Employee was exiting a storage building at an abandoned mine site. When he exited the door he slipped on a layer of ice. He dislocated his shoulder. The shoulder was reset by the hospital physician. He was released to return to work. He now has been given work restrictions and will seek further care. Is scheduled for an MRI on April 3.
I.E. was checking master switch on #12 Getman tractor when it suddenly rolled forward pushing his left leg under a mantrip that was parked in front of it. The injured sustained a fractured left fibula. The incident occurred on the 3 M.E. at XC #47.
The injured was operating #61 continuous miner when a piece of rock measuring 3' X 5' X 6" thick flipped out of the conveyor striking him on his left leg causing a fracture to the tibia and fibula that required surgery to stabilize. The incident occurred on MMU-003
IE was backing a tank duster on the travelway using a Getman Tractor, as he attempted to set the duster over the tractor articulated and caught his right elbow between the cab and back section of the tractor. The injured suffered a fracture to his humerus (funny bone) that will require surgery to repair.
At 10:17 am on 5/31/2012 the mine encountered a failure to operations to our elevator. After troubleshooting the elevator, it was found to have a blown control fuse. The fuse was replaced and the elevator was put back in service at 1:00 pm on 5/31/2012.
The I.E. bolting glove was caught between his bolting wrench and the drill chuck on the #29 Roof Bolter when he engaged the rotation. His left arm became entangled with the wrench. He sustained a closed fracture to his left ulna and radius.
The injured was struck by a bundle of roof plates that he had requested his partner to toss to him, they struck the knuckle of his left index finger causing a laceration and tendon damage.
The injured employee was traveling up the South Slope operating a Getman Tractor. The tractor suddenly lost all power and the brakes failed to work. The employee and tractor traveled backwards down the slope and struck a coal rib damaging the tractor. The employee suffered a contusion to his left shoulder and missed 4 days of work.
The injured was walking next to the #38 Roof Bolter preparing to spot bolts on a corner. A piece of roof rock measuring 48" X 19" X 9" thick fell from the mine roof causing the injured to fall to the ground. He suffered a compound fracture to his left middle finger. The incident occurred on MMU 001-0, XC #152, between #7 and #8 entries.
The injured was operating #96 wallace mantrip, the vehicle hit a bump in the road way causing the injured to strike his head on the canopy. The injured was treated for a neck strain and released without restrictions. On 7/13/2012 he was taken off work due to headaches and diagnosed with occipital neuritis.
A roof fall was discovered on the Main South belt line Entry #4. The fall measured approx. 30' X 19' X 4 6' thick. No reportable injuries, ventilation and escapeways not affected.
Hart Street Portal elevator computer experienced a malfunction. The computer was unable to determine the location of the elevator in the shaft. The computer was reset and the problem was corrected. A functional test run was performed and the elevator was returned to service
trying to jack a 14 person mantrip up to change a tire and strained his back
The injured was operating a continuous miner in Unit #1, entry #7L, XC109. A piece of roof rock measuring 52"W X 9'L X 1" struck the injured on his left side including his hard hat. The injured suffered a laceration to his skull that required three sutures to close.
Injured employee was caught between airlock door and diesel mantrip. Employee suffered non displaced fracture to right femur.
At 7:50 am the hoist at the Hart Street Portal failed to operate. At this time it is unknown the cause. The Secondary escape way has been re-routed to the South Portal.
IE attempted to couple an empty oil trailer to a tractor on Unit #1. As he picked up the trailer tongue, the trailer moved downgrade. His right leg was caught between the oil trailer and the tractor. A chock became disloged from the empty trailer when a loaded trailer was spotted beside it. He sustained a compound fracture to the tibia. An EMT on Unit #1 administered first aid.
The injured was helping lift a rub rail back on #65 miner when he strained his back. He worked until 9/15/2011 when he was not able to perform his duties.
Reportable roof fall - Main South, #3 entry, #52 cross cut. Dimensions: 60 feet in length, 20 feet in width and approximately 8 feet in height. No injuries. No property damage. Ventilation not affected. Escape ways not affected.
Reportable roof fall - Main South, #4 entry, #52 cross cut. Dimensions: 40 feet in length, 18 feet in width and approximately 5 feet in height. Fall discovered at 1818. No injuries. Conveyor components damaged. Ventilation not interrupted. Movement of material and people not affected.
