Slabs of concrete were caught in the Jaw. The worker was using a pry bar in an attempt to dislodge the concrete slabs. The worker's left hand was caught between the pry bar and the guardrail around the jaw.

EE WAS ATTEMPTING TO ADJUST FEED GATE OPENING BY SHORTENING THE 3/8 INCH CHAIN THAT HOLDS THE GATE OPEN ON THE FEED CONVEYOR UNIT 2200 THAT FEEDS THE CRUSHER. HE OPENED THE GATE & SECURED IT W ITH A BLOCK. THE BLOCK SLIPPED WHILE HE WAS ADJUSTING THE GATE & PULLED HIS RIGHT INDEX FINGER & THUMB THROUGH THE CHAIN OPENING CAUSING LACERATIONS.

EE CLIMBED ON FRONT END LOADER TO REPLACE FUEL LINE. WHEN HE SAT DOWN ON THE TIRE AND TURNED TOTHE RIGHT, REACHING INTO ENGINE COMPARTMENT HE FELT SEVERE PAIN IN BACK.
