Oaktown Fuels Mine No. 1 Coal

An unintentional roof fall occurred on the #21 cross cut entry #4 (travel way) of the 2C. The fall is approximately 30 feet in length, 27 feet wide and 10 foot high.

Employee was standing at the back of a battery coal hauler loading hog-panel in the bed when the operator inadvertently articulated bed down on employees right foot, fracturing foot.

Employee was trying to break apart a threaded water-line coupling, straining left shoulder. Employee has had MRI. On 10/29/2025 MRI results showed 2 tears in shoulder. Was seen by Ortho on 10/30/2025 and has surgery pending. Employee remained working till 10/29/2025.

Employee was pulling on belt jumper at Unit 2 beltline cross cut #1, entry #5, when they felt a burning pain in pelvic area. On 12/08/2025 employee was diagnosed with a hernia. Employee has worked normal duties and will continue to work until surgery date 12/30/2025.

Employee cut band on timbers. the timbers rolled off the bundle striking employees left thumb. X-Ray showed a fractured thumb.

Employee states they were lifting an equipment door when they heard and felt their right arm popping in the bicep and elbow area. MRI has shown a torn bicep muscle and the employee is scheduled for surgery. Employee has continued to perform normal work duties.

Employee was on the M7 cross-cut 32-33, hanging water line when they felt knee pop and fell to ground in pain. Employee continued to work until seen by Doctor on Monday September 15, 2025. Employee was placed on light duty and scheduled for MRI.

Employee was operating miner, when a rock measuring approximately 2ft.x2ft.x1inch fell out between hog panel and coal rib. Striking employee on hard hat and shoulder area. Employee has continued to work until 10/29/2025 when they were placed on restrictions, pending physical therapy.

Employee was setting up breaker on scoop, when the plastic knob on lever broke causing it to lacerate right hand. Employee was taken to hospital where they applied stitches.

Employee was taking top roller off belt structure when they felt a pull in their abdominal area. On 09/24/2025 we was notified that employee will be having surgery for hernia on 10/03/2025.

It is believed that water deteriorated the immediate roof causing it to fail, then allowing the upper strata to become unstable and overcome the supplemental roof support that had been installed causing the roof fall.

Employee stated EE grabbed the post on the mantrip to get in when another employee shut the mantrip door, striking EE's left pinky finger.

Roof fall, SC4 crosscut #15 Entry #5. Black shale and stack rock approximately 20x30 feet.

Employee was operating #29 Coal Hauler when they drove over a hole or something in haul-road, causing the employee to move in the seat fracturing EE's left side rib.

While employee was roof bolting when a piece of rock fell from top striking EE's left thumb. Employee was taken to hospital where they determined EE had a fractured thumb.

While getting in coal hauler employee had right hand on fender of hauler, when a rock struck employees hand requiring 1 stitch. Employee returned to work the next day.

Outby roof fall on the SD-4 XC 18 Belt Line.

Employee was pulling cable through intake wall on M2 crosscut #31 entry #6 when EE's right foot got caught in a pallet of block, causing EE to fall, twisting EE's knee. Employee had MRI and placed on light duty and referred to Ortho.

The employee was loading a bundle of roof bolt plates onto EE's bolter tray when the plates cut the employee's hand between the thumb and index finger. the laceration required 2 stitches to close the wound.

The employee was getting out of the mantrip at the end of the shift and cut EE's leg on a piece of metal. The employee was then taken to our clinic and received 2 stitches in EE's lower left leg.

Employee shut mantrip door on left hand, breaking and lacerating their left ring finger.

Employee was putting water on Unit#3 E-sled at when EE slipped on a rock twisting EE's right knee. A recent MRI has shown a meniscus tear with surgery scheduled for October 10th. Employee has been working normal duties and will continue until surgery date.

While setting timber, employee caught right middle finger between timber and roof top cutting the finger. Employee was taken to clinic where they applied stitches to the laceration. Making this a Reportable Accident.

Stack rock transition to sandstone, fell in intersection above anchorage point.

