The victim was in the process of cutting through the inner left-side side plate. When the cut was completed, the cat frame pivoted upward, pinning the victim between the cat track and frame of the feeder. The side plates connect the hopper jack assembles to the cat frame. The victim was working on n Auxier Welding Inc belt feeder.
While walking to #1 entry, employee bent over to avoid a roof bolt hanging from the roof and slipped/tripped and landed on right shoulder. Dr. put off work with a strain until MRI.
While installing the outside pin, a rock fell out from above the coal seam and struck employee on the top of left foot. Fracturing 2 bones.
While tearing down line curtain at the face, employee's knee gave out causing EE to strain something. Dr. put employee off work starting on 8/26/19 until MRI can be seen.
While reclaiming belt structure, a bottom roller slid across the tray and smashed employee's right pinky finger. ***laceration***
While building a return stopping, employee felt sharp pain in right shoulder. EE worked until EE was put off work on 8/30/2019 due to surgery to try and clean up the area.
As the 16 person mantrip was leaving the unit, it turned left to go outby. Employee was sitting on the left side of the ride and had their left hand on the outside of the frame. The ride struck the coal rib and smashed employee's left hand. Employee suffered two broken bones.
While 6467A Continuous Miner was tramming through the intersection of #6 entry, the injured employee was also hanging the roof bolter cable from the mine floor. As the miner came closer, a miner bit on the drum inadvertently came in contact with the injured employee's right foot, causing a puncture wound.
When traveling to the unit on a golf cart, employee went over a hill and noticed a getman broke down in the road. EE could not stop quick enough and had to swerve to miss it. Employee fell off golf cart and strained left shoulder.
While reclaiming High Voltage, employee twisted left ankle and broke 3 metatarsal bones in their foot.
Injured employee was pulling conveyor belting with a cable and clamp to prepare a splice. The clamp came loose and struck the employee on the left heel.
While roof bolting, a piece of shale rock that was already on the canopy slid off and struck employee on the shoulder causing the need for 2 stitches.
While cutting open shuttle car cable, the utility knife slipped off the cable and cut left thumb. **Received 2 stitches**
While setting props on the beltline, employee swung axe to tighten and struck right hand on existing prop, cutting right index finger. ***2 stitches***
An unplanned roof fall occurred on the 4th 48 MW travelway crosscut 219.
While moving the miner cable over EE felt something pull in right shoulder, EE was moving cable over b/c it was too close to EE's Shuttle Car route. Dr. put off work until further test results.
While cleaning walkway in beltline, bottoms were soft, EE's right foot got stuck in mud and when EE turned EE twisted right knee.
Employee was using a hammer and chisel while working on a golf cart. When they struck chisel a piece of metal from the hammer broke off and embedding in left wrist. **restrictions after metal was removed**
Unplanned roof fall on the Main West travelway at crosscut 102.
The employee slipped while walking underground and twisted left knee. Began missing work on 9/14/2017.
The employee rolled ankle while walking.
The employee was hooking up the diesel duster to a Getman and got their left ring and pinky finger caught between the tongue of the duster and the hitch on the Getman. This caused a laceration and sutures were required to close the wound.
The employee was installing a new section of exhaust on a piece of diesel equipment. During the installation the new piece of exhaust cut through the employee's protective gloves causing a laceration to right middle finger. Sutures were required to close the wound.
A roof fall occurred in the 1B5 Panel, cross cut #32, entry 8-9. The fall is approximately 18' wide and the height is unknown.
Steel slipped out of his hand striking his finger. Damage being, individual had to wear a splint on his finger.
Employee was removing a crib block from a pallet, when one fell over mashing his right index finger. This caused a laceration which had to be closed with sutures.
Employee was roof bolting on #4 unit between #5 & #6 entries. When he was swinging the boom back in, it hit a roof bolt in the tray causing it to pop out of the tray hitting him in the right eye lid. This caused a laceration that had to be closed with sutures.
While riding in a man trip, the trip moved over to pass parked supply trailers. In doing so it caused employee to hit his left leg on the trip causing a laceration which had to be closed with sutures.
EE was under a Getman Tractor removing a one inch hydraulic hose. As the hose came loose it struck him in the mouth cracking a least one tooth.
Employee was using a hammer and chipping hammer to remove a bolt from a classified cyclone. A piece of the hammer broke off and struck him in left hand. The metal didn't have to be removed by surgery. *This accident should not have been reported.
While removing the outer reel bearing on a shuttle car with a hammer, a piece of hammer broke off and struck the mechanic in the right hand. The piece of metal had to be cut out and closed with sutures.
