Forkllift operator was unloading pallets from the delivery trailer. The driver walked up to stow his tarp straps in the bin, Some pallets shifted and fell, striking EE on the head and foot.
EE and co-worker were removing some old discharge pipes from the screening deck. EE made his 1st cut and handed the torch to co-worker to walk around the other side. When he walked around the pipe, he stepped in a hole where a 6 inch pipe had been cut out the evening before. When he stepped into the hole he twisted his ankle and fell against a hummer shaker.
EE was working as a loader operator when they called him to came help because the bags were hanging up on the rollers. While he was checking the palletizer, operator automatically hit the door closure switch trapping his foot under the bar. This resulted in a hairline fracture to one of his toes.
EE WAS WORKING ON THE COOLER. AS HE EXITED THE COOLER HE HAD HIS FOOT BRACED ON A PIPE. HIS FOOT SLIPPED AND HE FELL.
EE WAS USING A 2 LB. SLEDGE HAMMER TO GET THE TAIL PULLEY OUT OF THE VARIABLE SPEED BELT. THE END CAME OFF THE HAMMER AND THE MOMENTUM CAUSED HIS FINGER TO HIT THE END OF THE TAIL PULLEY. THE MIDDLE FINGER OF HIS RIGHT HAND WAS MASHED & LACERACTED. IT REQUIRED 3 STITCHES.
A CAADIAN GOSLING HAD A PIECE OF PLASTIC MESH TANGLED AROUND HIS BEAK. INJURED CAUGHT THE GOSLING WITH 2 CO-WORKERS AND REMOVED THE MEST. IN THE PROCESS INJURED TWISTED HIS ANKLE.
EE WAS PULLING ON HIS POCKET KNIFE TRYING TO CUTTHE STRINGS ON THE DECORATION. THE KNIFE SLIPPED& STUCK HIS LEFT FOREARM REQUIRED STITCHES RETURNED TO FULL DUTY SAME AFTERNOON
EE WAS ADJUSTING THE BAG DISCHARGER ON THE 787 FORCE FLOW VALVE PACKER TO ACCOMODATE 50 LBS BAGS. AS HE REACHED DOWN HE SLIPPED HIS RIGHT THUMB WENT UNDER THE BAG DISCHARGER AND HIS RIGHT SHOU LDER HIT THE START BUTTON.T HE BAG DISCHARGER CLOSE ON HIS THUMB. THE RELEASE BUTTON WAS PUSHED 3-5 SECONDS LATER. HIS THUMB WAS SEVERLY LACERATED.