Outer door switch located at the bottom landing of the Hart Street Elevator did not functioning properly. Safety parameters operated properly resulting in a audible alarm and an associated PLC alarm. MSHA notified. Received call from District 8 Representative. Replaced switch. No injuries.
Reportable roof fall - Main South, #3 entry, #20 cross cut. Dimensions: 21 feet in length, 12 feet in width and approximately 7 foot in height. Fall discovered at 0458. No injuries and no damaged to equipment. Ventilation not interrupted. Fall restricted movement of material and people in the south area of the mine.
At 3:55 AM the elevator at the Hart Street Portal became inoperable. It is believed that an electrical short in the trailing cable is the cause. The secondary escapeway has been re-routed to the South Portal.

At approx. 1:20 am the elevator at the Hart Street Portal became inoperable. Upon investigation it is believed that the newly installed guttering system to help reduce the water in the slope came in contact with the trailing cable shorting it out.
At 1:14am on 5/26/2011 the elevator located at the Hart Street Portal failed to operate correctly. The incident was called into the MSHA "One Call" center at 1:33 am and MSHA was notified of a major incident with the elevator and the location of the cage was unknown. MSHA contacted the mine at 6:30 am
During an inspection of the Hart Street Elevator by MSHA Inspector a damaged place was discovered on the govenor rope near the bottom sheave. The hoist was immediately taken out of service and the MSHA One Call center was notified.
The injured tripped over a piece of 6X6 timber causing him to fall over the top of it contacting his right leg. The incident occurred while training an employee to change batteries on Unit #1. The incident caused a fracture to the right fibula just above the ankle.
Silver State Wire Rope arrived on the mine site to conduct semi annual rope test on the Hart Street Elevator ropes. At the conclusion of the test mine officials were notified of a 14.3% loss. The hoist was taken out of service and the secondary escapeway was rerouted to the South Portal. All employees have been notified of the change.
Employee diagnosed with epicondylitis (tennis elbow)required to have surgery on 3/30/2011.
An unintentional roof fall was discovered on the Main South Travelway at 9:16PM on 1/8/2011. The fall measured approx. 20' W X 70' L X 5' thick. The area was developed in April of 2000, and supported with 4' fully grouted bolts. This fall will not be cleaned up.
At 7:55 am on 12/31/2010, the elevator at the Hart Street Portal had the door pulley break. The secondary escapeway was rerouted to the South Portal and repairs were made. The elevator was back in service at 10:50am
A faulty over travel switch on the Hart Street elevator was discovered during an inspection by MSHA. The elevator was immediately taken out of service and repairs made.
Injured employee was working on #81 diesel mantrip and his left hand came in contact with the flywheel while the engine was running. Employee suffered avulsion to 4th finger and fracture to 3rd finger on left hand.
The mine encountered an unintentional roof fall on the Main South, XC68, between entries #2 and #3. The fall measured 17'W X 15"L X 4' thick. The area was bolted with 4' fully grouted bolts in May of 2000. This fall will not be cleaned up.
The injured was operating #85 Damascus, battery operated 2 man vehicle. He and a passenger were traveling down the South Slope. The vehicle struck the corner of the #9 shelter hole causing the injured to strike the steering wheel suffering injuries to his chest. He was transported by ambulance to Good Samaritans hospital, and later transferred to Deaconess.
The injured was a passenger on a Damascus MAC-XP ride that struck the #9 shelter hole in the South Slope. He was transported to Good Samaritans Hospital and later transferred to Deaconess in Evansville. He was treated for a laceration to his upper lip and and an avulsion to his left leg and released.
The mine encountered an unintentional roof fall on the 5A, XC32, from #0 entry to #2 entry. Dimensions: 160'L X 18'W X 8'T. Area developed in 10/2007 and supported with 6' fully grouted bolts. The fall will not be cleaned up. Approaches shall be timbered and dangered out.
The mine encountered a roof fall on the Main South Travelway, entry #3, XC #22. The fall measured 5' thick X 21' wide X 27' long. The area was initially bolted with 4' fully grouted bolts. The fall affected the haulage of supplies from the South portal. The fall area will be resupported for travel.