EE was pulling the head shaft out of the feeder. He had his left hand on the shaft. The other side of the shaft fell out of the feeder causing the end he had hold of to kick up and pinch his hand between the shaft and the feeder frame. This caused a laceration which had to be closed with sutures.
EE had finished bolting for the night and was just about to leave for the man trip when he realized he had left his wire cutters on the bolter. He reached back and grabbed the cutters, then turned around running into the edge of the ATRS on the bolting machine. This caused a laceration above the left eye which had to be closed with sutures.
As employee was walking along the West side of the 405 conveyor, he stepped in a hole, causing his right ankle to turn abnormally, causing a fracture to his ankle. He was treated with an ankle boot and returned to work.
Employee was changing a ball valve using a pipe wrench. As he was trying to tighten the valve the wrench slipped causing his right hand to strike the structural steel beam behind him. This caused a laceration which had to be closed with sutures.
Employee was cutting high voltage guarding with a utility knife when the knife slipped out of the guarding and cut his left thumb. The laceration had to be closed with sutures.
Employee was trying to match up his roof drill starter steel with the end pusher steel. As he was doing so, the web of skin between the thumb and index finger was caught between the two steels causing a puncture laceration which had to be closed with sutures.
Injured was assisting in construction of a MICON 120PSI seal and had his left hand resting on a partial constructed block wall, when a piece of rock from the roof fell striking his hand.
EE was trying to shear the head off a pin in a trailer hitch to get the pin out. As he was striking the punch with a sledge hammer a piece of metal flew from the pin and struck him in the left wrist. The metal was lodged, and had to be removed and sutured.
Employee was cutting the top of an oil can pulling the utility knife toward his body. The knife slipped and cut his left hand. The laceration had to be closed with sutures.
A crib had been built, and wire mesh panel had been removed. Injured miner was standing approx. two feet from the crib on the in by side. The rock broke loose, sliding off the crib and struck miner in left leg below the knee. The rock measured two feet in width, four feet in length and one to four inches thick.
On or about 7/14/2014 an unplanned roof fall occurred at cross cut # 59 on the 2nd Southwest Sub-mains at the approach to the #1 seal. The area was bolted with 8' and 10' roof bolts. The fall will be cleaned up. The fall was fractured laminated stack rock. The fall was approximately 18'Wide x 18' Long X 8' High.
While cutting a piece of ventilation curtain with a utility knife, the knife slipped out of the roll of curtain and struck his left leg. This caused a laceration which had to be closed with sutures.
While loading supplies on a roof bolter, employee raked his hand across a monster roof bolt plate causing a laceration on his left thumb. The laceration had to be closed with sutures.
While installing a roof bolt employee mashed his ring finger between the roof and drill pot. This caused lacerations that had to be cleaned and sutured.
Employee was resetting a breaker on a mantrip, under a cover that a co-worker was holding up. The cover fell down hitting him on the right hand causing a laceration that had to be closed with sutures.
Employee was winding the rope back on the spool after pulling on conveyor belt on a fresh belt extension. While he was in the process the scoop moved forward and his left foot was caught under the scoop bucket. This resulted in a fracture in his left foot.
As the employee stepped off the slope car at the bottom of the shaft he sprained his ankle.
While cutting a hole in a conveyor belt to pull old belt off, employee cut his left arm with a utility knife. The laceration had to be closed with sutures.
Employee was breaking loose the nut on the fan gear steering knuckle of #4012 shuttle car. The wrench slipped and struck him in the forehead and bridge of the nose. This caused a laceration which had to be closed with sutures.
Employee was installing roof bolts on #4 unit. A piece of rock fell and struck him on the right arm above his kevlar sleeve causing a laceration. This laceration had to be closed with sutures.
Employee and co-worker were making a mechanical splice on a roll of belt on the 1 A conveyor belt. EE was moving the plunger for the multidriver tool and was struck on his right index finger by a 4lb sledgehammer causing a fracture. Co-worker was driving the plunger with the sledgehammer to set the rivets in the mechanical splice.
Employee was changing cartridges in a grease gun in the slope belt entry. The hose got caught between the top roller and the belt. This pulled the grease gun and his right hand between the roller and the belt. This caused contusions and a fracture to his index finger.
The employee was putting a shovel on the back of the mantrip and a rock fell out between the roof bolts striking the employees right index finger. The laceration required sutures to close the wound.
The employee was raising his pinner steel to the roof to start drilling his hole. When the steel contacted the mine roof it caused a small piece of draw rock to break loose, contacting the employee's left arm. 5 sutures were required to close the laceration.
Employee was roof bolting when he struck his leg with a monster. This caused a laceration in the thigh area.
