Carlisle Mine Coal

Injured was changing a T-Bar on a roof bolter. While taking the bolts off EE strained shoulder. EE was seen by the Dr. on 12/16/2019 and placed on restricted duty, only, for a muscle strain.

A roof fall was found in our 6W Travelway, XC 4, Entry 4. The fall did not cause injury to miners, impede travel, or affect ventilation. An action plan was submitted and approved by MSHA to rehabilitate the affected area.

Injured was hanging miner cable and strained shoulder.

Injured was using a rock bar to pry on a glue tray on a roof bolter. As EE was prying EE strained EE's shoulder. EE was seen by the Dr. on 11/4/2019 and placed on restricted duty.

Injured was riding in a 14 man, mantrip. The mantrip door came open and as it shut EE's elbow was hit causing a small fracture. EE was seen at the Dr. and placed on restricted duty.

Injured was working on a mini-trac and hit elbow. EE was seen by the Dr. on 12/2/2019 and placed on restricted duty, a bone spur was found, by xray, in right elbow.

Employee was pulling bolter cable on 9/12/2019 and twisted EE's knee. EE had surgery on 3/26/2020 to repair the knee.

Injured bent over to pick up a timber and strained back. Injured was seen by a Dr. on the 29th and placed on restricted duty

Injured was walking and stepped in a hole and sprained their knee.

Inured was roof bolting. A piece of rock dislodged from the mine roof and slid down EE. EE was seen at the Dr. and placed on restricted duty due to contusions on EE's shoulder and leg.

Injured was opening a door to the mantrip garage as another employee was walking through the door. Injured hit thumb as the door sprung opened. EE was seen by a Dr. on 6/24/2019 and placed on restricted duty for a torn ligament in EE's thumb.

Injured hit an elbow on a gear shift while sitting down in a man-trip. Employee was examined by a Dr. on 4/29/2019 and placed on restricted duty due to a contusion to the elbow.

The employee was walking and tripped over a rock and fell on right knee. Employee was seen by a Dr. on 10/29/2019 and placed on restricted duty on that date.

Injured was walking, lost EE's balance and twisted knee. After seeing a Doctor EE was placed on restricted duty.

EE slipped in the parking lot on 1/24/2019 and hit an elbow. EE went to the Dr. on 3/5/2019 to have it seen and was placed on restricted duty at that time.

Technician was removing tire from a machine when a chain broke and caused laceration to left shin requiring four stitches

Injured was working at our Sister Mine on 1/10/2019. EE tripped while working on a battery coal hauler and bumped knee. EE seen a Dr. on 3/18/2019 and was placed on restricted duty due to a knee contusion.

Injured was cutting a flapper off of a piece of belt. The knife slipped and ee cut thumb. EE received two sutures to close the wound.

Injured was roof bolting. As ee swung the boom to install the rib pin, the boom hit leg and twisted ankle. A fracture was found in ankle upon review of the x-ray.

Injured was doing maintenance work on the outside of the preparation plant. As employee was filling up a tank, their right eye was hit by unknown material. The physician prescribed medicated eye drops for the damage to the eye.

Injured was roof bolting, as they installed the pusher steel into the starter steel it slapped back and hit their hand. Employee was taken to the hospital and received 4 sutures.

Injured was loading trash in a trailer. Foot was under the rub rail on the trailer. The trailer was pulled forward causing a small fracture to EE foot.

A roof fall, in an examination line of travel, was found during a pre-shift examination in our 6W Mains. The fall was located at X-Cut #5 in Entry #8. The fall did not impede passage of miners, affect ventilation or cause personal injury to miners.

On June 1, 2018 Injured was repairing a battery powered scoop. As EE was replacing a steel cover it slipped and hit EE's right pinky finger. EE went to the Doctor on June 28 and was placed on restricted duty due to a fracture found in that finger.

Injured bent over to pull a piece of steel out of a scoop bucket. As EE lifted the steel EE felt pain in right knee/leg. EE followed up with the Doctor on 9/20/2017 and it was determined EE strained lateral hamstring. EE was placed on restricted duty and will have 4-6 weeks of physical therapy.