While the elevator was going down into the mine, the power was lost causing the elevator to shut down. After this happened it caused a guide roller to become out of adjustment.

Employee was running miner, waiting on hauler to come in and was walking towards miner when EE tripped over rock injuring EE's right knee.

Pinner steel stuck together, pulled bottom steel to loosen connection and pinner steel came apart at connection striking employee in lower front teeth.

An unplanned fall occurred on the M17 1st. right entry 5 crosscut 18-19. The fall measured approximately 40' long 18' wide and 10'in thickness.

Employee was lifting metal off table and felt pain in their upper leg. Employee was taken to ER and placed off work until 05/05/2025 and must be released by ortho.

Injured was pulling on top roller on 4/8/25 and felt pain in arm but continued to work. On 5/13/25 injured was seen by doctor as EE's pain had not went away at that time injured was diagnosed with a partial bicep tear and placed on restrictions. With a possible surgery for repair scheduled.

Employee was stepping off of tractor onto the steering cylinder twisting their knee. Employee has been working with no restrictions and on 04/24/2025 Doctor decided they needed surgery.

Employee was removing a Kennedy wall and injured the left knee while kicking at one of the panels to break it free. An MRI showed a tear of the medial meniscus.

While cutting 8 right a piece of rock dislodged between the roof bolts and struck the employees right side causing them to land on the left shoulder. X-rays showed a tuberosity nondisplaced fracture of the left shoulder. Employee was placed on restrictions.

Employee was getting out of a battery scoop and felt back pain. Employee has been working without restrictions, but still was having some pain. Employee had an MRI done on 2/7/2025 and was placed off work for a possible herniated disc.

Employee was changing a hauler battery and felt a pulling sensation in the groin area. Employee was placed on restrictions for a possible hernia.

Employee bent down to lift a rail and felt the right knee pop and felt like it was grinding. An MRI on 1/8/2025 showed that the right knee had a meniscus tear. Employee is being scheduled for surgery.

Bolter operator was trying to remove a rock approx. 2'x3'x3" from the inside of the right-hand boom and the ATRS frame by hand. Operator was lifting and pushing at the same time when the rock broke, causing the operator to lunge forward and strike EE's right forearm against the rock, creating an avulsion Approx. 2.5"x3.5".

Employee was using an old drill steel to pry against a bundle of roof bolt. The drill steel broke when employee was applying pressure and lacerated the right forearm. 15 sutures were required to close the wound. Employee is off with restrictions.

Employee was walking through a wet area on the weekly exam and slipped, causing a twist/popping to the left knee. Employee was placed on restrictions.

Employee was installing a roof bolt. When the bolt was placed in the chuck the employee bumped the rotation lever. A piece of the glove was between the chuck and the bolt causing the glove to catch and twist. Employee fractured the right thumb.

Employee was beside the roof bolter and encountered some floor irregulars with the right foot. The employee twisted and sprained the right ankle. Employee was placed on restrictions.

Employee stopped to tighten the ratchet straps on a loaded trailer. During the process of tightening they felt a pop in the shoulder. Employee was placed on restrictions for a right shoulder strain.

Employee was pre shifting the unit and stepped in a hole twisting the knee. Employee has been working without restrictions but still having pain. An MRI on 12-12-24 showed a tear to the right knee meniscus. Surgery is being scheduled.

Employee was carrying a timber through a muddy area when the foot got stuck causing the knee to twist. Employee was placed on restrictions. An MRI shows a torn meniscus.

An unplanned roof fall occurred inby the loading point on the Unit #3 section. The fall measured approximately 123' long, 45' wide (up the entry) and 10' plus in thickness,

Employee was changing batteries on the coal hauler when they tripped on the jumper cable and fell on the edge of the battery with the right shoulder. Employee has been working without restrictions with no signs of improvement. An MRI on 11-14-24 showed a tear to the rotator cuff. Surgery is scheduled for 12-6-24.

An unplanned roof fall occurred on Unit #3 in entry 8 xc 15. The fall measured approximately 20' in length, 20' in width, and 9' thick.