While unloading crib blocks, the employee was straightening the blocks and had his finger caught between two of the blocks.
Bolter operator was having problems with the dust collection system on the bolter. As the operator cleaned the hose under the pot. The utility man on the section noticed the problem and came to help. He stuck his hand in the pre-dump on the bolter as the operator sat his foot jack. This activates the pre-dump. This fractured his right arm.
While making a splice on a conveyor belt, the employee cut his hand with a utility knife.
On 10/31/2013 at approximately 13:25 an unplanned roof fall occurred at cross cut #106 on the Main West, entry #4, beltline. The primary means of support in the area was 6' point anchor bolts, and the supplemental means of support was 10' point anchor bolts. The fall was approximately 12' high. The openings approaching the fall are approximately 18' wide.
Employee was nailing a header to the roof, he lost his footing and fell off the platform, injuring his left shoulder.
While splicing a cable to the charger for a golf cart. The employee cut his finger with a utility knife.
The employee was hanging the roof bolter cable from entry #3 to #4. While reaching above his head to tie the cable up he felt a sharp pain in his back.
The employee was in the process of setting "I beams" for a new transfer point. Before the beam could be secured it rolled over and smashed the employees left middle finger. The injury required 5 sutures
Employee was unrolling a ventilation curtain that was rolled up to the mine roof. A small piece of rock had slid down into the rolled up curtain. As it was unrolled the rock slid down the curtain and struck employee in the fold of his right arm. This caused a laceration that was closed with sutures
A roof fall above the anchorage zone occurred in entry #5, crosscut #12, on the 1st 42nd Southwest conveyor belt line on 8/1/13 at approximately 22:40. The fall was approximately 25 feet in length, 12 feet in height, and all openings measured 17 to 18 feet in width.
The employee had drilled his first pinner steel in the mine roof. While in the process of adding his second steel, the steel in the mine roof fell out and hit his left pinky finger causing a hairline fracture.
The employee and his partner were bolting #9 entry at cross cut #94 on #1 unit. After moving forward the atrs against the mine roof. When doing so the atrs loosened a rock in the roof. The employee on the opposite operator moved to his drill station to swing his boom out and the piece of rock fell out on his left ankle. The rock measured 4' long X3' wide X 4" thick.
On 5/16/2013 an unplanned roof fall occurred at approximately 5:15 AM on the North Main travel way, entry #4, at cross cut #10. The fall was approximately 30' long x 19' wide x 10' thick. The permanent roof support in the affected area was 10' point anchor roof bolts.
The employee was in the process of making a permanent splice on a roof bolter cable. He had the trailing cable situated on his left leg and while cutting open the cable to make the splice, his knife slipped causing a laceration to his left thigh. The laceration required 8 sutures.
A piece of rock fell while employee was roof bolting and struck his right hand. Causing a hair line fracture to one of the small bones.
The employee was in the process of building a stopping. He bent over to pick up a block and received a laceration on his right forearm from one of the metal bands, the block comes bundled in. The laceration required 6 sutures.
The employee was riding on golf cart and the golf cart became high centered on rock dust. The employee got off the golf cart to assess the situation and stepped in a hole and rolled his ankle.
The employee was walking from #6 entry to #5 entry. While walking he stepped on a rock and felt a pop in his right foot.
The employee was traveling outby in his shuttle car in #8 entry. He over corrected his steering and his cab was about to go into the rib. When this happened he had a reflex, or an unconscious motion, to catch himself against the rib. When he stuck his arm out of the car it got caught between the canopy post and the rib.
The employee was in the process of cutting a left in entry #4. He went to throw his miner cable over and felt a sharp pain in his right shoulder.
While helping the roof bolter travel under a wing curtain to keep the curtain from catching on the ATRS. The pad of the ATRS hit the helper on the left leg. Causing a hair line fracture approximately 4 inches above the ankle.
While installing belt hangers the employee smashed his finger between the roof and a pin board.
Employee was reclaiming water line underground. As he was cutting the hangers that attached the line to the roof the line came back and hit him in the mouth.
The employee was roof bolting in #3 entry on unit #2, and was in the process of installing a two piece 8 foot roof bolt. During the installation process he heard a pop come from his right shoulder and then felt a sharp pain.
Employee was splicing a shuttle car cable. Utility knife slipped and cut his right leg.
During a unit move the mechanic was repair a miner cable. As he was re-installing the receptacle on the cable the unit low trac was pulling slack on another cable. As the low trac went by the mechanic the operator didn't see him and the low trac ran over his foot.
The employee was building crib, as he threw a crib block under a conveyor belt. He struck his finger between a crib block and the belt structure.