The mine encountered an unintentional roof fall on the MainSouth travelway, entry #3, between XC #49 and 50. The fall affected transporting supplies from the South Portal. The area was mined in March of 2000 and bolted with 4' fully grouted bolts. The area will be resupported.
The mine encountered an unintentional roof fall on the Main South, Entry #4 (belt), between XC#62 and #63. The fall measured 40'L X 20'W X avg. 5' thick. The area was developed in April of 2000 and supported with 4' fully grouted bolts. Supplemental support consisted of 6' fully grouted bolts. The fall will be cleaned as per approved site specific plan.
An unintentional roof fall was discovered on 6/30/2010 on the Main South, XC #20, between entries #3 and #1. The fall measured 110' L X 18' W X 5' thick. The area had been supported with 4' resin bolts. The fall will not be cleaned up. It did not effect ventilation, travel, or escapeway.
The injured employee was removing boom canopy from Fletcher bolter when it fell off striking him on the right lower leg causing a fracture to his tibia. he was transported by ambulance to local hospital.
IE was struck on the left little finger by a hammer while making a belt splice.
The mine discovered a roof fall on the Main South, entry #4 at crosscut #50 - #51. The fall measured 35' L x 18' W x 4' T. The area was developed in 3/2000 and supported with 4' resin bolts. The fall shall be cleaned up.
Employee was loading roof plates onto roof bolter. He caught his right middle finger between two bundles of plates. He sustained a laceration to finger requiring seven sutures to close and a fracture to the end of same finger.
EE, a roof bolter on Unit #1 was on the operator side of #38 Fletcher bolter in Entry #9. He walked to the back of the machine to get a bag of rock dust. He stepped in a hole/depression in the mine floor twisting his right knee. He informed his Supervisor. EE was transported by Company vehicle to Good Samaritan Hospital where he was diagnosed with a right knee strain.
The mine discovered a roof fall on the 4 Main West, entry #3, at crosscut #151. The fall measured 18' X 18' X 7' thick. The area was developed in 2/2007 and bolted with 6' glue bolts. The fall will not be cleaned up. The 4 Main West is being recovered and will be sealed.
EE reported soreness in both wrists on 1/7/2010, was seen by a physician on 3/5/2010. Has exhausted non evasive treatment. Carpal tunnel surgery to repair required on 7/22/2010, this will be the first shift the injured has missed.
During an inspection of the Hart Street emergency hoist, it was determined that the hoist would not swing away from the shaft after making a trip in and out of the shaft. The investigation revealed an electrical malfunction that prevented the hoist from swinging away from the shaft. Escapeways were re-routed and people notified.
A roof bolter operator received a laceration to the tip of the little finger on his left hand when his hand was struck by a falling rock while he was in the process of drilling a hole for a bolt on a recently trimmed corner. The injury required 8 stitches to repair.
A roof bolter operator fractured a metacarpal bone in his right hand when he was struck by a drill steel while trying to align the steel in his drill chuck. He pushed on the steel with his boom and when he released the boom pressure, the steel flexed back striking his hand, causing the injury.
Our examiners found a roof fall in the 5 Main North worked out area in entry number 3 just inby the intersection of crosscut 41 and it extended thru the intersection of crosscut 42. The fall was approximately 80 feet long, 18 feet wide and 20 feet high.
The injured, was cleaning belt structure on the 1 "A" belt at crosscut #1. He was using his gloved hands wiping dust off when his glove got caught between the troughing roller and belt. The incident caused a partial amputation of the ring finger of his right hand just above the first joint.
An unintentional roof fall was discovered on 11/3/2009 by mine examiners conducting their weekly exam of an abandoned area of the mine. The fall was located in XC #5, between entries 4 and 5 of the 1 Left/5 main North. The fall measured 18' L X 18' W X 8' Thick and was bolted with 6' resin bolts. The area was developed in 5/07.
The mine encountered an unintentional roof fall on 10/29/2009 at 8:15 PM on the 2M.E. Travelroad between XC#2 and #3, the area was developed on 12/28/07 and bolted with 6' resin bolts. The fall measured 17' W X 20' L X 6' thick, the fall may be cleaned at a later date.
