EE thinks lower back pain is due to EE swinging a 20# sledgehammer driving a pin out of an outrigger on a forklift. EE thinks the torque of the swings resulted in a tightness in lower back that became painful a few days later.

Employee was steam cleaning a 980H loader and when finished employee was dismounting the machine and the bottom rubber, strap, step broke causing the miner's shoulder to strike a platform above bruising it. Report shows a contusion with restricted use of arm for 5 days.

Employee was using a pry bar to help open a barn door dump bed and when door opened employee leaned forward straining lower back muscles. Stiff/sore lower back & right hip.

Employee was pulling on a cable (drag cable) trying to situate it in a draw of the NW 95 drag line and subsequent to these actions complained of a burning sensation in right forearm. (Replacing the dump cable on the dragline)

Employee reached down to pick up a hose and said they tweaked their back. Employee was going to place the hose nozzle in a position to wash sand from beneath a tail pulley.

Employee was exiting the water truck after watering the work areas. Employee said when exiting the vehicle their elbow struck the door of the H2O truck causing a bruise to elbow. Employee did not slip or fall to cause the contact with the door.

Employee was leaning into the engine bay (beneath the hood) working on the engine. When EE let self down to the floor EE twisted knee.

EE was airing up a tire on a truck when the tire blew. The concussion of the explosion caused his ear to ring. EE has not missed any days or had any restrictions but just today we finally received a report from the insurance company that he has a 13% impairment in his left ear.

Employee was cutting an old splice from a belt and when the knife went through the end of the belt the blade turned (due to a sag in the belt) cutting the employee's thumb.

EE lost his balance when dismounting a screen and when he grabbed a pole, he twisted at the same time and tore the labrum in his left shoulder.

EE had bent down to examine a machine on a concrete floor and when he got back up to his feet he said his knee was bothering him. He did not fall or twist the knee, either kneeling down or coming back to the standing position.

Employee was using an angle grinder with a wire brush to remove rust from a steel plate when his glove got caught in the wire wheel. The wheel pulled his left index finger into the wheel by the glove. No skin breaks but slight fracture in middle phalange.

Employee was carrying tools to his truck when he tripped over an extension cord on the shop floor. The employee landed on his left elbow causing bruising.

EE was grinding pipe at the wash plant in prep to weld. The grinder got into a bind, jumped out of the pipe striking his left thigh. The injury was seen in the ER & the Dr thought he would leave it open to heal. (non-recordable) Recently the wound has flared up & the Dr wants to remove the object that must be present making the accident reportable now. (Surgery today 8/24/06)

EE was cutting a piece of rubber V-flashing to fit beneath a screen when he cut the tip of his index finger.

EE STATES HE SLIPPED ON ICY CATWALK (NEVER FELL) AND STRAINED HIS BACK BACK IN DECEMBER (NOT SURE OF DATE)BACK IS NOW BOTHERING HIM (REPORTED TO COMPANY 2/2/06) HE WAS WALKING CATWALK INSPECTION BELT LINE. HAD THE ACCIDENT BEEN REPORTED WHEN IT OCCURRED THIS FORM WOULD HAVE BEEN FILED THEN.

EE WAS WELDING ON A CONVEYOR AND AFTER A TIME HE USED A FORKLIFT TO MOVE THE CONVEYOR. WHEN HE RETURNED TO CONTINUE WELDING HE FORGOT HE HAD MOVED THE CONVEYOR. WHEN HE TURNED AROUND TO PUT ON HIS WELDING SHIELD HELMET, HE BUMPED HIS FOREHEAD AGAINST FORKTRUCK FORK, CAUSING HIM TO CLENCH HIS TEETH. THIS CAUSED A TOOTH TO BE DAMAGED.

EE DISCOVERED A KNOT IN HIS LOWER ABDOMEN AFTER WORK ON THE EVENING OF 7-6-04. WHEN ASKED TO LIST AN EVENT THAT COULD HAVE LED TO THIS HE COULD NOT PROVIDE ANYTHING. HE SAID HIS JOB ALWAYS HAS PHYSICAL LABOR PRESENT BUT HE COULD NOT ATTRIBUTE ANY SINGLE EVENT TO THE OCCURRANCE OF THE KNOT.

EE WAS CLEANING TRASH AND BRUSH THAT HAD BEEN THROWN ON THE COUNTY ROAD WHEN HE STRAINED HIS BICEP PICKING UP SOME BRUSH IN A TANGLE INJURY WAS NOT ON THE MINE SITE OR DID NOT INVOLVE MINING A CTIVITIES.

EE HAD PLACED A PIECE OF AR PLATE STEEL ONTO THETIPPLE TO BE WELDED IN AND HAD PLACED ANOTHER PLATE ON IT TO HOLD IT IN PLACE AND THE PIECE FELL OFF STRIKING THE EE ON THE FOOT, BEHINE THE STEEL TOE PROTECTION. THE BLOW SHATTERED THE LITTLE TOE ON HIS LT. FOOT.

EE WAS ATTEMPTING TO PULL THE LEVER ON HIS DUMP BED AND IT STUCK. HE THEN PUT A 2" CHEATER PIPE TO GAIN LEVERAGE AND WHEN THE LEVER WENT OVER CENTER THE PIPE CAME DOWN AND CUT HIS LEFT SHIN.
