Jarisa Cave Spur Mine Coal

Conditions were a piece of rock fell out of rib striking the injured party on lower half of leg, breaking his right tibia.

A non-injury roof fall was found by mine examiner. The fall was located 1cc inby spad #6654 on the 1-left bleeder panel. "J" order was issued later modified to a "K" order to allow additional roof support to be installed. Area has been dangered off. This fall will not be cleaned up. This is a non-producing mine that is in the process of being shut down. Fall was 40'Lx18"Wx8'H.

EE stated he was tramming Eimco scoop and hit a bump or a rock in road. This caused him to slam his head into the canopy. He felt instant pain in neck and left shoulder. He did not go to the doctor until 1/30/12.

Repairman was helping lift and hang the C.M. cable when his foot slipped in mud, twisting his left knee, right hip, and lower back.

EE was tightening a turnbuckle on the #5 Head drive. As he was repositioning this arm, he struck a piece of metal on the oil bath of the drive resulting in a laceration which required 7 stitches.

The EE was roof bolting in the #1 entry on MMU. No. 003 Section. The EE was using a wrench to beat a insert into another piece of steel while beating the steel he missed and hit his hand resulting in severe swelling.

A Roof Fall was discovered in a non-active area of the mine on 8-17-2011. The roof fall is located in the # 4 entry between crosscuts # 3 & 4 approximately 40' feet inby survey station # 7459 of the 2 Left Panel off the 1-East submains. The roof fall is approximately 40' feet long x 18 1/2 wide x 6' high. The roof fall was reported to the MSHA Hotline on 8-17-2011.

Roof fall occurred on the 3rd left panel off the North Mains at #13 crosscut. The fall is approx 15' outby the survey station #7384 & continues inby approx 15' from the lower or outby corner of the crosscut. The fall is 28' to 30' long by 19' wide by 6' thick. MSHA was notified & the area was dangered & timbered off. Ventilation was not impaired. No injuries.

Roof fall was found during a pre-shift exam on MMU. 003-0 section. Roof fall is located in the #3 entry at crosscut #10 at survey station 6979 of 1-east submains. Roof fall is approx 18' wide x 7-10' thick and 60' in length. Roof fall is approx 30' feet inby and 30'feet outby crosscut #10. No injuries associated with this roof fall. The roof fall is located on a in-active section.

EE was tramming feeder on belt move. Had to place crib blocks under it due to busted up bottom. Feeder slid off from crib blocks causing it to hit solid. EE's left ring finger was caught between lid and frame and pinched the end of his finger.

EE & co-worker were pulling piece of conveyor belt into new belt drive. The employees had installed a pulley onto a belt hanger. A steel cable was inserted through pulley & anchored onto conveyor belt being installed. As conveyor belt was being pulled, belt hanger broke allowing slack in steel cable which resulted in EE being struck in the back of the head with the steel cable.

A roof fall was found today in the bleeder entries where 3 left panel had been cut into 2 left. This fall was called into the MSHA Hot line at 4:33 p.m. There was no entrapment, and no apparent life threats. Fall was 1 bk inby and 2 bks outby spad # 7560. Width and Height is unknown due to this being a pillar section.

The employee was helping to move the continuous miner and MRS cables during retreat mining while the continuous miner was moving back the machine accidentally hooked the MRS cable and pulled them down. When the cable was pulled down it caused the employee to fall to the ground injuring his left foot.

Employee was cutting a piece of belt, getting ready to make an belt splice. The employee had layed the belt across his right leg. When the employee cut the belt with his knife he cut his right leg in the thigh area. (Laceration 12 stitches).

A roof fall was found today that was a continuation of a roof fall that had occurred on March 29th, 2010. The fall is located at #7201 spad in #5 entry & goes to #7202 spad in #6 entry. It ties into the fall at #7147 spad. Fall is approx. 75' long x 20'w x 16' high. This is on a panel that is not being mined. Area timbered off & dangered off. No injuries resulted in this.

Unit 1 was traveling W on old trace road. He was approaching a paving crew which was paving the W bound lane. A flagger waved him around into E bound lane to go around the paving crew. As he was going around, EE's left side wheels dropped off the E bound shoulder. Due to the steep grade, this caused him to overturn onto its L side, coming to rest on the E bound shoulder.