Our examiners found a roof fall during a weekly examination of a worked out area. The fall was located in the 1R/4L/ 4 Main North panel in entry #4 crosscut 5-6. The fall was approximately 50 feet long, 18 feet wide and 6 feet high. The fall was located in an entry adjacent to the examiners route of travel.
At around 1:30 am the Hart street elevator failed to operate. The cause of the malfunction was determined to by a governor trip, which locked the brakes on the elevator. The governor was re-set and the elevator was returned to service.
This incident was a result of a storm that knocked power at the Hart Street portal. After power was restored the elevator would not operate. It required re-setting safety switches to resume operation.
The Hart street elevator was going out on nuisance trips on overspeed. The investigation revealed that there was some mud build up on sheave wheels which increased the sheave wheel size enough to create the trip faults. The elevator was operational but it was decided to shut it down to clean the sheave wheels.
During an MSHA investigation of a squeeze area located at the junction of the 1L/2ME submain and 1R/1L/2ME panel, a roof fall was found in entry #10 at crosscut 7. The fall was the result of the squeeze condition.
The injured was unattaching a belt scraper on the 2 Main East, while using a chain hoist to lift the scraper it popped loose pinching his right hand between the scraper and the top roller causing an avulsion that required 3 stitches to close.
Our examiners found a roof fall during their weekly examination of a worked out area located in the 1 Left, 1 Right, 3 Left, 4 Main North panel. The fall was located adjacent to their route of travel into the panel. The fall was located at croscut #7 between entries 4 and 5. The fall was confined to the crosscut and did not fall in an intersection.
At approximately 3:20 pm some people got on the Hart street elevator at the bottom of the shaft. They attempted to come up the shaft to the surface but the elevator would not move. The doors could also not be released to open them. The persons were trapped on the elevator until electrical switch repairs were made. The elevator was returned to service at 4:10 pm.
The injured was cutting a gear case off of a continuous miner, he was laying under the head of the miner when a piece of hot slag fell into his left ear. The injured sought medical treatment and returned to work. On 9/23/2009 surgury was required to close a hole in his eardrum.
The injured employee, a section manager on MMU 001 was rolling up curtain in entry #1 when a piece of roof rock measuring 52" X 36" X 1 to 3" thick fell from the roof striking him on his hard hat and knocking him to the mine floor. The injured sustained a compound fracture to his lower right leg.
Our hoist used to hoist a mantrip car at the Wheatland portal experienced a blown hydraulic hose while hoisting the mantrip car out of the mine. The brakes automatically set on the hoist and the hoist became inoperable. The hoist was repaired and returned to service.
The injured was a utility man on a continuous miner section, he started complaining of back and leg pain 7/14/2009. He sought medical treatment and continued to work until 8/13/2009 when he was diagnosed with Piriformis Syndrome, he continues to be off work.
During an MSHA inspection of the elevator at the Hart street portal, it was determined that accumulations of material on the elevator governor rope was preventing the brakes on the governor rope from properly stopping the governor rope. The governor rope was cleaned and the elevator was returned to service.
The injured twisted his right knee after stepping on a piece of roof rock on 6/22/09, he has continued to work. An MRI conducted on 8/20/09 shows the lateral meniscus "degenerated severely", and the medial meniscus is also abnormal due to degeneration.
The injured was roof bolting in Unit #5 in the 20+04 XC between Ent. 5 and 6. He was swinging his boom to install the right rib pin and caught his right foot with the drill pot, rolling it. The injured was diagnosed with several fractures requiring surgery to repair.
A roof fall was found by our examiners on the 3 West C roadway at crosscut 136. The fall was located in the intersection and blocked travel thru the area. The fall will not be cleaned up and the roadway will be re-routed around the area with additional supports installed in the area.
EE was working on a coal hauler charger at the unit 5 coal hauler stations. EE had gone to the power center and was returning to the charger, he stepped in a rut and twisted his right knee. He continued to work until 7/6/2009, when he underwent arthiscopic surgery to repair.
The injured was traveling in the 4WA belt entry when he stepped off the concrete pad for the drive he stepped on some uneven ground causing him to twist his right knee. An MRI revealed a torn meniscus.