Employee was pulling up bolter cable and slipped in the mud, twisting the left knee. Employee received a left knee meniscus tear.

Employee was a passenger in the #58 mantrip when it collided with the corner of the coal rib at the S3-B and M17 turn. Employee received fourteen staples to the head.

Employee was a passenger in the #58 mantrip when it collided with the corner of the coal rib at the S3-B and M17 turn. Employee received a fractured L1 and a fractured knee.

Employee was a passenger in the #58 mantrip when it collided with the corner of the coal rib at the S3-B and M17 turn. Employee received a left shoulder and back strain.

Employee was a passenger in the #58 mantrip when it collided with the corner of the coal rib at the S3-B and M17 turn. Employee received a concussion and a left forearm contusion.

Employee was the driver of the #58 mantrip when it collided with the corner of a coal rib at the S3-B and M17 turn. Employee had a right ankle sprain.

Employee was a passenger in the #58 mantrip when it collided with the corner of the coal rib at the S3-B and M17 turn. Employee received a fractured knee.

Employee was drilling a hole in the top, when steels were added, there was a slight angle and employee did not realize the steel was not in the chuck all the way. When the boom was raised the steel popped out of the chuck and struck employee in the hand resulting in a fracture to the second metacarpal.

Employee was walking beside the roof bolter when a rock fell out between the bolts and struck employee on the hard hat then breaking and striking the shoulders and back. Employee was placed on restrictions for a spine strain.

Employee was helping change a top roller on the M3 beltline when the 2nd and 3rd digit on the left hand got smashed between the side rail and the roller frame. Both fingers have a fracture and the 2nd digit had a laceration that required sutures.

Employee was helping replace the diamond bar on the #5 roof bolter and smashed a finger on the left hand. Employee received sutures and was released without restrictions.

Employee was bolting in the face of entry 1 when the roof plate they were installing got stuck on their glove. When the plate released it slid down the roof bolt and cut the arm. Employee received five sutures and was released with no restrictions.

Employee was installing chain hanger when the corner of the rib came loose as the employee was swinging the boom out. A piece of the rib struck the lower back area. Employee was placed on restrictions for a lower back contusion.

Employee was in the process of installing a curtain board when they attempted to grab the stab jack lever but instead swung the canopy and hit themself in the head. Employee was diagnosed with a mild concussion and placed on restrictions.

Employee was walking to their coal hauler when a rock fell between the bolts and struck the back of the neck. Employee was placed on restrictions for a cervical spine strain/sprain.

Employee was walking to get their coal hauler when they got tripped up and fell landing on the right arm/shoulder area. Employee was placed on restrictions.

Employee arrived at tool hole to find it flooded. While running to see if pump at the nearby sump was off, employee tripped and landed on the right wrist. Employee was placed on restrictions.

Employee was lifting a piece of steel channel and felt a pull in the back and a burn down the right leg. An MRI showed a hamstring tear and employee was placed on restrictions.

An Intersection of the S2E beltline at XC-37 Entry 5 fell covering the belt structure and blocking the travel of the examiners and belt air in that entry only.

Employee was walking from the beltline to the haulroad when they tripped and fell landing on the buttocks. Employee has been working full duty and doing at home therapy but was still having symptoms. A follow up appointment was made and surgery was scheduled for 11/13/2024 for a Lumbar Radiculopathy. Employee worked full duty until surgery day.

An unplanned roof fall occurred on the 2D travelway at crosscut 13. The fall measured approximately 20' long, 18' wide, and 10' thick.

An unplanned roof fall occurred on the P2-2 beltline at crosscut 1. The fall measured approximately 40' in length, 18' wide and 15' thick.

A pallet was hanging over the side of a dumpster. Employee tried to push the pallet into the dumpster and strained the right bicep. An MRI was done on 7-2-24 and it showed a tear to the bicep. Employee was placed on restrictions and surgery was scheduled.

Employee walked the primary that evening and woke the next day with soreness and swollen legs. At first the employee attributed to age, but realized it wasn't getting better. Reported to supervisor on 06/30/2024. An MRI was ordered on 07/15/2024 and on 07/19/2024 employee was placed on restriction for L3-4 disc herniation.

