Slabs of concrete were caught in the Jaw. The worker was using a pry bar in an attempt to dislodge the concrete slabs. The worker's left hand was caught between the pry bar and the guardrail around the jaw.

EE WAS ATTEMPTING TO ADJUST FEED GATE OPENING BY SHORTENING THE 3/8 INCH CHAIN THAT HOLDS THE GATE OPEN ON THE FEED CONVEYOR UNIT 2200 THAT FEEDS THE CRUSHER. HE OPENED THE GATE & SECURED IT W ITH A BLOCK. THE BLOCK SLIPPED WHILE HE WAS ADJUSTING THE GATE & PULLED HIS RIGHT INDEX FINGER & THUMB THROUGH THE CHAIN OPENING CAUSING LACERATIONS.

HE WAS SEEN BY A PHYSICIAN-DIAGNOSES CORNEA ABRASION-WAS RELEASED TO RETURN TO WORK BUT EYE WAS PAINFUL AND HE RESTED IT ONDAY OF INCIDENT AND NEXT DAY. RETURNED WITH NO PROBLEM SINCE.

WHILE WORKING ON CONVEYOR BELT UNDER CRUSHER, THE EE WAS PUSHING THE BELT DOWN WITH A PRY BAR & IT SLIPPED OFF THE BELT WITH THE HANDLE HITTING THE PERSON BETWEEN THE LEFT EYE & EAR. THIS CAUS ED A SMALL PUNCTURE WOUND.

EE WAS USING RATCHET WRENCH WHEN SOCKET SLIPPED OFF BOLT HEAD AND CAUSED HIS RT HAND TO BE PINCHED BETWEEN THE WRENCH AND FRAME OF CONVEYOR HE WAS WORKING ON. THIS CAUSED A FRACTURE OF THE 5TH FINGER ON RT HAND IN THE PROXIMAL I-P JOINT.