Injured was walking through a x-cut towards the travelway as a tractor was tramming outby on the travelway. A tractor tire hit a steel pipe, laying on the mine floor, flipping it up striking injured on the head.

A fuse blew on the North Material/Man Hoist. No persons were on the cage at the time and no miners were trapped underground. The fuses were changed and the hoist was operational at 9:30am.

a roof fall was found during a pre-shift mine examination. The fall was located in our M4 Mains, x-cut #57, entry #4. The fall did not cause personal injury, affect ventilation or impede passage of miners

Injured was opening a tailgate on a Chevy Pickup. As injured pulled on the tailgate it did not open causing injureds knee to twist back and strain. Injured had minor surgery on 4/22/2016 to repair the strained ligaments.

A roof fall was found in our M4-Mains at crosscut 71 in the haulage road. The fall was not located in an escapeway, it did not impede passage of miners, affect ventilation or cause injury to miners.

A roof fall was found by an examiner while performing a weekend fire check. The fall was located on our M3 Travelway between x-cut 20 and 21.

Injured was laying out belt structure prior to a belt advancement. As he was walking he tripped and sprained his ankle.

Injured was driving a pin in a rub rail. The hammer broke and cut his hand

Injured was walking, stepped in a hole in the mine floor and sprained his knee.

During the hoist examination of our North Portal Man/Supply hoist it was found that the service breaks were locked up. A mine electrician was called in to fix the switch that controls the breaking system. No miners were trapped on the cage and no miners were underground while the hoist was down.

Injured and another employee were hooking a chain between a Dapco Tractor and a mantrip in a travelway. Another tractor turned a blind corner and pushed the mantrip into the Dapco Tractor pinching the miners in between the two pieces of diesel equipment. He sustained a severe crushing type injury to his right leg.

Injured and another employee were hooking up a chain between a Dapco Tractor and a mantrip in a travelway. Another tractor turned a blind corner and pushed the mantrip into the Dapco Tractor pinching the miners in between the two pieces of diesel equipment. He sustained severe crushing injuries to both lower legs and his left wrist.

The employee was sitting in a 6 man/mantrip attempting to pull a Dapco Tractor. While awaiting for two other miners to hook up a chain the mantrip was hit by another Tractor. He was seen by a Dr. on 6/10/2015 and placed on restricted duty due to a lower back strain.

While mining #7 entry, crosscut #40 of the Sub 4-F Sub Main (004-0 MMU) a methane ignition occurred. The face was approximately 25-30 foot deep. This ignition did not cause personal injury to miners or damage any company mining equipment.

Injured stepped out of a mini-trac and sprained his ankle. He seen a Dr and was placed on light duty

A roof fall was found on 005-0 working section, crosscut 26, entry 7 of the S9 Main. The fall did not cause injury to miners, impede passage of miners or affected ventilation.

Injured was operating a twin head roof bolter. His drill extension was stuck in the chuck of the drill boom. As he pulled it out it hit him in the mouth causing a laceration and chipping a tooth.

Injured was walking outby the working section, slipped on a rock and twisted his knee. he was seen by the Dr. on 4/22/2015 and placed on restricted duty

While roof bolting injured sustained a laceration on their lip from a piece of mine roof that fell through a roof matt.

A roof fall was found in our Primary Escapeway in the 5WD Sub Mains. MSHA was notified of the fall at 9:30am. The fall did not cause personal injury, impede passage of miners or affect ventilation.

Injured was setting a timber. As he moved the timber his thumb was caught between the timber and mine roof.

Injured was hanging a curtain. As he was driving the nails with a hammer to hang the curtain he hit his finger causing a fracture.

Injured was standing next to a twin head roof bolter. A piece of shale dislodged from the mine roof and struck injured on head causing a laceration.

Injured walked into a machine door and bumped his knee. After a few weeks he was still experiencing pain in his knee and seen a Dr. on 3-16-2015. The Dr. placed him on restricted duty

Injured was operating a twin head roof bolter. As he dropped the boom (pod) his foot slipped and went under the boom and broke his foot.