A rock fell from the top and struck employee in the knee. Employee was placed on restrictions.

While employee was loading roof bolt plates onto the tray of the bolter, the stack of plates fell over and struck the right hand. Employee was cut on the hand between the thumb and the index finger. Employee was placed on restrictions.

Employee bent over to lift the miner cable and felt a pain in the back. At a follow up appointment on 5/31 the employee was placed on restrictions and an MRI was scheduled for 6/11/2024.

Contractor was cutting the bands on a new dust hose when the knife went through the bands and into the lower palm on the left hand. Taken to the ER for four sutures.

An unplanned roof fall occurred outby the 2D xc 1 intersection in entry 5. The intersection had been previously cribbed and railed. The support in the intersection took weight and the pressure relieved itself outby the support. The fall measured 15' long, 17.25' wide and approximately 8'-10' thick.

Employee had just drilled a hole in the top. When the drill steels were being lowered the starter steel stayed in the roof. Employee noticed the insert was missing on the pusher steel and placed the left hand over the suction so their partner could hear them ask for advice. That is when the steel fell from the top and struck the middle finger.

Employee was walking away from the drive area and tripped and fell to the knees. The right knee landed on something hard and employee noticed some pain. On 4/16 employee went to the clinic for evaluation, After 4 weeks and still not better, employee was sent for MRI. It was determined on 5/20/24 that there was a tear to the meniscus. Surgery is scheduled for 5/24/24.

Employee was operating the #55 diesel mantrip when they lost control and struck the north concrete retaining wall at the base of the highwall in the pit. Employee received sutures in the bottom lip.

Employee was running the roof bolter and lacerated the right arm on a stack of roof bolt plates. Employee received five sutures.

Employee was assisting in the changing of a hauler battery when the driver backed up to far and the jumper cable got tight and took the feet out from under them. Employee was taken to the ER and treated for a broken ankle.

Employee was tossing a timber when it slipped and impacted a roof jack, causing the pinky finger to get smashed. Employee received sutures to the finger.

Employee was helping hang miner cable and felt something pop in the right shoulder. Employee was placed on restrictions.

An unplanned roof fall occurred on the 2D haul road (entry 4) at crosscut 2. The fall measured approximately 18' in length, 18' in width and 10' thick.

An unplanned roof fall took place on the 2D escapeway in entry 6 at crosscut 10. The fall was measured at approximately 47' in length, 17'-18' wide and 6' thick.

Employee was pulling on a steel I beam when they felt something pull from the wrist to the elbow. Employee had been working with no restrictions but was still having pain. A follow up on 1-24-2024 with the DR and an MRI showed the employee had a partial tear to the bicep tendon. Surgery is scheduled for 1-31-2024.

Employee was lifting a battery lid on a coal hauler when they felt pain on the right side of the lower back. MRI results on 10-25-23 showed a lumbar strain. Employee is working with restrictions.

Injured was walking from entry 5 to entry 6 at xc 26 when a rock fell from the corner and hit the hard hat and shoulder. Injured received 6 staples to the shoulder area.

Employee was helping hang the feeder cable and felt something pop in the right shoulder down to the elbow. Employee has been working WITHOUT restrictions but an MRI review on 10-27-23 showed a tear to the rotator cuff. Surgery is scheduled for 11-03-2023.

Employee was handling a battery jumper at the charging station when they swung the jumper onto the battery and felt pain in the back. Employee was placed on restrictions for a lower back strain.

Employee had walked outby to get hog panels when a rock fell and struck employee on the foot. The employee had injured this same foot in the past. X-ray showed a fracture to the 3rd metatarsal on the left foot. Employee was placed in a moon boot.

Employee was operating a battery scoop when EE struck their head on the canopy. Employee was placed on restricted duty for neck/head pain.

Employee was installing a tram motor on a scoop. When the employee dismounted from the scoop, EE felt a pop behind the toes of the left foot. Employee was placed on restrictions.

