene bottle from one location to another, set the bottle down to adjust it, felt pain in his lower back.

While walking slipped on a rock, tried to catch himself and hurt his right knee.

Rearranging belt structure in xcut strained lower back.

Right knee has been stressed from weak left knee. Going through a man door at xc 43 planted left foot turned in though door twisted right knee.

Went to pivot on right foot and toe of boot caught a chunk of coal. Causing twisting of knee.

The employee was setting a shield on the new long wall face. Before the shield could be set, the side shield needed to be pulled in. EE was kneeling down and had his hand in wrong position. Another employee closed the side shield after giving warning and caught EE's hand between the shield and side shield.

The employee was installing a roof bolt underground and caught his little finger between the clamp and the bolt. ***MSHA reportable injury***

The employee was helping load a piece of 5/8 inch sheet metal into the loader bucket and move it to the surface shop. The employee had the bulk of the plate in the bucket when an edge of the plate hit the bucket and bounced back and cut the employee's leg. The laceration he sustained required stitches. He returned to his regular job. MSHA reportable.

The employee was operating a scoop underground and was articulating the machine when he placed his hand between the door and the frame. The tire of the machine hit the door and pinched his finger between the two. This caused a laceration to his right little finger which required sutures. The employee returned to his regular job. MSHA reportable.

The employee was using a razor knife to cut a plastic cap off the end of a pipe. He was pulling hard when the knife slipped and cut this left index finger. MSHA reportable injury.

The EE was on his knees wedging a corner piece into the bottom of a crib to stabilize it, when a crib block fell off the top, about 8-9'up and struck him on the head. The EE did not request to see a Dr. until 02/08/11 at which time an x-ray determined he had a fractured nose. EE RTW .

The employee was walking through a crosscut in the 10-Left section with another employee walking behind him. The second employee lost his balance, tripped and fell on him, which resulted in the first employee breaking his left leg in two places.

The EE was underground packing a roll of Hueskier on his shoulder, when he slipped and fell. The floor was wet, slick and uneven and his left leg was folded under him when he landed. Fractured left leg in two places. ****MSHA LOST TIME ACCIDENT ON 9/4/10****

On 09-28-2010 we received substantiating information from Dr. that an employee sustained a new injury. On 08-26-10 the employee was helping install a bottom roller on BC-101, as he pushed the belt and lifted the roller, it caused his shoulder to dislocate. Restricted duty 08-27-2010.

The employee was shoveling coal off the 1st floor of the sample tower and wheel barreling it outside. His left shoulder was hurting him after shift. The employee was placed on restricted duty 9/7/10 after seeing a doctor. MSHA restricted 9/8/10.

The EE was hanging rib mesh underground. He was looking at the roof and stepped on a piece of coal, which caused him to fall to the ground and land on his left knee.

The employee was helping change out belt rollers. He picked up and handed a top roller to another employee when he reported that he felt a sharp pain in his lower back. This injury was determined on 1/18/2011 by an Independent Medical Examiner to be a new industrial injury. MSHA Lost Time Accident 10/25/2010.

The employee had a ladder set up outside the BC108 building. He was almost to the top disconnecting a conduit with a pipe wrench. The pipe wrench slipped and he lost his balance and fell off the ladder. He first hit a snow cover which was below him and then he hit the ground. MSHA lost time accident***05/28/10.

The EE works at our Rail Road facility. EE was pulling on a release pin with a pair of channel lock pliers. The pliers slipped off the pin and struck the EE's tooth, breaking it in half. The tooth did not need immediate attention, in that the nerve was not exposed.

The employee was underground hammering with a pick hammer when a sliver broke off and struck him in the arm

The employee was helping take off the headgate cover. He was pulling on a stuck bolt while another employee was using a sledge hammer. The sledge hammer glanced off the cover and struck the employee in the cheek. MSHA reportable.

