Employee inside screen deck, another employee slid screen down to him. While pulling screen towards him, the screen cloth caught employee's right index finger. The sharp edge of screen cloth cut right index finger, requiring sutures.

Employee was working on the jaw crusher. Employee lifted a 30-pound take-up bearing and stepped back and had a sudden pain in his middle back causing him to double over. Employee received a strain/possible muscle spasm to his middle back. Employee has lost time and restricted time. He has not returned to full duty at this time.

Employee was walking to the water pumps to prime. Employee stepped down where two catwalks connect and rolled his left ankle. Employee received a sprain to his left ankle. Employee was to return to restricted duty on 3-17-09.

Employee was walking from one location to another at the plant and stepped on a raised area of ground and turned his right ankle. Employee received a sprain to his right ankle and is restricted to light duty work.

Employee was working on the processing plant. He was guiding a new metal chute into position when his finger got caught between the chute and an angle iron. Employee received a laceration to his left index finger. He received stitches. There is no lost or restricted time.
Employee was working on the loader. Employee slipped off the Hydro Tank and fell partly to the ground. His supervisor was close by and caught him before he hit the ground. Employee hit his right leg on the tank causing a laceration that required several stitches. There was no lost or restricted time.
Employee was putting a cable on a truck to pull it out of the mud. Employee stepped in a hole on frozen ground and turned his ankle. Employee left work and went to the hospital and then called his supervisor that evening and told him that he injured himself on the job. He received a bad sprain to his left ankle. He was given an air cast. Pending per workers comp.