A Rock Fall was discovered in #2 Entry #14 x-cut. Fall was approx. 20' X 25' X 7'H. There were no injuries, and ventilation was not interupted. There are several more entries available to make this weekly exam.

EE was walking down beside the roof bolter in the #4 right entry when he stepped on a piece of draw rock causing him to slip and fall. EE twisted his left leg and knee. **Employee quit voluntarily on 11/16/10.

Rock fall in 1-left panel off of 1-east sub main in #5 entry at spad No. 7287. Rock fall fell in x-cut went inby for 30' outby for 35', to the left for 50' and to the right for 30'. Fall liiks like it might be 8' to 10' high. Rock burped real small pieces. No vent interupted in this area. We don't intend to clean this rock fall up.

Employee was operating a shuttle car. While going to feeder his tire struck a hole in the roadway causing him to be bounced up. He landed crooked, injuring his lower back.

The weekly examiner was making his weekly examination when he found a roof fall in the outby area of the mine. The roof fall is located at survey station # 6829 in the #5 entry at crosscut #24 of the north mains. The roof fall is approximately 6 - 8 feet thick, 20 feet wide x 26 feet long. No injuries or equipment involved at the roof fall area.

Was going up intake when found rock fall in #6 entry 1 break inby spad 6883 about 19' wide, 20' long, 7 to 8' high.

At 11:15 a.m. EE found a rock fall in intake of North Main at BK 22. This area is a paneled started and pulled back from. Fall was from #7 intersection to #6 intersections. Approx. 110' L. X 18' W. X 6" H. Area is 2 bks inby spad 6812. Area timbered and dangered off. Ventilation still moving and no injuries.

Fall at #2 entry, 20' inby spad 6813. Fall was approx. 20'l X 18'w X 6' thick. Fall is not blocking airflow.

Employee was bolting in the #1 heading. When he went to move the next bolt he had placed his left hand on the canopy jack. Then he grabbed the wrong lever letting the canopy jack down on his little finger.

At 5:30 am pre-shift found a rock fall in #5 entry on 004 section(in active section) at spad #7147. Fall is approximately 40' L x 16' H x 20' W. Area timbered and dangered off. No injuries reported.

There was rock fall on 1 left panel about 7' thick 20' long 18' wide in NO. #6 acress from spad 6689 in #5 entry it was found on weeky exam.

Employee was drilling hole for roof bolt when a piece of draw rock fell from top striking his right hand between pinky and ring finger. Rock was approx 12" x 6" x 4".

There was a rock fall found on weekly exam in the return on 1 left panel - 1st set of room to left at #8 entry 2 breaks to the right from spad #6638 it is just in the intersection 80' square 8' thick.

Fall was found on weakly at 9:00am 2/17/10 it is in the #1 entry in the north main 2 breaks inby spad 6600 it is about 70' long 8 to 10' thick.

Employee went to pick up an wet bag of seal garbage, when he felt burning pain in stomach. Employee did not go to a dr for medicare treatment until 2/11/2010. He had surgery on 2/12/2010.

At approx 11:20 am belts went off. At 11:50 am a fall of roof was found between #7 cc and #8 cc on 5 belt. This is non 4 entry on 2 left off of north mains. Fall was between spad #5, 6871 and 6872. Fall is approx. 50' long x 18' wide x 8'-10' high. No injury occured. Area was timbered off and dangered off. Federal was notified. via 1-800 number at 12:05 pm

EE was cutting belt out of the head drive while injured EE was holding the belt when the belt knife cut through the belt and cut EE's right thumb.

Roof fall found on the weekly exam at break 65 on 3 west mains in the return #6 entry of #7 entry. Fall was inbetween spads #6857 in #7 entry and spad #6252 in #5 entry. Fall was 30' long X 8' thick. Airflow was not disrupted.

EE was operating the shuttle car traveling through the #3 CC. The operator hit a bump and the operator bounced out of his seat hitting the top of his head against the canopy. When the EE fell back into his seat the EE injured his back and lower legs. The EE did not go to the hospital until late 12/18/09. No broken bones found possible bruise and strains.