Injured was installing roof bolts with a twin head roof bolter. As he was inserting a pusher into his starter steel the pusher slid out of the drilled hole and struck his thumb.

Injured was loading a machine door in a scoop. As he was lifting the door it slid down the bucket of the scoop and struck his leg.

Injured walked through a curtain and tripped over roof mats spraining his right knee

A continuous miner operator was loading coal in entry #3 on #1 MMU. As he finished his second lift a methane bleeder was ignited. The flame was extinguished without further incident. No miner was injured due to this methane bleeder.

A roof fall was found in our 5W-D Mains at crosscut 22 in the secondary escapeway. The fall did not cause injury to miners, impede passage of miners or affect ventilation.

A roof fall was found inby the section loading point on our #3 MMU, entry 1to2. The fall did not cause injury to miners, impede passage of miners or affect ventilation.

A roof fall was found during a weekly examination in our S-8 Mains, x-cut 15, entry 6. The fall did not cause injury to miners, impede passage of miners or affect ventilation.

The employee stepped out of a SkyTrak and sprained his ankle. Injured had submitted a two weeks notice prior to the accident to pursue a job opportunity with Marathon. His last day of employment was on December 30th.

The employee put his hand on the outside of the mantrip to push the signal button in the slope. His finger was caught between the mantrip and button causing a laceration to finger.

While installing roof bolts with a twin head roof bolter injured bent a bolt, it swung back and hit him on the wrist.

Injured was stacking roof matting against a coal rib. The roof matting fell over and hit his knee causing a strain.

Injured was walking around a twin head roof bolter. the ATRS struck injured in the stomach causing a contusion. He was seen by the Dr. and is currently placed on restricted duty.

Injured was throwing continuous miner cable against the coal rib. As he picked up the cable he strained his left shoulder and was put on restricted duty.

on 10/26/2014 Injured was trying to reset a scoop breaker, his foot slipped and strained his knee. He went to the Dr on 11/6/2014 and was placed on restricted duty only.

Injured was helping to move a twin head bolter. As he stepped over the bolter cable he tripped, landed on his foot wrong and fractured his leg.

Injured was riding in a mantrip. As the mantrip passed a Getman Tractor in the haul road the injured caught his foot between the mantrip and trailer of the Getman causing bruising to his foot/ankle

Injured was tramming a double boom roof bolter from the cab. A piece of coal rib was laying on the ground and as he trammed the coal went in the cab and caught his foot between the cab and the coal rib. X-Rays showed a hairline fracture in his foot

The employee was advancing belt structure. As he was installing steel water line he strained his lower back. He has been put on restricted duty only

Injured was cleaning a scalping screen and twisted his lower back. Restricted Duty Only

A roof fall was found adjacent to our secondary escape way in a travel way leading to a bulk rock dust hole. The fall was found at 7:30pm and called into MSHA Hot line at 7:45pm. The fall did not cause personal injury to miners, impede passage of miners or affect ventilation.

A roof fall was found by an outby examiner in our M4 Mains. The fall did not cause injury to miners, affect ventilation or impede passage of miners. A plan has been submitted to MSHA to clean up the fall.

The employee was changing bits on a continuous miner. A piece of steel hit his hand and on 10/16/2014 injured went to the Dr. and a piece of steel was found imbedded in his hand. Injured had surgery on 10/20/2014 to remove the sliver of steel imbedded in his hand and was placed on restricted duty.

Injured was operating a twin head roof bolter. A piece of mine roof dislodged from the mine roof and struck injured on the arm. He sustained a small laceration and received a couple of stitches.

Injured was assisting the roof bolter operators. As he cut the bands on a bundle of roof bolts the bolter advanced forward and struck his foot causing an acute fracture in his 2nd metatarsal.

Injured failed to maintain three points of contact while climbing down from a dozer. He slipped and fractured his left leg.

Injured was operating a continuous miner. As he was mining a piece of mine roof dislodged from the mine roof, hit the continuous miner and then struck him on the ankle.

