Crew was excavating a pipe out of a trench w/a loader. Employee A was on the loader being directed by Employee B on the ground. Employee B directed Employee A to pull forward when they did the pipe wedged between the teeth on the bucket. This caused the pipe to rotate & struck Employee B in the shoulder & head. No broken bones concussions or contusions. Just sore.

Caught hand between 150 pound hammer and cone nut wrench.

Years of driving cause of numbness w hand carpal tunnel syndrome.

Dismounting excavator slipped off step had hold of hand hold pulled shoulder

EE was helping do ground fault check on plant, turned to step off walkway, twisted knee.

Removing logs from new stockpile site slipped on loose dirt and fell.

Employee was checking for air leak on left rear when plunger moved to set position lacerating right index finger.

BIG ROCKS IN ROCK PILE EE WAS PICKING UP THE BIGROCKS OUT OF PILE: SLIPPED ON ROCK & WENT DOWN ON PILE HIT HIS SHOULDER ON PILE OF ROCKS

CAT WALK WAS WET, SLIPPED AND HIT HIS LEG, CUTTING HIS RIGHT SHIN.