During weekly exams, examiner found a roof fall at BK #20 of North Mains in #3 entry (old roadway). Area what fell is 20' x 20' x 8' in intersection where spad # 6811 was located. Area was timbered off and dangered off. This is an old area, non-producing. This does not affect ventilation to working section.

A roof fall was found on the 002 section MMU. The roof fall is on the belt conveyor e cross cut #5 of the 2 left panel. The roof fall is approx 30'-40' long x 20' wide x 10'-12' thick. The roof fall was reported to MSHA by phone. The roof fall will be cleaned up and the roof re-supported (See map for location)

A roof fall was found in the cross cut between the #5 and #6 entry at crosscut #4 on the MMU. N9002 section on the 2-left panel. The roof fall is approximately 30'-40' long x 20' wide x 12' thick. The roof fall has been dangered off and will not be cleaned up at this time. Roof fall reported by phone to MSHA on 9/14/09.

EE was coming from the outside into the mine clerks office and stepped into a hole in front of the doorway. While stepping into the hole the EE fell inside the doorway landing on both hands and knees. EE felt pain in the left knee, rt. wrist and rt. shoulder and pain in lower back. EE went to the hospital and continued to work. EE has been placed on restricted duty.

EE was operating roof bolter while ee was drilling a test hole he put on a second piece of steel. When ee finished the hole the roof bit got hung in the top causing the connector between the 2 pieces of steel to break and let the second piece of steel strike the ee on the left hand and index finger causing a bruise to the hand.

There was a rock fall in the intake inbetween the black of spad 5922 and 5929 about 5 feet thick and about 20ft long it did not about air flow, the air was that to go over it and the was a rather entry for except relay.

We had a roof fall in the 3 West Mains in the Return in intersection 6258 spad number about 40' long & 9' thick. Fall has been timbered off and there are 2 other entries still open that can be traveled.

Employee was bending a roof bolt when a small piece of drawrock fell against t bar causing him to jerk back. Sudden motion caused severe pain in lower back and testicles.

We had a roof fall in an intersection in #6 entry 6 break inside in the intake about 35' long x 4.5' thick in the highest place. Rock fall has been timbered off & dangered off. The air has 2 other entries to go up.

Employee was cutting brattice cloth that was wrapped around head of continuous miner when utility knife slipped cutting his left index finger.

A rib roll knocked ee into a shuttle car causing injury to his ankle, left shoulder and left colarbone.

Employee was drilling a hole to install a roof bolt when his glove got caught on the drill steel, which caused his hand to twist with the drill steel before he could stop the rotation of the steel or get the glove off.

Employee was walking around shop area and stepped on a nail injuring the bottom of his left foot. On work restrictions until 4/21/08.

While roof bolting, the roof bolt bent causing employee to accidentally hit the rotation control lever. In doing so that caused the roof bolt to spin around and hit ee's left hand and wrist.

Employee was roof bolting and had his left hand under the roof drill canopy and his right hand on the controls. When he moved the boom it caused the canopy to swing and hit a piece of cap coal smashing his left index finger.

Employee was disconnecting haulage trailer from tractor. As he pulled the pin out, the tongue slipped out and fell on the top part of his left foot.

Employee bent down to clean the head out of the roof drill. He said he got up in a hurry and twisted his left knee. *Employee did not start missing work due to this injury until 4/2/08.

Had a rock fall in between #425 Entry. 2 breaks inby spad numbers 6066 in #5 Entry & 6067 #4 Entry. We have set crib around fall. Fall is abou 50' long, 10-12 ft high.

The results of a medical examination indicates the the miner may have pneumoconiosis.

While operating shuttle car, EE ran over a rock or hole in the roadway, causing him to hit his head against canopy of shuttle car.

Employee was building brattice when it fell on top of him.

Employee was putting bottom secondary scrapper on #2 head drive when scrapper fell against tailpiece cutting right 3rd finger. Laceration required 2 stitches.

Employee was drilling through a roof strap when a piece of carbide flew off and struck him in his right eye.

We had a rock fall in an intersection in the intake one break outby 5305 spad. The rock fall is approx. 30' long, 20' wide and about 18' high. Fall was found on weekly examination. It is in 6 entry. There is another travelable intake. It is not affecting the ventilation at the mine

Employee was bending a bolt. He said that he felt severe pain in his lower back & legs.