Five miners were connecting pipe together on the ground. They then lifted the one continuous piece overhead to hang from the roof. This employee was the tallest and probably endured the bulk of the weight. He complains of neck pain. On 3/13/2010 he was seen by a dr and put on light duty. MSHA restricted duty 3/15/2010

The employee was working in the section when he stepped on a loose piece of coal and rolled his left ankle, causing it to pop. ****MSHA restricted duty****2/22/2010.

The employee was helping lift a screen out of 34A in the preparation plant. One of the side bolts fell off the screen and struck him in the corner of his eye causing a laceration requiring sutures.

The EE was lifting belt structure (frame on skids) and overexerted himself. MSHA Restricted Duty 11/02/09

Walking down BC-101 belt line doing inspections and tripped over a water hose. Fell on right hand causing pain in the elbow shoulder.

While helping lift 4 rolls of electrical wire spools onto the wire stand, (The stand is app. 40" off the ground) he pulled up on the pipe stand causing a sharp pain in his right shoulder. The EE was seen by a doctor on 9/15/09 and placed on light duty. MSHA restricted duty 9/15/09.

The EE was holding the multi-cable hook out of the way, while the roof bolter was trammed out of the entry. The roof bolter slid in the mud and swung over and smashed his left ring finger.

The EE was wrenching on a bolt. The wrench slipped off the bolt, causing him to strike his hand on the frame of the shuttle car.

The EE was driving a scoop around the surface shop when he ran into a ditch causing him to bounce up and hit his head on the cab of the machine.

EE was pulling cable for the roofbolting maching. EE felt left shoulder pop and had a lot of pain.

As the employee was roof bolting a rock ripped through the plastic mesh and struck him on the right forearm. Sutures were required. MSHA Reportable

The employee was using a pipe wrench to remove a stuck drill steel in the chuck of the roof bolter. He hit the rotation lever by mistake and the wrench spun around and struck him in the face.

While helping guide a 50'X 7' culvert across uneven ground, the EE stepped into a soft spot and lost his balance. He caught himself as he was falling but in the process he jammed his left leg and knee. The EE continued to work full duty through 07/16/09. MSHA Lost Time Accident 07/17/09

The employee was moving longwall shields in when a rock approximately 12" round fell from between the shields and smashed his finger against the cover of the tailgate drive.***MSHA Reportable***

The employee was helping hang vent tubing in the section. He turned and walked back to get some wire to tie the tubing together when the joint slipped apart and the vent tube struck him on the left shoulder.***MSHA Restricted Duty 4/7/09***

The employee was at the upper material yard on the surfae when he lifted a timber and felt a burning in his groin area. He continued to work until 4/9/09 when he went into the emergency room. He was diagnosed with a hernia and taken off work. *** MSHA Restricted duty - First day out 4/9/09***

The employee was helping unload a 4 wheeled cart from the back of a pick bed. He didn't have a good hold on the cart and the cart fell and smashed his fingers between the cart and the bed of the truck.

The employee was hanging sample tubing. He was standing on a ladder that was leaning up against a roof support can. The ladder slipped and the employee's leg got twisted up in the ladder and he fell to the ground. Restricted duty 3-21-09.

The employee got off the John Deere Skid Steer, and stepped between the bucket and frame when his left foot slipped out from under him, and he twisted his right knee. He requested to see a dr on 3/3/09 and was placed on light duty

The employee was bolting on the right side, the bolt was at an angle and he was using a wrench to hold it. The bolt spun and caused his finger to be smashed between the jaws and wrench.

The employee was packing a roll of huskar (mesh) to the face. When he set the roll on the ground he felt pain in his upper back. MSHA restricted duty 2-2-09

The employee slipped on the slot storage steps and grabbed the handrail to regain his balance when he felt a pop in his right shoulder and he became sick to his stomach. MSHA restricted duty 2/2/09.

EE was preshifting the underground section when he stepped in a hole and twisted his left knee as he fell down. The EE continued to work regular full duty through 06/21/09. MSHA lost time accident 06/22/09.

