Employee was out on the boom of the loader cleaning the windshield. He fell when he was crossing back over the handrail onto the catwalk. He fell from the catwalk to the ground.
Miner head was retracted due to damaged coupling, employee has slipped while near head, and sharp cutter bit pierced his right leg.
While checking fuel levels in grease truck, he slipped and caught himself with his arms on handrail, pulling back muscle in process.
While digging solid rock with bucket, the back of loader was raised up, during that time bucket slipped, dropping cab back down suddenly, jarring operator.