Employee was a passenger on mantrip when the mantrip ran over bump in roadway.

Employee was putting up a roof strap. He said that he lost his balance while trying to push the strap into place and the strap cut his left wrist. He recv'd 10 stitches in wrist.

Employee was rock dusting when he tripped over a piece of cinder block causing him to fall backwards & hit his right shoulder.

Employee was repairing the headjack of a miner when he lifted up on a jack and strained his back.

While building a brattice, employee was chopping on a cinder block when a piece of block flew up and went into his right eye. Employee had his eye flushed out and was given prescription for antibiotic.

Employee was bending roof bolt when he felt sharp pain in his lower back.

Had rock fall 2 breaks inby spad 5541 in right crosscut 90' inby rib to other intersection. There are other entries to travel out. Fall was approx. 20' wide, 60' long. Reported to MSHA.

Employee bent down to pick up miner cable when he felt a sharp pain in his lower back.

Employee was bending a strap that was hanging down. The strap sprang back and struck his left hand causing a cut between his middle and third finger. Received two stitches and restrictions for one week.

While roof bolting, drill steel popped back out & punctured employee's left hand between his thumb & index finger. He received 5 stitches.

Employee injured his back while hanging miner cable. He bent over to pick up more cable & felt a sharp pain in his back.

There was dirt, rock, coal & mud that had fallen in at the back end of the fan house. There was insufficient air. Had to shut down and remove dirt, rock and mud with excavator and rebuild fan house.

EE was rolling a spool of cable off low-trac when it twisted and caughthis right arm & hand against the forks of the low-trac.

Employee placed his left hand on the part of the sectional edge of the jack while roof bolting and let the canopy down on his hand resulting in an open wound that required stitches.

Employee was tramming miner when he slipped & fell catching himself with his right hand causing a strain to his right shoulder. Employee did not miss any work due to this injury until 2/8/06 - Had an MRI on 2/8/06.

On the weekly examination of the mine the examiner found an unintentional roof fall in the return side of the 001 section (west mains) which has 2 other ways around it. Therefore it will not be cleaned up as it was timbered and dangered off. the fall was about 15' inby spad #3988 and measured about 50' long, 18' wide and 8 to 10 ft high. This old section is to be sealed.

Employee was roof bolting when rib rolled off on his right ankle and back.

On weekly examination-there was a roof fall found int he #7 intake entery at break #19 10' inby spad #462 in a timbered off area, the fall was approx 7-8' high and about 40' long, it does not have ventilation on intake travelway blocked it may a may not be celaned up, fall was found at about 730 am

ROOF FALL IN AN INTERSECTION IN AN AREA WITH 3 INTAKE ENTRIES. AREA WAS NOT USED, DOESN'T BLOCK TRAVEL OR VENTILATION. WE HAVE 2 OTHER INTAKE ENTRIES TO USE. THIS FALL HAS BEEN TIMBERED & DANGERED OFF & WILL NOT BE CLEANED UP. THIS FALL OCCURRED IN THE 2ND BREAK OUTBY #4 INTAKE SEAL AT SPAD 5062 & WAS NOT ON THE ACTIVE SECTION. FALL WAS APPROX 20'X20'X UP TO 10'H

We had a roof fall in an area of the mine that had been cribbed and dangered off for about 3 weeks, 20 ft in by spad #5125 or 7 brk out by face in the #3 neutral entry. The fall was approx 18' wide by 70' long and 10-14' high. The fall did not block intake or return airways, belt entry or neutral travelway. Fall will not be cleaned up.

WHILE MAKING A BELT SPLICE, THE EMPLOYEE WAS STRIKING THE BELT PIN WITH A HAMMER & A PIECE OF METAL CHIPPED OFF THE HAMMER, PUNCTURING EE IN THE LEG. EE RECEIVED A TETANUS SHOT & 2 STITCHES.

EE had gotten on his mine buggy to go outside. Without knowing it, his block hammer was hanging halfway out of his belt strap & had twisted around. He sat down on buggy seat & leaned back ont he blade of his hammer. The hammer cut his lower back a little to the right of his spin requiring 3 stitches.