Employee was working on the unit feeder and slipped on a pile of coal. The knee twisted one way and the body went the other. An MRI on 8-22-23 showed a tear to the meniscus. Surgery is scheduled for 9-1-23

Injured had finished EE's first cut out of the 1R. EE was backing the continuous miner out when a rock fell and struck EE from behind. The rock knocked EE forward and came to rest on the lower part of the legs. The injured received a subtle L1 fracture, a 2nd metatarsal fracture and both great toes were dislocated.

Injured was leveling belt structure and turned to take a step, that is when EE felt a pop in the knee. An MRI on 7/24/2023 showed a small bursa/meniscus tear. Surgery is scheduled for 8/4/2023

An unplanned roof fall occurred on the haul road of the S3-B between crosscut 35 and 36. The fall measured 36' in length, 15' wide and approximately 7' thick.

Injured was driving an insert in the drum of the continuous miner when a small object flew and hit injured in the upper part of the right cheek. Injured had small sliver of metal removed from the cheek and received sutures.

Injured was hanging curtain wire from the mine roof. When EE turned, the wire got caught under the safety glasses and made contact with the eye. The injured was diagnosed with a corneal abrasion to the right eye. Injured was given prescription eye drops and placed off work for 2 weeks.

Injured was pulling on a loop of cable to place on the continuous miner when they twisted the knee and instantly felt pain. An MRI showed a left lateral meniscus tear. The injured is waiting on a surgery date.

An unplanned roof fall occurred in the Main East haul road at crosscut 84. The fall measured 35' in length, 61' in width, and approximately 12' thick.

An unintentional roof fall has occurred on the 1st North 1st Right Belt line at the #12xc. The fall occurred above the anchorage horizon of the supplemental support and the primary support that had been installed in the area. Supplemental support has now been installed in the immediate vicinity of the fall area.

Injured was pulling PTA cable around the corner and slipped and fell on the concrete at belt drive, hurting the left shoulder. After an MRI on 3/20/2023 it appeared that the left shoulder had a tear in the rotator cuff. Injured was placed on restrictions awaiting a surgery date.

An unplanned roof fall has occurred on the Main East at xc 1 between entry 5 and entry 6. The fall measured approximately 30' long, 17' wide and 18' thick.

Injured was raking concrete and felt something odd in the left shoulder. After some physical therapy and a second opinion it was determined that there was a tear in the rotator cuff and surgery would be scheduled for 06/15/2023.

An unplanned roof fall occurred on the SB-4. The fall took place at crosscut 30, in entry 4 and 4-5. The fall measured approximately 20' long, 40 wide and 20' thick.

Injured and partner were installing tail skirts while a crew was adjusting/setting the remote discharge. The discharge swung and a come-a-long came undone allowing the head roller of the discharge to fall onto the tail landing on the injured leg/foot. Injured received a severe contusion to the lower left extremity.

An unplanned roof-fall occurred on the M3 travelway at crosscut 34 entry 4.

Injured had just finished drilling a hole in the top. The pusher steel became lodged in the drill pot when the steels were lowered. Injured was spinning the drill steel back and forth to free the lodged steel when EE's glove/sleeve got caught by the steel and twisted the arm. Injured received a fracture of the midshaft radius and ulna.

Injured was lifting discharge line to the mine roof. The pipe shifted and the injured lost footing causing a popped feeling in the knee. An MRI on 12/2 showed that the meniscus in the right knee was tore. Surgery is scheduled for 12/16/22.

Injured was loading 6' timbers into a scoop bucket when they felt something awkward in the shoulder area. After weeks of physical therapy, the ortho suggest that rotator cuff surgery will be needed. Surgery is scheduled for 5/5/2023.

Injured was walking toward the M10 belt drive when injured tripped over a belt block that was left behind. As injured lost balance the shoulder came into contact with the coal rib. An MRI on 11-10-2022 showed a tear to the rotator cuff. The injured was placed on a 25lb work restriction. On 1/16/2023 they scheduled surgery for 1/18/2023.

Elevator showed a brake limit switch fault. The brake adjustment had come loose.