Injured was taking wire off of a bundle of roof mats. As he untwisted the wire that tied the mats together he sprained his thumb.

The employee pulled down a curtain panel and aggravated an pre-existing personal injury. He seen the Dr. a few days later and was put on restricted duty. It has since been deemed a work related injury.

Injured was walking in an outby location. As he was walking he ran into a belt hanger and cut his ear.

The employee grabbed a crib block from a bundle of cribbing. He cut his hand on metal banding requiring stitches.

The employee was operating a twin head roof bolter. He backed his machine up to fix a hose. As he was working on the hose a piece of mine roof fell and struck him causing bruising to his left wrist.

Injured was roof bolting and a piece of mine shale hit his finger causing an abrasion to his finger.

Injured was operating a twin head bolter. As he swung the boom over, a piece of mine roof hit the boom lever causing it to continue to swing. The boom hit his leg resulting in a fracture.

Injured was standing at the outby side of a twin head roof bolter. A piece of mine roof dislodged from the mine roof and struck the employee on the head. He was transported to the hospital and put on restricted duty due to a neck sprain.

A roof fall was found in our P4-2 Panel, x-cut #19, entry #3. The fall occurred in the straights as we were moving the MMU to an intake room set up. The fall was found at 1:55am and phoned into MSHA at 2:08am. The fall did not affect ventilation, impeed passage of miners or cause injury to miners.

Injured was tramming a twin head bolter over cable crossovers. The bolter slid on the crossovers toward the coal rib. His arm was fractured when it was caught between the coal rib and tram lever.

A gate on the North Man/Material hoist was not completely shut and came open during operation shutting the hoist down in the shaft. There was not any miners on the hoist during this failure. All affected miners were withdrawn out of the mine via our south slope.

The employee was operating a battery coal hauler. He hit a hole in the road jarring his back. He went to the doctor the next day and was placed on restricted duty.

During mining operations on our #2 MMU a methane ignition, bleeder fire, occurred at approximately 1:15 pm. The bleeder fire was approx 3 feet long and burned for about 30-40 seconds. MSHA was notified at 1:25pm and a plan was developed and approved by the District Manager addressing this incident. No miner was injured due to this bleeder fire.

The employee was changing out a V-Wiper at a transfer point. The V-Wiper fell on his hand causing a laceration

The employee was using a hammer to bust a steel band on a pallet of blocks. The band snapped back and cut his knuckle causing a laceration.

The employee was adjusting impeller on a pump. He rolled the pump over by hand and his glove got caught in between the pully and the belt that drives the pump. He fractured one finger.

The employee was hand loading roof bolt glue on a twin head roof bolter and twisted his back....

Injured was cutting bands off of a stack of roof mats. As he cut the band it snapped back cutting his right thumb.

A roof fall was found in the primary escapeway of our S7-1st Right Panel in the Primary escapeway. The fall was found at 11:55pm and phoned into the MSHA Hotline at 12:00am. The fall did not cause any injuries to miners, impede passage of miners and did not affect ventilation.

The employee was working on a battery coal hauler. As he was removing a bolt from the panel his wrench slipped causing his hand to slide across a steel tag cutting his hand. He received 7 stitches to close the wound.

The employee was operating a twin head roof bolter. As he was dropping the boom he inadvertently dropped the pod on his foot. X-rays showed no fractures but a contusion to the foot.

The employee was working on a #4 MMU outby entry mud hole. As he was pushing a scoop through the mud hole, with another scoop, the scoop bounced up and jarred the employees back. The employee was taken to the Dr and put on restricted duty.

The employee was operating a battery coal hauler. As he drove through a check curtain a piece of rock dust entered his eye. He was taken to the Dr and his eye was flushed out and the dust removed. He was given prescription medication for pain.

The employee was installing secondary roof support with a double boom roof bolter. A roof bolt plate caught on a brace on the bolter and cut right thumb.

A roof fall was found at 8:50am in our S7-1st Right Panel at crosscut #12, entry #6, a primary escape way. The fall measured 18'x18'x6'. The MSHA Hotline was notified at 8:55am. This fall did not impede passage of miners, affect ventilation or injure any miner.