Employee was in the process of making a splice in the trailing cable for a shuttle car. He was splitting the two leads apart with a utility knife when his hand slipped, causing a laceration to his left index finger. This laceration required 3 sutures.
The employee was pushing a roof bolt into the mine roof with his left arm when he felt a sharp pain in his left shoulder. The employee was able to continue working at his normal occupation, with no restrictions, while attending therapy sessions. His Dr. decided he was going to need surgery in order to make a full recovery. He began missing work on 4/30/2013.
The operator of the 6010 roof bolter was reeling up the cable slack left after pulling into the face. His partner was behind him hanging the cable, when the cable kicked causing the person's arm, who was hanging the cable, to pull backwards.
Employee was splicing a shuttle car cable when he struck the back of his hand with a belt knife.

An unplanned roof fall occurred at 7:00 P.M. ,CST, at cross cut #47 on the first main west, number 5 entry. The roof fall was approximately 8 foot high and 15 foot long. The openings of the intersection where the fall occurred were approximately 18 feet wide.
Employee was turning around in shuttle car to head back to the miner after dumping his shuttle car at the feeder. When he was turning around he twisted his left knee.
The employee was picking up a wooden pallet and a nail cut his finger.
While riding in the personnel van from the prep plant to the bath house, the driver of the van applied to brakes and employee fell forward and hit his face on the dashboard and windshield. This caused a laceration under his left eye. First aid was applied and he started to travel home. On the way home he stopped at the ER and was told he had a hair line fracture in his cheek bone.
Operator was adjusting pin plate before tightening against mine roof; and pinched left middle finger between mine roof and pin plate. This caused a laceration to the right middle finger. The laceration required sutures.
As employee was installing a roof bolt he felt something pull in his right shoulder. He continued to work, but soreness continued. He had a MRI and the doctor determined that his shoulder has ligament damage which needs repaired. Employee started missing work on 4/24/2012.
The operator placed his right hand on the outer edge of his shuttle car canopy as he was tramming from #3 entry toward the feeder. His hand was caught between the rib and canopy causing a fracture and lacerations to his fingers which were closed with sutures.
On 03/04/2012, an unplanned roof fall occurred on the 1A travel road, 1st SE Submain off the 2nd SW Submain at cross cut #8 in #5 entry. The fall measured approximately 18'W X 18'L X 9'H. The area was bolted with 8'and 10' point archer bolts, with additional truss bolts. The fall will not be cleaned up.
On 3/1/2012, an unplanned roof fall occurred on the 2nd Southwest Submain at cross cut #63 in #6 entry. The fall measured approximately 18'W X 18'L X 10'H. The area was bolted with 8' and 10' point anchor roof bolts. The fall will not be cleaned up.
EE was knocking down a brattice using a sledge hammer. As he hit the brattice with the hammer, he mashed his little finger on his left hand, causing a hair line fracture and laceration.
Employee was pulling down a piece of loose rock with a pry bar when the rock slid down the bar and struck his thumb. This caused his thumb to be smashed between the bar and the rock.
EE was installing his rib line roof bolt and had his 4' starter steel in the roof. When he repositioned his boom to add a 4' extension steel, he had his left hand on the 4' steel still in the roof. The halo on the canopy pinched his index finger on his left hand against the starter steel as the boom was repositioned. This caused a laceration which had to be closed with sutures.
Employee was pulling rock of the Continuous Miner with a pry bar. As he was pulling a rock, the rock slipped and caught his left middle and ring finger between the rock and the top of the machine, causing a fracture to his middle finger.
Employee loading & securing a package on an F-250 truck. As he attempted to get down off the truck bed, the hinged side board fell open. This caused him to fall onto his wrist causing a Sprain.
EE was installing a new bit insert in the MMU 003-0 continuous miner. He had the insert almost completely in when he struck his right index finger with a 16lb sledge hammer. This caused lacerations and a fracture to his finger.
Employee was working on the suction on his roof bolter. He asked his partner to assist him. He was reaching under the boom area and had his hand laying on the long arm of the boom and asked his partner to raise the boom. As he did so, his hand was caught in the scissor pinch point causing a crushing injury.
Employee was assisting pulling belt with a wire rope when the chain holding a sheave wheel broke and some of the flying material struck him on the left arm causing a fracture and muscle damage.
EE was holding a drill steel with his right hand. While swinging the boom inward, to allow the steel to be lowered, he mashed his hand between the steel and canopy. This caused a laceration which had to be closed with sutures.
EE was removing a drill steel from a drill hole, when the top section dropped out of the hole striking him on the left hand. This caused a laceration which had to be closed with sutures.
