EE was working in the crusher tower repairing the hammer mill. He handed another employee some tools and turned around and struck his upper front tooth on the lid of the mill. His front tooth was chipped toward the midline.

EE reported he slipped and fell on ice, under Bin House #1, injuring his right shoulder.

While driving on the haul road he hit a chug hole in the road causing him to strain his neck and shoulder.

EE REPORTED AT START OF SHIFT THAT A DR TOLD HIM HE HAS CARPAL TUNNEL SYNDROME AND THE EE ALLEGES IT IS DUE TO DRIVING STOCK PILE TRUCK TO BE DETERMINED IF ILLNESS IS RELATED TO WORK.