The employee was replacing foam insulation on the pump piping at the alluvial wells. He was cutting out a new piece with a razor knife. He was pulling the knife through the insulation, when the blade went all the way through his leg. ***MSHA Reportable***

A coal rib where the employee was working sloughed off. The employee ran out of the way when he twisted his knee and fell down at which point a piece of coal hit the same knee. Restricted duty on 10-8-08 and became lost time 10-9-08.

Roo ffall was due to weak mud stone top. Fall was at #7 x-cut. #2 entry of 8th left section. No damage and no injury.

While the employee was installing 12 foot roof bolts he felt a pop in his right shoulder. His shoulder still hurt Monday 09-22-08 and he decided to see a doctor and was placed on restricted duty 09-22-08.

The employee was pulling out a crib block from under a stabilizer jack, from the miner, when he smashed his finger between the crib block and a rock.

The employee was hanging cable bolts in the section when he reached overhead and felt pain in his left shoulder.

The crew of 8-Left section was headed out of the mine in the mantrip when an employee became angry, he turned around in the cab of the truck and knelt on the seat at which time he punched out the window between the cab and bed of the mantrip with his fists. The window shattered and he cut his forearms. Stitches were required. MSHA reportable injury

The employee was walking to the section when his knee became irritated and painful. He has had problems with his knee popping out of place in the past. On 10-08-08 the employee had surgery. MSHA lost time accident 10-08-08.

The employee was stepping down off a ladder after completeing tail chute work. He was holding a partial bag of rock dust in his arms. He missed the bottom rung of the ladder and fell to the ground straining his right knee in the process. MSHA restricted duty 7-23-2008.

The employee was traveling back underground with a load of material when the brakes failed on the truck. The truck ran into the left rib causing the steering wheel to spin around and strike his right hand. MSHA restricted duty 7-19-08.

The employee was unloading monorail hangers from the back of a flatbed truck into a crate. She had two hangers in her left hand and was swinging them to the rear at which time she felt a pop in her left shoulder. She later complained of sharp pain from neck to wrist. MSHA restricted duty 7-19-08.

The employee was lifting a roof jack when he felt a pop in his shoulder. He continued to work his normal job until 09-03-08 when he saw a specialist. He returned to work light duty on 09-04-08. MSHA Restricted Duty 09-04-08.

The employee was pulling on a Kennedy panel and cut his left middle finger on a sharp edge. First aid was given at the mine. The laceration would not stop bleeding and he went to an emergency room on 5-21-2008. Doctor applied derma bond and the bleeding stopped. ***MSHA reportable injury

The employee picked up a belt bar and spindle to un-hang it when he felt pain in his lower abdomen. He continued to work and then on 8/26/08 went in for surgery to repair hernia.

The EE was getting down from the roof bolter. As he stepped down, he miss judged how far it was to the floor and when his foot hit the floor, his knee buckled and he fell to the ground. He finished out his shift and worked the next day's shift. He then wanted to see a dr. On 4/9/08 he was seen by a doctor and released to light duty. MSHS restricted duty on 4/9/08.

The employee was walking back to the longwall face after lunch when he slipped and fell down. He punctured his left forearm on a piece of wire during the fall. Sutures were required to close the laceration. MSHA Reportable Injury 4/3/08.

The employee was helping move the panline back so a broken dog bone could be fixed. His foot got caught between the toe of the shield and the panline. MSHA lost time Accident 4/3/08

One employee was hammering a roll pin into the connector link on the longwall. A piece of metal flew off the pin and struck the supervisor. The piece of metal imbedded into the bridge of his nose.

The EE was hanging miner cable on the high voltage hanger. The miner cable swung back and one of the smaller hooks, on the hanger, hooked him in the nose and it ripped out when the hanger swung back. ***MSHA REPORTABLE***

The EE was moving 7' resin bolts to the bolter. A loose band that had been cut was protruding and cut the EE's right forearm/wrist. Sutures were needed to close the wound. ***MSHA REPORTABLE***

The EE was installing a 4' roof bolt. He attempted to connect the wrench with the drill steel. His hand slipped off the joy stick, causing him to activate the steel clamp, which closed on his right thumb and middle finger.

