Employee was climbing the ladder of 605-8 haul truck, while doing so the second rung of the ladder broke free from the left support. This caused the employee to drop roughly 12" to the first rung of the ladder, employee notified the mechanic and the supervisor who replaced the ladder. Employee was given days away from work by clinic.

Employee was working in the plant whenever EE stated EE felt that EE was having chest pains, EE contacted the foreman who then contacted the plant manager. Managment then called an ambulance, and the employee was taken to the hospital. The employee stated that EE had suffered a stroke about 3 years prior and was on medication, which EE had not been following the recommended use of.

Working on motor connections, cutting tape away from splice, sitting down in front of motor for access to equipment, employee was using "box cutter" type knife for tape removal. Knife hit wire insulation causing knife to jump up. Employee received a 2 inch laceration to left forearm requiring 5 stitches. Employee was released to return to work with no restrictions that same day.

EE WAS PERFORMING A V-TURN AFTER PICKING UP FELLOW EE TO TRANSPORT HIM TO ANOTHER HAUL TRUCK. WHILE PERFORMING THE V-TURN THE RT. SIDE OF THE TRUCK HIT ANOTHER TRUCK. THE HANDRAIL & MIRROR WERE BENT FORWARD & EE BEGAN PULLING ON THE HANDRAIL. THE WELDS BROKE & THE EE FELL WITH THE HANDRAIL TO THE GROUND.

EE WAS LINING UP THE VSI CRUSHER WITH THE PIN HOLDERS. WHEN HE STARTED LOWERING THE LID OF CRUSHER HIS GLOVED (THUMB) CAUGHT BETWEEN THE LID AND PIN HOLDER PULLING HIS THUMB INTO THE AREA. THE EFFECT WAS MASHING THE END OF THUMB. THE THUMB WAS FRACTURED & THEY REMOVED THE NAIL,SET THE FRACTURE AND SUTURED/SPLINTED THE THUMB.HE RETURNED TO WORK THAT DAY.

POWDERMAN WAS LOADING A FACE IN THE MINE WHEN A PIECE OF ROCK SLID DOWN THE FACE AND CUT HIS RIGHT ARM ABOUT 2" ABOUT THE WRIST. THE CUT WAS ABOUT 1 1/2" LONG AND SEVERED A TENDON. THE ROOM HA D BEEN SEALED WITH SCRATCH MARKS ON THE FACE ON ABOUT 6" CENTER. LOOSE MATERIAL HAD BEEN REMOVED PRIOR TO THE POWDERMAN STARTING WORK.