WE HAD A ROCK FALL IN AN INTERSECTION IN THE RETURN AIRCOURSE ONE BREAK OUTBY SPAD #817. THE FALL WAS APPROXIMATELY 40' LONG, 20' WIDE AND AT THE HIGHEST POINT ABOUT 12' HIGH. THE FALL MAY BE CLEANED UP AT A LATER DATE.

ROOF FALL IN AN INTERSECTION IN THE NEUTRAL ENTRY ADJACENT TO #5 BELT AT SPAD #3992. HAD BEEN DANGERED & CRIBBED OFF FOR ABOUT 2 MONTHS, THIS FALL WAS PROBABLY CAUSED BY PRESSURE FROM SOME BLOCKS OF COAL LEFT IN THE HIGHSPLINT SEAM ABOVE THIS PARTICULAR AREA. APPROX. 20'WIDE X 10-15'HIGH AND ABOUT 25'LONG.

Ee was struck in the right side of head by the mrs cable which knocked him down into a mud hole.

ROCKFALL AT SPAD # 4290 ON 2 LEFT MAINS IN # 2 NEUTRAL ENTRY 8 BREAKS BELOW # 3 DRIVE. FALL STARTED IN NEUTRAL & RODE OVER TO BELTLINE & CAUGHT ABOUT 1/2 OF THE BELT. FALL WAS APPX 18-20'WIDE, ABOUT 130' LONG & APPX 6-12'HIGH. BOLTED WITH 4' RESIN BOLTS & APPEARS TO BE AN UNDETECTED FAULT AREA WITH SOME PRESSURE FROM BLOCKS OF COAL LEFT IN THE HIGHSPLINT SEAM ABOVE.

Ee was struck in legs by shuttle car cable causing him to fall backwards on his back.

HAD SMALL ROOF FALL 1 BREAK ABOVE SPAD #4076 IN THE INTAKE AIR COURSE #4 ENTRY IN A TRANSITION AREA OF SLATE TURNING TO SANDSTONE (FAULTY AREA). THE FALL WAS APPROX 5'HIGH, 18'WIDE AND 20'LONG. BOLTED WITH 4'5/8 RESIN BOLTS. IT'S NOT GOING TO BE CLEANED UP. WE HAVE A SAFER TRAVEL WAY AROUND IT AT TIME OF INVESTIGATION AND IT IS NOT RESTRICTING THE AREA.

WE HAD A ROCKFALL IN ONE OF THE 2 NEUTRALS ABOUT TWO THIRDS OF THE WAY UP BESIDES #8 BELT AT SPAD #4566 MEASURING APPROX. 20' WIDE, 40' LONG AND FROM ABOUT 4' TO 12' HIGH. IT BOLTED GOOD WHEN WE MINED THROUGH IT BUT APPEARS TO BE AN UNDETECTED FAULT AREA. REPORTED IT TO MINE INSPECTOR.

Rock fall in one of the neutrals about 1/3 way up #8 belt area(not in belt entry) at spad #4423. Timbered it off & made roadway around rock fall into the other neutral rock fall. Fell sometime between 2pm Sat 5-15-04 & 8pm Sun 5-16-04. Rock fall was approx 20' wide, 40" long & from 4' to 10' high, it bolted good at the time it was mined w/4' resin bolts.

EMPLOYEE WAS POSITIONING A PUMP IN THE WATER AND LOST HIS FOOTING DUE TO SLIPPERY CONDITIONS. HE REACHED TO GRAB FOR THE RIB TO KEEP FROM FALLING AND WHILE DOING SO, HE TWISTED HIS LOWER BACK.

TALL CRIBS HAD BEEN SET IN THIS AREA, BUT DUE TO THIS AREA (1 BREAK SPAD #3962) HAVING WET TOP - THE TOP MUST HAVE DETERATED BECAUSE OF BEING WET. THE FALL WAS APPROX 20' X 30' X 15' HIGH.

EMPLOYEE WAS ROOF BOLTING WHEN A PIECE OF DRAW ROCK FELL LANDING ON HIS LEFT WRIST, BRUISING IT.

HAD ROOF FALL IN INTERSECTION 18' X 40'AT SPAD #4460 IN INTAKE OUTBY SECTION DUE TO UNDETECTED FAULTY AREA. DIDN'T BLOCK INTAKE. HAD 2 OTHER INTAKE ENTRIES.