Three EE's were installing a new crusher guard. They were using a come-a-long to pull the guard up and into place. The guard was sitting to far from center and was going to slide back. So the EE got behind the guard and gave it a shove to center it up. In the process of pushing, he felt severe pain in his lower back. MSHA lost time accident 1/28/08.

The EE was walking to the warehouse to begin her shift. She slipped and fell on the ice and caught herself with her left hand.

A roof fall occurred at 7-Left section, #2 entry at crosscut #34. Height of the fall was 9 feet high, 20 feet wide and 50 feet long. No workers were in the section during the fall. It was found during a preshift examination and reported to MSHA by phone.

The EE was standing on the roof bolter hanging tube wrap. The EE hopped off the bolter & when he hit the floor, he slipped & twisted his ankle.

The employee was beating on a drill steel with a cable bolt wrench when he experienced a sharp pain in his left elbow.

Employee was loading monorail onto belt; as he threw the monorail up the end caught a belt chain & threw it back. It struck his middle finger between the monorail & the belt rail.

The employee was standing on a shield that was activated. His foot was caught between the shielf toe and the panline.

The employee was dropping monorail in the headgate of the longwall. As he let the monorail down it swung forward. As the monorail swung back it struck the ladder the employee was standing on and knocked it out from under him. He landed on his back on some rocks. MSHA lost time accident 04-04-07.

The employee was teaching an electrical class to our Mechanic/Electrician Trainees when he became disoriented, slurred speech and unable to stand. He was immediately taken to the emergency room in Rangely. Later that evening a doctor indicated he had suffered a stroke.

The employee was hooking up a trailer (in the surface yard) to his hauler. He was using a handy man jack. The employee's hand slipped off the handle of the jack and the handle struck him in the head. A laceration above the right eye needed butterfly sutures. ****MSHA reportable injury.

The employee was parking in the mine parking lot before work. As they went to get out of vehicle, they slipped on the ice and fell down. EE stood up and had a hold of the truck when they slipped again and tried to hold themselves up causing a strain to the back and groin.

The employee was removing potting compound from a signal cable. He was using channel locks to hold the cable and a razor knife to cut away the compound. The pliers slipped and he cut his right ring finger with the razor knife.

The employee was roof bolting and pulling the drill steel out of the roof, it slipped and smashed his finger against the bolter. ***MSHA restricted duty 02-12-07.

The employee was changing a tire off a trailer located in B-Mains xc19, underground. When he stood up he lost his balance and stumbled into the trailer and struck his right side rib cage into the trailer. ****MSHA Restricted Duty 02-06-07.

The employee was lifting the cover on the continuous miner when he felt a sharp pain in his right shoulder.****MSHA Restricted Duty 01-30-07

The employee was working in the longwall section on the panline when a rock fell from the roof and struck him in the back of the neck and pinched his left forearm on the panline.

EE was cutting mesh out. He was sitting in the mustang skid steer when a rock fell from the roof. The rock glanced off the mustang skid steer bucket and fell into the cab, hitting the EE in the right knee. MSHA restricted duty 11/17/06.

Employee was using a grinder to clean up a torch cut on a conveyor bent. The grinder kicked back and struck the employee's cheek. Employee was wearing proper safety equipment (shield, safety glasses, & gloves).

A roof fall occurred in the crosscut #48 of 7th left section. The fall was above the anchorage point. The fall was estimated at 17' wide, 20' long & 20' high. The fall is a result of loose laminated shales. This was the case above the anchorage zone also. The fall was cribbed off & will not be cleaned up.

The roof in cross-cut #26 of B-Access fell approx 17' wide x 8' high. This fall is a continuation of another fall. The area is timbered off on all accessible sides. The fall was above the anchor zone.
