A ROOF FALL OCCURRED ONE BREAK INBY PAD 79 IN THE INTERSECTION APPROX. SOFTWIDE 20 FT IN LENGTH, AND APPROX 30 HIGH 42 CONVETIONAL BOLTS WERE INSTALLED ON 4X4 CENTERS AND TWO ROW OF TIMBERS W ERE SET THE CAUSE OF THE FAIL WAS A FAULT IN ROOF. WE WERE FOLLOWING THE APPROVED ROOF CONTROL PLAN.

A ROOF FALL OF 48" THICH AND 20 FT. LONG OCCURRED IN THE NUMBER 2 RETURN AIR COURSE. THE FALL FELL AT THE ANCHORAGE POINT OF 48" CONVENTIONAL ROOF BOLTS. THIS FALL IS 60 FT. OUTBY SURVEY STA TION #118. THIS AREA IS 486 FT. OUTBY THE SECTION LOADING POINT. JOE QUARTARO CMI ALSO AIDED IN THIS INVESTIGATION.