EE SLIPPED & FELL ON SOME DRAW ROCK CUTTING HIS RIGHT HAND.

EE AND OTHER EE WERE PUTTING BUSHINGS INT HE MINER. EE WAS STANDING NEAR OTHER EE WHILE HE WAS HAMMERING THE BUSHINGS INTO THE MINER A PIECE OF METAL FROM THE HAMMER BROKE AND FLEW OFF STRIKIN G EE RIGHT HAND.

HAD ROCK FALL IN INTERSECTION 1 BREAK ABOVE SPAD #4568 IN THE NEUTRAL ENTRY PROBABLY CAUSED BY WHERE WE WENT THRU A HOLLOW. THE AREA WAS BOLTED WITH 4' RESIN BOLTS AND 10' ROPE BOLTS, CRIBS AN D BEAMS WILL SIT AROUND THE AREA.

FOUND ROOF FALL ON #9 BELT RETURN SPAD #4494 AND SPAD #4488 BREAK BELOW #11 HEADLINE FALL WA SNOT IN INTERACTION IT WAS APPROX. 4' HIGH AND BY THE TIME IT F. IT WAS APPROX. 25' LONG LOOK HIT A N UNDETECTED KETTLE BOTTOME IS WHAT TUSSED IT LOOSE.

HAD ROOF FALL 1 BREAK INBY SPAD #4560 IN INTAKE IN AN INTERSECTION. DOES NOT AFFECT VENTILATION. FALL WAS APPROX 21' WIDE, 40' LONG, 3 TO 10' HIGH. REPORTED IT TO MSHA, MINE INSPECTOR WAS AT T HE MINE.

EE BENT OVER TO PICK UP CABLE AND BLACKED OUT.

EE FELT SHARP PAIN IN LOWER LEFT SIDE OF BACK & LEFT LEG WHILE SHOVELING AROUND BELTHEAD.

EE HAD JUST ARRIVED ON SECTION HE WALKED UP BETWEEN THE MINER AND THE RIB WHEN THE RIB ROLLED HITTING HIM ON HIS LEFT LEG.

HAD ROOF FALL ON #5 BELTLINE 1 BREAK OUTBY SPAD #4097 FIN UNDETECTED FAULTY AREA APPROX 7 TO 8' HIGH. HAS TO BE CLEANED UP.

A ROOF FALL OCCURRED IN THE BREAK BETWEEN #1 & #2 ENTRIES IN 2 LEFT MAINS ACROSS FROM #5 BELT, APPROX. 8'HIGH X 50'LONG. CRIBS AND 10'ROPE BOLTS WERE INSTALLED IN ADJACENT ROADWAY TO PROTECT IT IN THIS FAULTY AREA. 6'TORQUE TENSION BOLTS WERE ORIGINALLY INSTALLED. FALL WAS IN BREAK TO THE LEFT OF SPAD #3973.

HAD ROCK FALL BREAK ABOVE SPAD #1064 IN INTERSECTION IN THE #2 MAIN RETURN SIDE APPROX. 18'WIDE 50'LONG AND 10'HIGH, BOLTED WITH 6' TORQUE TENSION BOLTS. CAUSE MAY HAVE BEEN AN UNDETECTED FAUL TY AREA OR COULD HAVE BEEN CAUSED FROM PRESSURE FROM THE WORKED OUT MINE ABOVE US. WE ARE CURRENTLY DOING SOME CRIBBING IN SURROUNDING AREAS OF THE FALL AND HAVE SOME JACKS AND BEAM ORDERED.

EMPLOYEE WAS BENT OVER PULLING ON MINING CABLE WHEN HE FELT A SHARP PAIN IN HIS LOWER BACK AND RIGHT LEG.

EE WAS HANGING MINER CABLE WHEN ALL OF A SUDDEN HE FELT PAIN IN HIS STOMACH ALL THE WAY DOWN TO HIS GROIN AREA.

ROCK FALL IN AN ABANDON AREA IN A CROSSCUT ONT HE OLD 001 SICTING DOES NOT HOUR ANY ENTERIES BLOCKED APPROX. 6' HIHG 18' WIDE AND 50' LONG THRU TOP WAS ELAMINATED SANDSTONE AND WAS WITH 6' RES IN BOLTS ONE SIDE OF THE CROSSCUT WAS CATCHED OFF THE FALL WAS IN CROSSCUT BETWEEN #6 AND #7 ENTRY AT SPAD #4056 IN #7 ENTRY APPARENTLY THIS WAS A FAULTY AREA.