The mine encountered an unintentional roof fall on 1 WA, XC #29, between entries #4 and 5. The fall measured 20'L X 18'W X 4' thick. The area was developed 3/1997, supported with 4' glue bolts with straps. This fall will not be cleaned up.
At approximately 4:25 am the Hart street elevator failed to operate. The investigation revealed that a limit switch located in the head house had water in it, which caused it to fail. The source of the water was believed to be condensation. The limit switch was replaced and the elevator was returned to service at around 6:00 am.
The injured was in the process of installing a roof bolt in the mine roof. He pushed the bolt into the roof as far as the drill boom would push it. He placed his extension wrench in the pot and was attempting to get it on the bolt head. He spun the wrench and it flew out and struck him in the forehead causing a laceration to it requiring stitches to repair.
A roof fall was found by examiners along the 3 West roadway at crosscut 36. The fall was located at the edge of the roadway between the belt and the roadway. The fall is between entries 4 aned 5.
Our examiners found a roof fall during their weekly examinaton of the 3 Left 4 Main North (5A) panel. The fall was located at crosscut 16 between entries 2 and 1. The fall was approximately 50' long, 18' wide and 7 feet high. The fall did not extend into either intersection and was confined to the crosscut. The fall was located adjacent to the examiners route of travel.
A belt person found a roof fall located on the 3 West A travelway at crosscut #9 in entry #7. The fall was approximately 30' long, 20' wide and approximately 8' high. The fall was a continuation of a previous fall. The area had been timbered but it overrode the timbers and fell across the travelway.
The cage doors on the Hart street elevator were damaged. The door cable broke and the door brackets got bent in the process. The elevator was removed from service while the door brackets were repaired.
The injured was in the process of installing tie wire on roof bolts for a power move. While installing a piece of tie wire a piece of rock fell from the mine roof and struck him in the upper lip. The rock fell from the area between the rib and the end of the wire mesh panel. The incident lacerated his upper lip and chipped a lower tooth.
The injured was walking down #3 entry on #1 unit when he struck his arm against a damaged roof bolt protruding from the mine roof. The injury required five sutures to repair.
The injured was in the process of installing an 8' bolt in an intersection. As he was drilling his hole, a wing curtain got caught on the drill steel and caught his left arm, jerking it toward the steel. The injured pulled his hand back away from the steel and this caused a laceration at the base of his two middle fingers.
Injured was removing packing from a conveyor motor lead with a pry-bar and struck his hand with a small sledge hammer in the process. His hand swelled and he continued to work. He sought medical attention on 1/13/09 and was removed from work on 1/20/09 because the swelling was not going down.
An examiner found a roof fall in the 4 Main North intake in entry #7 between crosscuts 166 and 167. The fall was located in the entry adjacent to the primary escapeway. The fall was not located in a crosscut, but was present in a pillar line.
Our examiners found a roof fall during their weekly examination of the 4 West return air courses. The fall was located in #2 entry and extended into the crosscut toward #3 entry at crosscut 116. The fall was located in an entry adjacent to the examiners route of travel. The area was timbered.
At around 4:55 pm the Hart street elevator failed to operate properly. The elevator failed to go up and down in the shaft. The investgation revealed that the brake coil failed and had to be replaced. After the coil was replaced the elevator was returned to service.
One of our examiners found a roof fall during his weekly examination of the 2 Main North seals. The fall was located at crosscut 12 of the 2MN from entry #1 to half way between entry #2. The fall has blocked access to the #1 seal.
The injured had finished installing his two roof bolts and was watching his buddy install his bolts. A rock fell from in front of the bolter ATRS flipped back and struck him on the left calf muscle area of his leg. The action bruised his calf muscle. He went to a follow up appointment on 12/5/2008 and was removed from work.
One of our examiners found a roof fall during the weekly examination of the 4 Main North return. The fall was located in entry #3 between crosscuts 9 and 10. The fall was located in the entry that the examiners normally travel. The fall was timbered and the examination route was re-routed.
Our examiners found a roof fall in the 4 West return during their weekly examination of the area. The fall was located in a crossut between entries 2 and 3 at crosscut 79. The fall was adjacent to the examiners route of travel but did not fall in either entry. The fall has been timbered.
Our examiners found a roof fall during their weekly examination of the 3L4MN (5A)return. The fall was located at crosscut 46 between entries 2 and 1. The fall was located adjacent to the examiners route of travel.