A high concentration of methane was found in our 3W-1st Left Seal Area. MSHA was notified at 9:30am and all miners were pulled from the mine. The investigation showed that our #8 De-gas well had been plugged for an unknown reason. A new well was drilled and the methane in the active areas of the mine was reduced to harmless levels.

The employee was drilling in advance of our mining operation. As he was pulling the core barrel out the puller broke causing the core barrel to fall. It struck him in the ankle resulting in a hairline fracture.

The employee was tramming a battery powered scoop in an outby entry. While driving the scoop back to the unit it bounced and the injured hit his head on the canopy straining his neck. He was taken to the Dr and put on restricted duty.

A roof fall was found in our M3 Mains. The fall is specifically located at x-cut 56 in entry #2, a primary escapeway. The fall measured 18' x 17' x 15' high. The fall did not impede passage of miners, affect ventilation, or cause injury to any miners.

The employee was pulling bottom rollers off of the forks of a mini-trac. One of the bottom rollers fell off and struck him in the leg.

The employee walked through a check curtain. A rock fell from the mine roof as he walked through the curtain and as he jumped back he strained his knee.

On 12/29/2012 the employee strained his left arm and neck. On 9/5/2013 he re-injured it while operating a Getman Tractor and was transported to the Dr. and was put back on restricted duty.

The employee was operating a twin head roof bolter. He put his drill steel in the chuck and swung the boom. The steel caught on a hog panel, snapped back and hit the employee in the face causing a laceration.

The employee was installing a rib pin with a twin head roof bolter. A piece of rock fell from the mine roof and hit him on his foot.

The employee was working in the fabrication shop. He stood on a piece of steel, slipped and landed on his hand breaking his thumb

The employee was changing an articulating jack on a battery coal hauler. He put his finger in the pin hole to line it up as another employee was holding the jack with a rock bar. The bar slipped while his finger was in the pin hole and the jack fell cutting his finger.

A roof fall was found in our M4 Mains, x-cut 64 in a neutral entry adjacent to the belt line. The fall was 25' x 20' x 12'. The fall did not affect ventilation, impede passage of miners or cause harm to any miner.

The employee was fixing a hinge pin on a battery coal hauler. The lid fell fracturing his right index finger.

The employee was helping to move a roof bolter. As he tied the cable to the mine roof the wires broke and struck him in the face. His left eyelid was cut and the laceraton was glued to repair

The employee was changing hauler batteries on 5/2/2013. The battery nip slipped from his hand almost hitting his other hand. He flinched and felt pain in his stomach. After consulting a Dr. he was found to have a hernia and surgery was needed. The surgery was completed on 5/28/2013, his lost time/restricted duty will begin on that date.

The employee was working outby operating a battery powered scoop. As he stepped out of the scoop he tripped and hit his head on the scoop cutting his head. He received six stitches as a result of this injury.

Early last year the employee injured his shoulder lifting a steel beam. He was seen by a Dr and was released. Recently he has had more pain in his shoulder, went to the Dr last week and put on restricted duty on 4/4/2014.

The contractor was operating a coal hauler outby the working section on a construction project. As he stepped out of the coal hauler he twisted his knee.

The employee was changing batteries on a coal hauler. As he lifted the battery lid he pulled his groin.

An employee damaged a gate on the material/man hoist located on the North side of our mining operation. The gate was then removed and repaired without further incident. The hoist was still available during this time for emergency evacuation.

The employee started feeling pain in his left arm and neck. After consulting a Dr it was found he had a pinched nerve. He is unsure what he did to inflict this injury.

A roof fall was found on our #5 working section. The fall happened in 5-6 entry in the last open crosscut. MSHA was notified at 8:40 pm. Steps are being implemented to correct this accident. No miners were injured due to this fall

A roof fall was found in No. 2 entry on #4 MMU, last open crosscut. MSHA was notified at 5:40 pm. No Miners were injured during this accident. A plan has been submitted to MSHA to correct this hazard. The fall was in the intersection only.