EE WAS LIFTING MINER CABLE AND STRAINED HIS BACK.

EMPLOYEE WAS JACKING UP SUPPLY TRAILER TO TAKE IT OFF FROM DIESEL. THE TRAILER SLIPPED OFF JACK AND FELL ON HIS FOOT.

EMPLOYEE WAS HELPING MOVE MINER CABLE WHEN RIB ROLLED OFF STRIKING HIM IN RIGHT LEG AREA.

EE WAS HANGING CABLE ANCHOR FOR SHUTTLE CAR AND TWISTED HIS RT KNEE.

EE INJURED LOWER BACK WHILE LIFTING A BAG OF ROCK DUST.

EMPLOYEE WAS INSTALLING A ROOF BOLT AND WAS ATTEMPTING TO PUSH THE BOLT THROUGH TWO ROOF MATS WHEN HE FELT A SHARP PAIN IN HIS RIGHT ELBOW.

ROOF FAILURE IN BETWEEN INTERSECTION 6 ABD 7 ENTRY OF 3 LEFT OFF 1 LEFT MAINS BETWEEN SS 357 AND 367. NO EQUIPMENT OR PERSONNEL INVOLVED, VENTILATION WAS NOT AFFECTED.

EE WAS STARTING TO DRILL A HOLE TO INSTALL A ROOF BOLT. THERE WAS SOME COAL ON THE MINE ROOF & WHEN HE STARTED THE ROTATION, A PIECE OF IT BECAME DISLODGED STRIKING HIM IN THE RIGHT EYE. EE ST ATED HE HAD SAFETY GLASSES ON.

EMPLOYEE WAS ATTEMPTING TO PUSH A PIECE OF DRILL STEEL UP TO DRILL A HOLE. HE HAD HIS HAND ON THE STEEL, WHICH WAS NOT ALL THE WAY IN THE DRILL CHUCK, WHEN HE APPLIED TOO MUCH PRESSURE CAUSING THE DRILL STEEL TO SLIP AND VIBRATE INTENSELY BREAKING HIS THUMB.

EMPLOYEE WAS ATTEMPTING TO PULL A ROOF BOLT FROM THE MINE ROOF THAT WAS NOT PROPERLY INSTALLED WHEN THE BOLT BECAME DISLODGED STRIKING EMPLOYEE ON LEFT FOREHAND.

EMPLOYEE WAS OPERATING MINER WHEN A PIECE OF MINER BIT BROKE PUNCTURING EMPLOYEE ON BACK OF RIGHT HAND. HAD A PIECE OF METAL REMOVED FROM HAND & RECEIVED 2 STITCHES.

EE WAS TRYING TO MOVE (LIFTING) A PIECE OF ROCK OUT OF THE ROADWAY AND SAID HE FELT SMALL BURN- ING SENSATION IN LOWER ABDOMEN.

EE WAS WALKING OFF SECTION BEHIND ANOTHER EE WHOWAS CARRYING A BELT CUTTING TOOL OVER HIS RIGHT SHOULDER. THE TOOL SNAGGED A PIECE OF PHONE WIREHANGING FROM A PIECE OF CABLE. THE WIRE FLEW BAC K AND HIT HIS RIGHT EYE.

14-15 CM JOY MINER WAS CAUGHT IN AREA WHERE PILLARS WERE BEING EXTRACTED. MINER WAS IN #7 ENTRY TAKING THE #6 LIFT ACCORDING TO THE APPROVED PLAN. THIS IS 20' IN BY SS 2095 ON THE ACTIVE MMU 001 LEFT OFF LEFT MAINS. ROCK WAS SOLID SAND STONE 4' THICK BY 40' LONG BY 15' WIDE MINER WAS PULLED OUT WITH RETRIEVER WITHOUT ANY DAMAGE.

