Employee was using a punch and hammer to punch holes in side of the wipers on the conveyor belt. Employee missed the punch and hit his left thumb with the hammer. Employee went home but the next morning he could not move his thumb and was sent to the company clinic. He received a contusion to his left thumb.

Employee was leaving the shop and he reached to shut the shop door when the wind caught the door and the door closed on his left middle finger. His finger was smashed and cut in three places. Employee received a laceration to his finger. Steri strips were applied and he had to wear a finger guard but was able to do his regular work.
Employee stopped the 95NW Crane to check the cables on the spools and spray gear lube on the ballgear and deck gear. Employee crossed the center of the crane and started to step off the house onto the tracks when he slipped and twisted his left ankle. Employee received a sprain to his left ankle. He is restricted but is working.

EMPLOYEE WAS HELPING A CO-WORKER REMOVE A REAR GUARD ON THE SAND STACKER AND FELT HIS RIGHT SHOULDER POP OUT OF SOCKET. EMPLOYEE RECEIVED A DISLOCATED RIGHT SHOULDER WITH SHOULDER STRAIN.
EE WAS SPLITTING FIREWOOD FOR THE PLANT. EE WAS LIFTING AND MOVING LARGE PIECES OF WOOD AND STRAINED HIS LOWER BACK. EE RECEIVED A LUMBAR SPRAIN.
EE WAS WALKING UP LADDER TO ELECTRIC TRAILER IN THE YARD. ON THE LAST STEP EE FELT HIS LEFT KNEE POP & GIVE WAY. EE COULD NOT PUT WEIGHT ON HIS KNEE. HE HAD TO HAVE HELP TO THE CAR.
EE WAS LOADING AN ENGINE STAND INTO BACK OF SERVICE TRUCK WITH A 988 CAT LOADER. THE CHAIN WAS WRAPPED AROUND THE STAND, SLIPPED AND CAME UNHOOKED CAUSING THE STAND TO KICK BACK TOWARDS EE AND HIT BOTH OF HIS LEGS KNOCKING HIM BACK ABOUT 6 FEET. LEFT ANKLE WAS BROKEN AND BONE BELOW LEFT KNEE. RIGHT LEG - LACERATION.
EE WAS RE-LINING SHOOT TO LOG WASHER WHEN HE TOOK A HAMMER & TRIED TO DRIVE UP A PIECE OF STEEL ON THE BOTTOM OF THE SHOOT & CAUGHT HIS LEFT THIRD FINGER BETWEEN EDGE OF STEEL & HAMMER. HIS FI NGER WAS SMASHED & FRACTURED - THE TIP OF THE BONE.
EE WAS PUTTING PADDLES ON THE LUG WASHER. EE TRIED ONE MORE TIME AND WHEN HE HIT THE PADDLE A PIECE OF METAL GOT INTO EE S RIGHT EYE. HE WORKED THE REST OF THE DAY WENT HOME, EYE GOT WORSE, HA D TO GO TO ER.

EMPLOYEE WAS WATCHING AUTOMATIC WELDER BUILD UP ROLLS ON CRUSHER. WHILE EE WAS LOOKING AT THE WORK, HE RECEIVED WELDING FLASH BURNS TO BOTH EYES. HE WAS GIVEN SALVE FOR EYES AND PAIN MEDICATIO N PRESCRIPTIONS.