On December 5, 2012 the employee was installing a drainage pipe at our #2 Impoundment. The pipe rolled and hit one of his fingers. After a follow up visit to the Doctor it has become reportable on January 3rd 2013. (He is now on restricted duty)

The employee was installing roof support with a double boom roof bolter. As he inserted the pusher steel into the starter he hit the rotation and it whipped back hitting him in the thumb. An X-Ray showed a hairline fracture and a cast was placed on his hand/thumb.

The employee was constructing a block brattice. As he was moving a block he tripped over a steel band and strained his back. He went to the doctor on 11/9/2012 and was put on restricted duty.

The employee was moving miner cable. A rock fell between the coal rib and roof panel hitting him in the head. Stitches were required for the laceration

A roof fall was found in x-cut 18, entry #5 in our #4 MMU. This small fall did not affect passage of miners, cause injury to miners or affect ventilation. The cause of the fall, and steps that need to be taken to prevent further falls in this small area of the mine, are under a joint investigation by MSHA and Sunrise Coal.

A small roof fall was found on our company number 4 MMU. The fall was located in the second open x-cut in #6 entry. It measured 10' wide, 6' long, and 3' high. The fall did not cause injury to miners, impede passage to miners or affect ventilation. It was found at 1:30am and called into the MSHA hotline at 1:40am.

The employee was changing a duel on a pan. As he was aligning the nut and bolt his helper tightened the bolt catching his thumb between the bolt and frame of the pan. This resulted in a small fracture to his thumb.

The employee was operating a roof bolter. As he inserted the second steel the steel broke and hit him in the face causing a laceration.

The employee was hanging continuous miner cable and strained his lower back. He was seen by a Dr on 7/2/2012 where he was put on restricted duty.

A roof fall was found by an examiner during a weekly examination. The location of the fall is in our Sub 1C Panel, Entry #3, X-Cut #12. The fall did not cause injury to any miner, impede passage of miners, or affect ventilation.

The employee was hanging a curtain and got dust in his eye. He was seen by a doctor and was given prescription eye drops to prevent infection.

The employee was operating a battery coal hauler. He trammed over a rock in the road bouncing him inside the cab. He sustained contusions to his ribs.

The employee was walking to the mantrip, he slipped on some mud twisting his knee.

The employee was moving miner cable as he bent over to grab the cable he twisted his back.

A roof fall was found in our P2-2 abandoned panel at x-cut 17, intake room 6-7. the fall did not affect ventilation, impede passage of miners and no one was injured due to this fall.

The employee was welding in a gob bin. a foreign object got in his eye.

The employee was lifting a steal cover off of a roof bolting machine. As he lifted the cover he twisted his back. He went to the Dr. on 2/23/2012 and was put on restricted duty.

During a weekly examination a roof fall was found in an abandoned Sub 1-1st Left Panel. The fall was found at x-cut 43 entry 4. The fall did not affect passage of miners, impede ventilation, or cause injury to miners.

The employee was operating a roof bolter and as he reached for a roof bolt he tripped over a rock and hit his elbow on the machine.

An unintentional roof fall was found in our company No. P5-1 Panel, x-cut #7, entry #23, during a weekly examination. The fall did not affect passage of miners, affect ventilation or cause harm to miners.

A roof fall was found by a shift manager in our M3 Main Return Entry. Specifically x-cut 17 entry #3. The fall did not cause injury to miners, affect ventilation or impede passage of miners. It was found at 7:40am and the MSHA hot line was called at 7:45am.

During an idle weekend examination a roof fall was discovered on our 002-0 MMU. The fall is located in the second open cross cut (xc-42) in number 3 entry. The fall was found at 11:07am and called into the MSHA Hotline at 11:17am. The fall did not cause injury to miners, impede passage of miners, or affect ventilation. The fall occurred in an intersection only.

The employee was working on a battery coal hauler. As he was trying to find a hyd leak with his hand he had the hauler operator pressurize the hyd hose. Hyd fluid got in his thumb and had to be seen by a Dr. and put on meds.