The Section Foreman was helping a roof bolter remove a bent roof bolt from the drill hole. When the bolt was pulled out of the hole the monster plate on the roof bolt struck the Foreman on the right index finger causing a laceration which had to be closed with sutures.
EE was pulling out a drill steel that was hung in drill hole. When the steel broke free it fell out and hit the drill pot, causing the bit to strike the operator in the chin causing a laceration which had to be closed with sutures.
EE was installing rib bolts with a double boom truss bolter when a piece of rock fell from between the rib and first row of roof bolts. The rock struck him on the lower right leg causing a laceration which had to be closed with sutures.
EE was cutting a header hole with a continuous miner. A small piece of rock fell from the mine roof striking him on his left hand. This caused a laceration that had to be closed with sutures.
EE was installing the clamp that holds the air filter into place on the Diesel Generator. As he was doing so, his hand slipped off the clamp and his left forearm made contact with the other clamp band causing a laceration. The laceration had to be closed with sutures.
On or about 8/05/2011, an unplanned roof fall occurred on the 1st Main North, #4 entry at cross cut #11. The fall measured approximately 18'W x 18'L X 10'H. The area was bolted with 6' bolts and additional support. The fall will be cleaned, and resupported.
EE was working on the suction on his side of a roof bolter. While doing so, his drill steel fell out of the hole he was drilling. His right index finger was mashed between the drill steel and the drill pot. This caused a laceration and fracture to his finger.
As #5716 Alpha Mantrip was traveling on the 3rd Main West travelway, a piece of rock fell on the hood and proceeded into the passenger area striking ee in the right eyebrow area. This caused a laceration which had to be closed with sutures.
Employee was changing bits on a continuous miner. As he was doing so, a piece of the hammer he was using broke off and struck him, and stuck in his little finger on his left hand. It had to be removed at the local ER and sutured.
On or about 6/21/2011, an unplanned continuation of a roof fall occurred. The fall was located at cross cut #197, #7 entry, in the Main West entries. The area was bolted with 6' point anchor roof bolts.
On or about 06/09/2011 an unplanned roof fall occurred on the 3rd N.W. Sub Main in #3 entry, cross cut #5. The area was bolted with 8' & 10' point anchor roof bolts. The fall will not be cleaned up. The area was fractured laminated stack rock. The fall was approximately 17 3/4'wide X 18' long X 10' high.
EE was removing supplies off his roof bolter, for MSHA inspection, when a piece of rock fell from in between roof bolts striking ee on the head, back, and left arm. This sharp rock caused a laceration to the back of his left arm, which had to be closed by sutures.
EE was getting out of the cab of #80 scoop and made contact with a sharp edge on the frame. This caused a laceration on his right forearm which had to be closed with sutures.
EE was hanging a shuttle car anchor when something popped in his left shoulder.
EE was installing roof bolts on #3 Unit in #10 entry when he strained his back.
EE was driving a loaded shuttle car back to the feeder as he turned into the belt entry, he struck the coal pillar. As he struck the rib he had his arm resting on the side of the cab. His upper and lower arm was pinched between the cab and rib. This caused a great deal of soft tissue injuries.
Employee was driving a 14 person Man Trip on the Main West travel road. As he was driving the trip, it hit an arch on the passenger side. This caused employee to hit the steering wheel, causing broken teeth.
EE was driving a Getman Tractor when the right front tire ran over a concrete block in the roadway. This action dropped him about 8", causing him to slam his teeth together, which broke one of his rear teeth.
Employee was eating a sandwich, and as he was doing so, he broke a tooth.
Employee was helping change bits on the continuous miner. When he struck the bit wrench with a hammer, a small sliver of metal broke off the hammer and struck him on the left wrist causing a laceration. The sliver had to be removed and the laceration closed with sutures.
Employee was in the process of changing out a conveyor speed reducer on a shuttle car. As they were mounting the new reducer his right ring finger was caught between the reducer and main frame. This caused a fracture to his finger.
EE was working on the suction on the operator side of the bolter. Another EE came around from the other side to help. Other EE regained suction behind the boom. EE put his hand up inside of the pre-dump box to clean it out. As he did this, the other EE started the bolter and put down the stab jack which closed the pre-dump door, mashing EE's hand.
EE was roof bolting in #8 entry on #3 Unit when a small piece of rock fell from between bolts. The rock landed on his left hand that was laying on the bolter and caused a fracture to his thumb.
EE was performing a permissibility on a continuous miner. As he was inspecting the master control panel, he caught his finger in the outer lid, causing a laceration to his left ring finger that had to be closed with sutures.