An unintentional roof fall occurred on the ME beltline at crosscut 48. Earlier in the month the area outby was found with a crack in the roof. The said area was cribbed, and rail barred. It is believed that the pressure in the top shifted and caused the fall at crosscut 48.

Injured was connecting steel to steel while roof bolting. The steels were not properly aligned when pressure was applied. The steel popped the injured on the back of their hand. EE was diagnosed with a fracture at the base of the thumb.

An unintentional roof fall has occurred on the S3B beltline between crosscuts 37 and 38. The fall measured approximately 46 feet in length, 17.5 feet in width and 7 plus feet high. The roof fall occurred on the idle weekend. An action plan has been submitted and approved to start rehabilitation of the affected area.

Injured was standing beside the roof bolter when a rock fell out and pushed EE's face into the guarding of the side controls. Injured was treated for laceration to the lip and received 9 sutures.

Injured was driving a coal hauler away from the continuous miner and ran over a hole in the road. Injured hit head on the canopy of the hauler and was diagnosed with a mild concussion.

Injured was helping make a splice on the S4-A beltline. When the injured reached for the rivet plunger, another employee was in the process of swinging a 4lb hammer. The hammer struck the injured on top of the hand and broke the left index finger.

While helping to get #14 mantrip started, the injured was resting hand on top of the radiator. When the mantrip started the fan severed the right index finger.

While roof bolting in entry five, the injured connected the pusher and the starter steel together. The bolter chuck and the steels were misaligned when pressure was applied and the steels popped the hand. The injured fractured the proximal phalanx of the right index finger.

Injured was outby the unit spotting hog panels. Injured had just finished hanging fence and injured's partner had backed the bolter up when a rock fell out striking injured in the foot. The injured had a displaced fracture of the fifth metatarsal bone.

Injured was placing jacks in the beltline. When injured went to lift a jack from the pile it became unbalanced and fell. Injured grabbed for the falling jack and it smashed injured's fingers between the falling jack and the pile of jacks. Injured received sutures to the left index finger.

Injured was lifting a 6' timber when EE felt something abnormal in their arm. EE went home after EE's shift and applied ice. The next day EE was still in pain and was sent to the doctor. A follow-up appointment on 7/20/2022 determined that the right bicep had a tear and was going to require surgery.

Injured and EE's partner had just finished bolting the face of entry 5. They were both on the same side of the bolter discussing their next place to go when a rock fell from the corner of the rib and top striking the injured in the ankle/shin area. Injured was transported by ambulance with a broken fibula.

Injured was lifting a lid on a coal hauler when EE's right arm gave out and EE fell towards the ground jerking EE's arm. Injured felt pain but kept working and making adjustments to be able to function. On 07/21/2022 the injured was pushing backwards and the pain worsened. It was determined on 07/22/2022 that the injured needed the subscapularis tendon repaired.

An unintentional roof fall has occurred on the 1 North A at xc41 in entry 6. The fall was approximately 40' X 17 1/2' approximately 12'-14' high

Injured was going down the slope when the Dapco started to slide. Injured started tapping on the brakes but could not get it under control. The Dapco came to a stop when it hit the barricades at the bottom of the slope. Injured received stitches above and below the right eye.

Operator was driving the #2 Dapco down the slope when they encountered a slick spot due to the humidity. Operator slid until it was stopped by the safety barriers at the bottom of the slope.

Bolter operator backed up to put a hog panel on the ATRS. While putting panel on a rock fell from the top and hit the ATRS causing the rock to flip back and strike the hand between the canopy and the rock.

Injured bent down to pick up a saddle (belt structure). When injured stood up EE's shirt got caught causing EE to lose footing. EE fell and smashed EE's fingers between two saddles. This resulted in a fractured little finger with six sutures.

Injured was attaching two drill steels together. The steels were not completely connected when hydraulic pressure was applied. This caused the steels to come apart and strike the injured on the hand.

Injured was helping line the bearings up for the feeders gear box when the gear box slid off the forks and pinned EE's leg between the feeder and the gear box.