At around 10 :55 am the Hart street elevator failed to operate properly. The elevator cage brake dogs locked and would not release properly. This event occurred during an MSHA quarterly inspection of the elevator. The brake dogs were re-adjusted, greased and the elevator was returned to service.
The injured a section foreman was in the process of placing his gloves on the roof bolter when a piece of rock fell from the mine roof and struck him on his left hand. The incident fracured his left pinky finger and cut it. Surgery was required to repair the injury and a portion of his finger was removed due to damage.
Our examiners found a roof fall during their weekly examination of the 5 West worked out area. The fall was located adjacent to the examiners route of travel. The fall was located at crosscut 61 between entries 2 and 3.
Our examiners found a roof fall during their weekly examination of the 4 Main North returns between crosscuts 148 and 149 in entry #3. The fall was located in their normal route of travel.
Our examiners found a roof fall in the 4 Main North returns at crosscut 181 to 182 in entry #7. The fall was located adjacent to the examiners' route of travel.
Upon attempting to start the Main South belt for day shift production, the belt would not start and run. An examination revealed that a roof fall had occurred between crosscuts 48 and 49 on the belt and structure. The examination revealed that a slip was present in the mine roof that caused the fall.

An examiner found a roof fall on the 3 West A belt during his roadway examination. He started smelling rubber smoke and traced it to the area where the mine roof had pushed timbers into the wet mine floor. The fall was located at crosscut 25 in entry #4 on the 3 West.
A supply tractor operator found a roof fall on the roadway leading to #4 unit while taking supplies to #4 unit. The fall was located in the 4 Main North mains at crosscut 180 in entry #6. The fall was located in the alternate escapeway for #4 unit.
A roof fall occurred on the 4 Main North C belt at crosscut 135 to 136. The fall was located in the belt entry between crosscuts 135 and 136. The 4 Main North C belt is the belt for units 1 and 4.
Injured was roof bolting in entry #6, on unit #3, inby crosscut #21 when this incident occurred. He was bolting when the unbolted cut started to work and rocks started to fall. As he moved away from the bolting machine from under the bolter canopy a rock fell in front of him and landed on its end on top of his right foot, fracturing three metatarsal bones in the process.
A roof fall occurred on the 4 Main North at crosscut 179 in entry #4. The fall was located inby the tailpiece for the 4 Main North C belt. The area had been previously recognized as being weak and longer bolts and timbers had been installed in the area prior to the fall occurring.
A roof bolter operator was installing his second roof bolt in a row, when a piece of a coal rider fell from the mine roof and struck the canopy of his bolter. As he turned to run away from the machine, a piece of the rock broke as it hit the canopy and struck him in the lower back. The incident resulted in contusions to his lower back.
An examiner found a roof fall during the weekly examination of a worked out area on the 1 Left 4 Main North. The fall was located at crosscut 86, entry 2-3. The fall was located adjacent to the examiners route of travel.
Our examiners found a roof fall in the 1 right, 3 left, 5 West panel at crosscut 8 between entries 4 and 5. The fall was located in a worked out area. The fall was located adjacent to the examiners route of travel.
At around 10:45 am the Hart Street elevator malfunctioned. A brake shoe got out of place on the cage causing the malfunction. The elevator failed to stop at the proper level at the bottom landing and had to be raised manually.
Our examiners found a roof fall in the 4L4MN panel. The fall was located in #4 entry at crosscut 38. The fall was located in the next crosscut inby from the examiners route of travel.

The injured was operating a scoop and was in the process of pulling into a crosscut to get some boards to build cribs with. As he drove the scoop into the crosscut, he ran over some loose roof bolts and one of the bolts came up into the operators compartment and struck him in the mouth. The incident cut his lip and broke off four teeth.

The rock landed on the spinnig plate that holds the chuck. This caused the rock to fly off the drill pot and strike him in the mouth, breaking a portion of his left front tooth.

A roof bolter operator was in the process of adding an extension to his drill steels to finish a drill hole for a bolt. During this process, the drill steels fell from the mine roof, striking the injured on the left middle finger causing a laceration on it. The individual was wearing metacarpal gloves at the time of this incident.