EE was walking to the continuous mining machine to help clean the scrubber screen when the corner of a pillar block rolled off and struck him on the right leg. The rib roll caused a fracture to his right foot.
EE was walking to his shuttle car, as he was doing so he struck his right ear on a roof bolt plate causing a laceration which had to be closed with sutures.
EE was installing a roof bolt, when a piece of rock fell and struck him on the right forearm, causing a laceration which had to be closed with sutures.
An unplanned roof fall occurred on the 2nd S. W. Sub-Main at cross cut #96, entry #5 on the conveyor belt. The area was bolted with 8' & 10' point anchor bolts. The roof fall will be cleaned up.
Employee was pulling a rock off the continuous miner with a scoop and nylon strap. The strap failed, or slipped and recoiled back striking him under the right eye, causing a laceration which had to be closed with sutures.
EE picked up the trailer hitch for a Getman tractor and felt a sharp pain in his back. EE continued to work until 10/27/2010, at which time he started missing work.
On or about 09/30/2010 an unplanned roof fall occured on the 3rd S.W. at cross cut #100 in #2 entry. The fall was approximately 18' Wide X 18' Long X 10' high. The area was bolted with 8' & 10' roof bolts. The fall restricted ventilation, and was cleaned up.
EE had placed a 3' drill steel in the tool tray, but left it sticking out of the tray too far. When he lowered the boom, it caught the drill steel making it spring up and hit him in the forehead causing a laceration. The laceration had to be closed with sutures.
EE was loading monster roof plates onto a roof bolter and cut the base of his left thumb. The laceration had to be closed with sutures.
EE was helping move the unit power cables. As he was walking down the belt entry his right ear came in contact with a belt hanger key hole plate causing a laceration which had to be closed with sutures.
EE was lifting a coupling on a rock dust push out car to hook it up to a Getman Tractor. His right ring finger got mashed between the two couplers, mashing the end of his finger off.
The injured was operating a double boom roof bolter. When he placed a 3' wrench into the drill pot, the drill steel ( which was still in the roof) fell out of the hole mashing his right thumb between the drill steel and wench. This caused a laceration which had to be closed with sutures.
An unplanned roof fall occured at cross cut #7, entry #5 of the 2nd Northwest Sub Main, off the 1st Southwest Sub Main. The fall was approximately 35' long X 17" X 10' thick. The area was roof bolted with 8' and 10' roof bolts.
EE instructed the scoop operator to pull back the stinger on the scoop. They were pulling a roll of belt into the bucket, as the stinger retracted, EE's hand was caught between the frame and stinger causing a fracture and soft tissue injury to his left hand.
EE had moved the continuous miner to the #5 entry on #2 unit. As he was setting up the machine and dropping off the trailing cable, it appears that he came in contact with the miner tail. It is unclear to him how the accident happened and there were no witnesses.
Employee was removing lug nuts from a tire on a roof bolter. He was pulling a ratchet toward himself when the socket slipped on the lug. This caused the ratchet handle to strike him in the upper lip causing a laceration which had to be closed with sutures.
On or about 8/16/2010 an unplanned roof fall occurred on the Southwest Sub Mains, cross cut # 89, entries 3 & 4. The area was bolted with 8', 10' point anchor roof bolts with additional prop setter props. The fall was approximately 30' long x 110' wide x 15' thick. The fall in #3 will be cleaned.
At an unknown date an unplanned roof fall occured at cross cut 181, #2 entry, in the 4th Main West. The fall was approximately 18' wide x 20' long x 18' high. The area was bolted with 6' point anchor roof bolts. The fall has been cleaned.
Cardiac Event
Employee had cut a hole in top of a Hyd. oil can so he could pour additional oil in the can as it fills machines without stopping. The can started to fall over, he tried to catch the can and cut his index and middle finger. The lacerations had to be closed with sutures.
On 7/17/2010 an unplanned roof fall occurred on MMU 002 in #6 entry, cross cut #4 in the 1B Submain off the 4th S.E. Sub-Main. The fall was approximately 25' long X 17' wide X 8' thick. It was a non injury accident, and the fall will not be cleaned up. The area was bolted with 8' and 10' roof bolts.
While attempting to remove a bearing race with a hammer and pry bar, EE hit the bar with a hammer, causing a piece of the bar to brake off and strike him in the left forearm. The metal had to be removed from his arm by a Doctor.
EE was using a slinger type rock duster to dust the section. A paint can became stuck in the auger. As ee was trying to remove the can, a piece of metal became inbedded in his left index finger. It had to be removed by a Doctor and closed with sutures.