The employee was operating a twin head bolter. His drill steel caught on a roof screen when he swung his boom. The drill steel then sprung over hitting his thumb.

A weekly examiner found a roof fall adjacent to the weekly examiners rout of travel. The fall is located in our M4 Mains, x-cut 63, entry #2. The fall did not affect ventilation, impede miners travel, or cause injury to miners. The fall measured 17' x 17' x 8'.

The employee was hanging insulated wire. The wire flipped over and hit him in the eye.

During a weekly examination a roof fall was found in our M5 1st-Left Panel. The fall is adjacent to the weekly examiners travelway in x-cut 21 between entries 3 and 4. The fall did not cause injury to miners, affect ventilation, or impede passage of miners.

The section supervisor found bruising and swelling in his wrist after working day shift on 9/28/11. He is not sure what he did to cause this injury. He followed up with the Dr. on 10/13/2011 and was placed on medication to aid in reducing the swelling. The employee did not miss work and was not placed on restricted duty.

The employee was loading a reducer for a Joy Battery Ram car. He slipped as he picked up the reducer and it fell on his thumb.

The employee was examining the belt entry. He tripped over a bridge and fell on his knee.

A roof fall was found during a weekly examination in our M5 1st Left Panel x-cut 21 entry 4-5. The roof fall is located next to the examiners rout of travel. No one was injured, no equipment was damaged, no one trapped, and ventilation was not interrupted.

The employee was removing a ventilation device with a hammer. As he pulled a spad out of the curtain board his elbow hit a coal rib. He started having pain on 10/7/11 and was seen by a Dr. on 10/10/11 who put him on restricted duty.

The employee was moving belt structure. As a co-worker moved a bottom roller it hit his finger. An accident report was filed that day. He was seen by a Dr. on 10/12/11 due to still having pain in his finger. Medication was prescribed to aid in reducing the swelling. The employee did not miss work and was not placed on restricted duty.

the employee was operating a diesel scoop. He had his leg resting on a hot hyd hose in the cab and burnt his leg. He seen a Dr. on 9/2/2011 due to infection and was placed on restricted duty.

The employee was advancing a belt line. He hit his finger between a bottom roller and the coal rib. No fractures were found but a prescription was given for pain.

During an examination of our redundant phone line a roof fall was found in the primary escape way, M4 Mains, XC 64, Entry #6. No miners were injured, ventilation was not affected, and no miners were trapped due to this roof fall.

A roof bolter operator was removing 2'x 4'x 18' boards from his machine. As he was removing a board he was bit by a spider. He was seen by a Dr. on 8/9/2011 given medicine for infection caused by the bite.

During a weekly examination a weekly examiner found a roof fall in our M4 main. The fall was located in #7 entry at x-cut #71 adjacent to the primary escapeway. The fall did not affect ventilation, passage of miners, or cause injury to miners. The fall was 37' x 17' x 8'.

A roof fall was found by a weekly examiner in our M5/1st Left abandoned sub-main, cross-cut 21 entry #6. The fall measured 20' x 16' x 15'. It is not located in or adjacent to the examiners route of travel, it did not cause interruption in ventilation, impede passage of miners or cause any injury to miners.

The employee was working on a bolter cable on number 4 MMU. As he was fixing the cable a piece of rock dislodged from the mine roof and struck the employee fracturing his pelvis.

A roof fall was found during a weekly examination. The roof fall measured 20' x 17' x 8'. It was located in our M3 Main Return x-cut #14, entry #2. It was not located adjacent to an escapeway. no miner was injured, ventilation was not affected and travel was not affected due to this roof fall.

Employee was stopped on equipment. He was adjusting his CB radio antenna, which is located on the outside of the equipment. While moving the magnet to which the antenna is attached, the antenna flipped back and hit him in the right eye. The sclera (white part) of his right eye was lacerated.

The employee was operating a continuous miner. As he was mining a piece of mine roof spun off of the head of the machine and struck the employee in the face causing a laceration.