Injured was loading bolts onto the roof bolter and the bundle of bolts slid and struck the ring finger on the right hand. This caused the end of the finger to deglove and fracture at the tip.

Injured was trying to break a bolt loose on a scraper. When pushing on the wrench, the injured felt a pop and then pain in shoulder. An MRI on 4/29/2022 showed a shoulder supraspinatus partial thickness tear.

Injured was raising the bolter pod to insert an extension for the drill steel and popped hand resulting in a broken thumb.

Elevator went into alarm for a fault, after further investigation one of the brake pistons was not actuating properly. At that time the elevator was taken out of service and the repairs began to correct the problem.

Injured was setting the miner over. While walking backwards EE lost balance and fell. A rock struck EE's face and caused a laceration. Injured also strained ACL on right leg.

The injured was unbolting the shaft from the fan, when the injured twisted and felt a pop in the left knee causing a tear in the lateral meniscus. Injured was placed on restrictions on 1/5/2022

Injured was driving down travelway over corrected and front right bumper connected coal rib causing mantrip to come to stop, causing injured nose to come in contact with the steering wheel. This caused a laceration and fracture of the nose. Injured received stitches for the injury.

Injured was stepping over a concrete slab and stepped awkwardly coming down and strained left leg. On 12/9/21 injured was seen by doctor and placed on restrictions for knee strain and sent for MRI then on 12/14/21 MRI results revealed a tiny peripheral tear of the meniscus.

Injured reported on 12/2/21 that EE had strained EE's back on 11/16/21 while operating a battery scoop. Injured has made an appointment for 12/6/21 and on this date was placed on restricted duty.

Injured was walking to hook up trailer on the pickup truck and strained knee. Continued to have pain and then on 11/24/21 MRI results revealed a torn meniscal in EE's right knee and was placed on a restricted duty.

Injured was lifting on a piece of belt and strained lower back. Injured was seen on 11/5/21 for the injury and placed on restrictions for this.

Injured was roof bolting on the #1 Unit in the #9 Entry and while raising the boom to connect the pusher steel, EE hit their hand with the steel. Injured was seen the following day and diagnosed with a hand contusion and placed on restrictions.

Injured had just parked the bolter and was getting out to walk up to front controls and a piece of draw rock fell and struck in the Shoulder. Injured was placed on restrictions for a bruised shoulder.

Injured and another employee were putting covers back on the Roof bolter and when they went to pick up the third cover injured said EE felt something pull in EE's left bicep area. Injured was taken for medical attention and then placed on restrictions pending MRI results.

The elevator would not run in inspection mode in the up direction. During the troubleshooting the elevator became inoperative at 11:40am. During the investigation it was found that the wires in the jacketed cable to the pendant controller were broken.

The emergency hoist was being inspected during the quarterly exam and when the examiner engaged the down function on the hoist the hoist malfunctioned and started unwinding while still sitting on the surface platform. During the investigation it was discovered that the cage rest sensor was bad.

Injured was scaling the roof in entry 5 and scaled a piece down then the piece in front of the piece that was being scaled fell also causing the first piece to bounce and strike the injured below EE's knee. This caused hematoma and leg contusion EE's right leg.

Injured was walking and stepped on uneven surface at a battery station twisting a knee. On 5/14/21 injured wanted to have knee looked at, so was set up to have MRI done on knee and was placed on restrictions on this date.

Injured was lifting a drill feed cylinder on 4/27/21 and claims they strained their back and then on 5/12/21 injured reported that they had hurt their back a few weeks ago and wanted to be seen by Doctor. Injured was seen and was put off work.

Injured pulled on piece of tie wire that was overhead and when doing so the tie wire did not give causing the tip of finger to break. Injured was diagnosed with a non-displaced spiral fracture and placed on restrictions.

Injured was pulling into a crosscut on a battery transporter. EE was hit by a plastic pipe causing contusions to the leg.

Injured said they hurt their back on 4/9/21 but did not report the accident until 4/19/21 and then, was sent for observation and placed on restrictions the same day for a back strain.

