Our technician was working onsite at the C&J Gravel shop in Durango, CO, Mine ID#0504034. While removing jaw wedge with an impact wrench the pusher bolts and wedge became free pushing the impact wrench into EE's chest, causing EE to fall on the grizzley fingers. EE was treated and released at Mercy Medical missing one day of work on Friday 1/15/16

He was climbing into the trackhoe and slipped on the step and injured his left knee

While he was cleaning out material buildup on a chute, which is suspended by chains and one of the chains broke causing him to fall against the conveyor railing frame and he bruised his left knee.

After servicing the loader, EE started walking to personal truck to go home and stepped wrong then slipped on snow. (It had been snowing all day) resulting from the slip & twisting of his ankle he fell to ground onto his right side. He said when he slipped he felt something pop. EE said he was going to wait until morning to have Dr Check ankle.
Carried a heavy roll of vapor barrier from pickup to office, was squatting to set roll down on the floor and dropped roll on floor. Next day was experiencing some pain in groin area. Two days later was not able to walk very easily.

EE WAS LEANING AGAINST THE STACKER AND HE HAD HIS FINGERS IN THE ADJUSTMENT HOLE WHILE THEY WERE LINED UP. ANOTHER OPERATOR WAS ADJUSTING THE HEIGHT BY LOWERING THE STACKER, AND EE'S FINGERS W ERE SEVERED BECAUSE THEY WERE IN THE SLIDE AREA.
WHILE ADJUSTING THE CONE, A LOADER WAS PULLING UP TO HOOK A CABLE USED TO ADJUST HTE CONE ON THE BUCKE TOF HT ELOADER. THERE WAS A SMALL BERM THAT MADE THE LOADER ROLL FORWARD & FORCED THE BU CKET UP AGAINST THHE CONE. PINNING THE EMPLOYEE'S LEG BETWEEN THE LOADER BUCKET AND THE CONE. EMPLOYEE'S LEG WAS BROKEN.