The employee cut a steel band on a skid of block. The band slung towards the employee hitting his arm causing a laceration.

The employee was moving a stored SCSR Cache and felt a tear feeling in his abdomen area. After consulting with a doctor on the following Monday it was determined that the employee was in need of surgery do to a hernia. The employee was placed on restricted duty.

Employee was working and at some point was bitten by a spider.

As the miner operator was loading his last car on the left side of a blow through he noticed a pile of coal in the cut was possibly burning. It was determined through company and MSHA investigation that a methane bleeder from the mine floor and sparks from the miner bits is what possibly caused the ignition. No one was injured and no equipment was damaged.

The employee was installing a pump and hit his knee on grating. He went to the Doctors office on 5/16/2011 and had to have fluid drained behind his knee.

The employee attempted to cross over an idled conveyor belt thinking it was locked and tagged out. As he stepped on the belt it started causing the employee to ride the conveyor belt app. 60' traveling under a crossover before another employee shut the belt off.

The employee was standing next to a twin head roof bolter during drilling operations. As the operator advanced to install the next row of pins he ran over the employees foot causing bruising and swelling to his foot.

The employee was loading bottom rollers onto a structure trailer. As another employee threw a bottom roller it caught the injured employees finger between two rollers.

The employee was removing a guard on a dozer. He slipped and landed on his side fracturing two ribs

A Continuation of a roof fall that was reported on 7/16/2010 was found during a weekly examination. The fall was found in our M4 Mains, X-Cut 72, #3 entry and was the intersection only. It measured 20' X 20' X 15' high. The continuation did not affect passage of miners, affect ventilation, or cause injury to miners.

The employee was retrieving HV cable. As he and three other miners were pulling on the cable it snagged on a crib. As the cable suddenly stopped the employee felt a strain in his groin and stomach. After consulting a DR. on 2/25/11 it was determined he has a hernia and will need surgery.

The employee slipped on ice as he was walking across the plant yard. Since his accident the pain has gotten worse, he is now required to have surgery to correct the problem.

During a weekly examination of our M3 Main Returns a roof fall was found at x-cut 26, entry #1. The fall was found in the intersection only. It measured 20' long, 18' wide, and 15' high. the fall did not cause personnel injury to miners, impede passage of miners or affect ventilation.

The employee was operating a roof bolter. He placed his hand on top of the operators canopy and swung the boom pinching his hand between the mine roof and the canopy.

The employee was bolting. As he attempted to put a bolt in the drilled hole, using the hyd. of the bolter, the bolt bent and hit his hand which caused a strain.

The roof fall did not cause personal injury to miners, affect ventilation, or impeed passage of miners.

The employee was hanging a continuous miner cable and strained his back.

The employee was installing roof bolts. As he started the drill steel in the mine roof a peice of mine roof dislodged from the mine roof and struck the employee.

The employee was installing roof support on the off side of a twin head roof bolter. As he was standing at the middle of the machine the operator inadvertantly ran over his foot.

The employee was standing on a three foot ladder and fell on his side. He went to the Dr. on 9/21/2010 and was put on restricted duty.

An unintentional roof fall was found in our M4 Main, Entry #1, Cross Cut #75. This fall is located in one of our return air courses. The fall did not cause injury to miners, affect passage of miners, and did not affect ventilation. The fall measured 18'X20'X10'high.

The employee was hauling supplies down the slope with a getman tractor. He lost control and hit an arch. His head was thrown into the canopy causing a laceration to his head.

An unexpected roof fall occurred in our M3 Mains, x-cut 26, entry #7. The fall did not cause injury to miners, affect passage of miners, or change ventilation.

The employee was using a slate bar to straighten up a cable horn on a continuous miner. The bar slipped, he lost his balance and twisted his ankle.

The employee was pulling on a chain from under a slinger duster and strained his groin. He had to have surgery on 8/31/2010 as a result of the injury.

A roof fall was found in our M4 Main x-cut #67, entry #8. The roof fall is 20' wide, 17' long and 7' high. The roof fall did not cause injury to miners, impede passage of miners, or affect ventilation.