Employee was removing a drive shaft from the rotary breaker. As he cut the last bolt holding the shaft, the shaft shifted and mashed his left hand causing crushing injuries and fractures to his left hand.
Employee had completed drilling an 8' hole, and had made up a roof bolt. When feeding drill boom, it came in contact with the bolt causing the bolt to travel in the operators direction, striking him on the arm causing a laceration.
EE was using a utility knife to cut a piece of conveyor belt when the knife slipped and cut his right leg. The laceration required sutures to close.
EE laid his right arm on the discharge boom on the continuous miner, as he did so his right forearm was cut by a steel burr on the machine. The laceration had to be closed with sutures.
An unplanned roof fall has occured from cross cut #26 to #27 in the # 7 entry, in the 3rd Southeast Submain off the 1st Southwest Submain. The area was fractured stack rock, bolted with 8' & 10' point anchor roof bolts. The fall will not be cleaned up, and MMU 002-0 has been moved from the area.
On 3/5/2010, an unplanned roof fall occured at cross cut #24 in #5 entry of the 3rd S.E. Sub Main, off the 1st S.W. Sub main. The fall measured 17'Wide X 18' Long, X 11' high. The area was bolted with 8' & 10' point anchor roof bolts. The fall area was fractured stack rock.
Injured was reaching for a pinner drill steel, and cut his left forearm on a monster roof plate. The laceration was closed with sutures.
EE was roof bolting in #8 entry, when a rock fell from the roof, striking the canopy. It slide off the canopy, striking him on the left leg, straining his left knee.
EE was removing a bolt and nut from an overcast with a socket and ratchet. The ratchet came off the nut, causing the ratchet to hit EE on the nose and right cheek. After further investigation, it was discovered he had a fractured nose.
EE was drilling a hole for a roof bolt when the suction on the bolter became restricted. He put his hand over the drill pot to check suction. As he did this, the drill steel still in the roof fell and struck his left hand, causing a fracture to his index finger.
EE was helping build an overcast at cross cut #99 on the 3rd North. As he was filling in between the rails with rock dust, the rail spread, causing him to fall through the overcast. Several concrete blocks struck him, causing a laceration to his left ear, which had to be closed with sutures.
EE was helping move equipment doors when another EE dropped a solid concrete block. The block fell on EE's right hand causing a laceration which had to be closed with sutures.
A roof fall was discovered on #2 Unit (entry #7) cross cut #22, in the 2nd S.E. Panel. The fall was approximately 90' long x 18' wide x 15' thick. The area was bolted with 8', 10', and 15' roof bolts. The fall will not be cleaned-up.
EE was walking a conveyor belt checking the remote line. As he was walking, he stepped on a piece of rock and lost his balance and fell onto his left hand, causing a fracture to his little finger.
EE was flagging off a damaged roof bolt West of a block curtain on the blind side of an oncoming shuttle car that was traveling to #8 entry. The car driver rang his bell before traveling through the curtain. EE tried to get to a slider in #6 entry, but the car knocked him to the ground. EE tried to roll out of the way but both legs were run over below the knee by the car.
An unplanned roof fall occured on the 1st Southwest sub main off the 2nd Southeast sub main in entry #6 at cross cut #8. The fall was on the supply road and will not be cleaned up. The area was roof bolted with 6' and 10' roof bolts. The fall appeared to be fractured stack rock. The fall was approximately 17' W x 17' L x 15' H.
EE was clearing loose coal off the rabbit box (gob switch) at the 2B header. A piece of rock 6" x1' x 2"-3" fell from the roof, hitting his right middle finger, causing a laceration and fracture.
Employee was standing at the side control station of the #6009 Roof Bolter when he was struck on the top of the head by a piece of falling roof rock. The rock knocked him to the ground and part of it fell on his midsection. The rock measured 4' long x 2' wide x 1" thick.
EE was installing an 8 foot roof bolt. As he went to bump his chuck to get the bolt into the chuck, it slipped out causing the monster roof plate to slide down the bolt. The monster plate hit his left forearm, causing a laceration which had to be closed with sutures.
Injury was attaching a hitch to a getman tractor for a rock duster. He was holding the hitch up while putting the pin in the hole. He dropped the pin in and let go of the coupling mashing his finger under the pin when the coupling fell in place.
EE was using a pry bar to unhook a rock duster from a Getman tractor. The pry bar slipped and the pointed end of the bar struck and cut his right hand. The laceration had to be closed with sutures.
On 09/14/2009 an unplanned roof fall occurred at cross cut # 23, in #4 entry on the 3rd S. W. Panel off the 1st N. W. Submain. The area was roof bolted with 6' and 10' point anchor roof bolts. The fall appeared to be fractured stack rock. The fall will not be cleaned up.