ROOF FAILED ABOVE ANCHORAGE RESULTING IN ROOF FALL ONE BREAK OUTBY SURVEY SPAD 775 ON 1 LEFT MAINS AT INTERSECTION, #5 ENTRY EXTENDING ACROSS #6 INTERSECTION TO THE EDGE OF #7 NO EQUIPMENT INV OLVED. IT DID TAKE A BRATTICE BUT DID NOT SEEM TO AFFECT VENTILATION TO 001 MMU. BRATTICES ARE BEING MOVED.

ROOF FALL IN THE INTERSECTION #2 ENTRY 1 LEFT PANEL OFF 1 LEFT MAINS LAST OPEN CROSSCUT OUTBY PILLAR LINE. FALL ~5'HIGH, 20'WIDE, 40'LONG. DANGERED OFF & ROOF CHECKED BEFORE MINING CONTINUED.

EE WAS KNEELING DOWN TO PUT A BELT PIN IN AND HIS RIGHT KNEE MADE A POPPING NOISE.

LOST POWER ON 001-MMU. UPON INVESTIGATION OF POWER LOSS A FALL WAS DISCOVERED AT SURVEY SPAD 758IN #5 ENTRY ON 1 LEFT MAINS OF 001-MMU. HIGH VOLTAGE CABLE WAS RE-ROUTED AND POWER RESTARTED. FALL WAS TIMBERED OFF AND SUPPLEMENTAL SUPPORT INSTALLED IN OTHER INTERSECTIONS. DIDN'T AFFECT VENTILATION.

EMPLOYEE WAS INSERTING WIRE THRU ROOF BOLT PLATES TO HANG CABLE FROM WHEN A PIECE OF WIRE STRUCK HIM IN THE EYE.

EMPLOYEE WAS INSTALLING A ROOF BOLT WHEN HE CAUGHT HIS GLOVE IN BETWEEN THE BOLT WRENCH AND ROOF BOLT. HE SAID HE JERKED AWAY AND FELT SHARP PAIN IN HIS SHOULDER.

ROOF FALL WAS DISCOVERED ON ROADWAY ON 1 RIGHT OF THE 001-MMU MAIN LINE AROUND 4:30 AM. FALL WAS TIMBERED OFF AND THE ROADWAY WAS MOVED TO ANOTHER ENTRY. ADDITIONAL BEAMS WERE INSTALLED IN BY AND OUT BY AREA AFFECTED. THIS DID NOT AFFECT VENTILATION.

FALL WAS DISCOVERED DURING PRESHIFT EXAM IN NO. 5 ENTRY 1 X-CUT INBY SS 738. FELL IN INTERSECTION & EXTENDED TOWARD FACE. MEASURED 6' THICK, 20'WIDE, 30' LONG.

MOVING A PIECE OF METAL. PICKED IT UP TO THROW IT OUT OF THE WAY CAUGHT FINGER BETWEEN RIB AND PIECE OF METAL.

MECHANICAL FAILURE OF TRAM SPOOL ON VALVE BANK. EE WAS TRAMMING FEEDER. HE LET OFF TRAM LEVERS, TRAM SPOOL WOULD NOT DISENGAGE. EE WAS TASK TRAINED OPN FEEDER BEFORE HE BEGAN MOVING FEEDER AND IS EXPERIENCED AT THIS JOB.

ACCORDING TO MEN I INTERVIEWED, EE SAID HE BENT OVER TO PICK UP A PULL ROPE AND HIT HIS MOUTH AGAINST THE STANDOFF OF THE MINER.

EE WAS USING A HAMMER & MINER BIT TRYING TO KNOCK INSERT OUT OF A BIT LUG.

A ROOF FALL WAS DISCOVERED AT THE TOP END OF THE 2 LEFT OFF EAST MAINS PANEL. THE FALL WAS IN THE NO.3 ENTRY AND EXTENDED FROM THE GOB TO THE LAST OPEN CROSSCUT OF THE PANEL. FALL WAS 20 FT WI DE AND 10 FT HIGH.

ROOF FALL WAS #5 ENTRY EAST MAINS 001 SECTION; INTAKE AIR COURSE BETWEEN 2ND AND 3RD X-CUTS OUT BY INACTIVE FACE OF EAST MAINS BETWEEN SURVEY SPADS 230-231; FALL IS 45' L, 19' W, 4-5' THICK. NO EQUIPMENT INVOLVED; DIDN'T AFFECT VENTILATION.