Injured was picking up a bucket of miner bits off of the miner and said EE felt a pop in EE's back, and continued to work but started experiencing upper back pain and on 4-5-21 injured was placed on restrictions for a herniated disc.

An unintentional roof fall has occurred on the S4M at the 11xc of the #5 Entry the fall occurred on the idle weekend and was discovered during the fire watch.

Injured was lifting a battery lid and strained back. On 12/11/20 injured reported the injury to safety department and was sent for medical attention. On 12-12-20 injured was placed on restrictions for strained back.

Injured was walking around the feeder to meter a cable and hyper extended left knee. Injured continued the shift and then on 11/2/20 went for medical attention and was then placed on restrictions for left knee sprain.

Injured claims hurt arm on 10/6/20 but did not fill out report until 11/2/20 and then was sent to DR. on 11/4/20 and was recommended for an MRI. ON 11/10/20 results from the MRI revealed a Bicep tear.

During an inspection of the Emergency hoist it was discovered that the brake pad for the drum had pulled through the rivets and caused the pad to break in two. This was the only braking system for the drum as there is no secondary system. The hoist was immediately taken out of service and repairs started.

Injured was picking up a cable and pulled a muscle in their back and was place on restrictions on 9/15/20 for the injury.

Injured hit EE's elbow on a scoop canopy while trying to move a curtain. Continued to work but on 9/3/2020 had EE's elbow drained because of an olecranon bursitis on EE's elbow. EE was placed on restriction on this date also.

Injured was helping lift a 4" empty waterline and strained EE's lower back. Injured was placed on restrictions on 7/14/20.

Injured was picking up structure and twisted a knee. This resulted in injured being placed on restrictions for the injury

Injured was unloading a pallet with a canopy jack on it and the jack slipped and smashed EE's pinky finger. X-rays were taken and the finger was broken.

Injured was swinging a sledge hammer underground and pulled a muscle in back. On 5/11/20 employee was placed on restrictions.

Injured strained EE's back while bending over to pick up a come-along and was placed on restrictions on for this incident.

Injured was dragging a suck line for a pump and when EE stepped over a berm EE said EE felt a pop in EE's knee. Injured was seen by medical personnel on 3/16/2020 and placed on restrictions.

The elevator at the south portal went into alarm and stopped due to a governor rope switch that got out of ailment at the bottom of the shaft. There were three examiners on the elevator at the time the elevator went down.

Injured claims they were just bending over while underground and felt a sharp pain in back. Injured was diagnosed as having a Myofascial strain in back.

Injured was lifting a roll of curtain and said EE felt a pop in EE's shoulder. On 2-12-2020 injured had an MRI and was placed on restrictions for a shoulder strain.

Injured was walking toward the continuous miner along the intake side of the ventilation curtain when a battery hauler was leaving the miner and as the hauler was leaving it turned and the tailgate end of the hauler struck the injured causing a pneumothorax and a broken rib.

Injured claims they strained their back when they reached over their head to get a curtain in place in the #8 entry. Injured was placed on restrictions for their strained muscle.

An unintentional roof fall has occurred on the working section the fall was located at the 20th XC of the S4I between the entries of 9 and 10. The fall was approximately 30 feet by 17.6 feet by 6.9 feet high.

Injured was trying to remove a drill steel insert out of the drill steel and when the insert broke free it struck them on the cheek causing a laceration that required stitches.

Injured was installing a pan jack the miner and smashed a finger and received stitches for the injury.

Injured claims injured was servicing the miner and turned and felt a pop in right knee. Injured was taken for medical attention and then given an MRI and revealed injured had suffered a small tear to the meniscus.

Injured was tightening up the bolts on a set of belt blocks and the wrench slipped causing discomfort in left shoulder. Injured was sent for an MRI which revealed a slight rotator cuff tear. Injured as placed on restrictions.

Injured was building a hollow block stopping and strained a shoulder lifting the block. Injured was placed on restricted duty on 10-15-19.

Injured was lifting cable and felt a pop in EE's shoulder. Injured was taken for medical observation and placed on restricted duty.