EE was drilling a roof bolt hole. As he was putting the two drill steels together, his right hand got caught in between the two drill steels, causing a fracture to his right hand.
Employee was building a roof support crib. As he was putting up the last crib block, he dropped the crib block onto his left hand. This caused a laceration that had to be closed with sutures.
Employee was cleaning off his shuttle car when a piece of rock fell from between roof bolts and struck him on his left hand causing a contusion.
EE was looking for bad belt splices with the belt on slow inspection mode. A loose strip of belt caught the glove on his left hand and pulled his hand and arm between the bottom belt and roller. This resulted in a fracture to his left arm and lost work days.
Employee was building a solid concrete block brattice when he mashed his left thumb between two concrete blocks causing lacerations that had to be closed with sutures.
Injury had bent a roof bolt to install the bolt, as he tried to straighten the bolt the roof plate spun around stricking him in the right forearm causing a laceration that had to be closed with sutures.
Employee was tightening a roof bolt with a wrench, the wrench stuck to the bolt. As he lowered the boom, the wrench came loose. The wrench hit the drill pot and bounced up striking him in the lip causing a laceration that had to be closed with sutures.
A roof fall occurred in an area that had been cribbed and dangered off in #7 entry, cross cut #25 on #2 unit (MMU 006-0). The area was roof bolted with 6' and 10' bolts. The fall appeared to be fractured stack rock. The fall will not be cleaned up.
Injury was hanging a miner trailing cable. As he was doing this, the miner pulled the cable tight, pulling the cable down striking him on the neck and back driving him to the ground.
Injury was placing resin in a hole drilled for a roof bolt when a rock fell from the roof. This caused a fracture to his right arm. The rock slide down toward the operator. Our assumption is the rock hit his right arm and drove it into the drill boom resulting in the injury.
Employee was performing maintenance on a doubleboom Fletcher roof bolter, when he leaned against a monster roof plate causing a laceraton which had to be closed with sutures.
The miner was turning up #6 entry, he smashed a ladder, he stopped, moved the miner off the ladder, hit the e-stop on the machine. He traveled between the tail of the miner and the rib when the machine relaxed, shifted, striking him on the hip/pelvis. Witnesses agree he was not in the red zone when he deenergized the machine before he moved toward the front of the machine.
EE had loaded some monster roof plates on the deck of a Mini-Trac to transport them to a roof bolter. As he was traveling down an incline, the monster plates slid into the deck, causing injury to his right leg and ankle.
EE stated that he stepped in a rut and twisted his right knee. EE did not report this accident to his Foreman or anyone else on this Friday. On Monday he called in and said he had car trouble, then later, when questioned, he said he hurt his knee. We feel this was a home accident and not work related.
EE was trying to pull down a piece of ventilation curtain. When he jumped up in a pull down motion. When he landed on his feet, his left ankle twisted and popped. He was transported to the hospital where an x-ray showed a fracture.
An unplanned roof fall occurred on 1st South East submain at cross cut # 31 in # 4 entry. The area was roof bolted with 6' and 10' bolts. The fall appeared to be fractured stack rock. The fall will not be cleaned up.
As employee was installing a roof bolt the plate spun around. He tried to straighten the pin plate and pinned his right index finger between the mine roof and the pin plate, causing a crushing injury.
EE was stripping insulation off the conveyor belt remote line, when his hand slipped and cut his left thumb, which required sutures to close.
EE was standing on a 4' step ladder while plastering a brattice. As he was doing so, he slipped and fell off the ladder, striking his head on the brattice, causing a laceration that had to be closed with sutures.
An unplanned roof fall occurred on the 4th Main West Mains at cross cut 221 in #6 entry. The area was bolted with 6' and 10' point anchor roof bolts. The fall is in old works and will not be cleaned up.
EE was pulling a drill steel out of the roof when a piece of rock fell striking his right hand, causing his hand to hit the drill pot. This caused a fracture to his right hand.
Injury was standing in #3 entry waiting for a scoop to remove some rock from behind the roof bolter. As He was watching the scoop a small piece of rock came from the direction of the scoop striking him above the left eye. This caused a laceration which had to be closed by sutures.
Employee was starting to drill a hole for a roof bolt in #7 right entry on Unit #2, when a small piece of rock fell from the roof striking him on his upper lip. The rock caused a laceration which had to be closed with sutures.
Employee was roof bolting in the face of #7 entry on #2 Unit. He went to the back of the roof bolter to get supplies. As he got to the back of the machine, a piece of roof rock fell from between roof bolts striking him on the right leg causing a fracture.
