EE was working in the crusher tower repairing the hammer mill. He handed another employee some tools and turned around and struck his upper front tooth on the lid of the mill. His front tooth was chipped toward the midline.
While changing screens in bin house, stepped back to put screen in place and stepped in hole, left leg hit against grate and cut leg.
EE reported he slipped and fell on ice, under Bin House #1, injuring his right shoulder.
EE reported while using an impact gun to tighten nuts he felt a pain in his left arm below the elbow
EE stepped on rock on the catwalk platform, twisting his right ankle.
Employee alleges driving over rough roads at the quarry jarred his back (doctor stated this is from a prior injury which the employee has already received a settlement).
EMPLOYEE WAS MASHING TRASH INTO A TRASH BARREL WITH A PIECE OF SQUARE TUBING & HE CAUGHT HIS FINGER BETWEEN THE BARREL & THE TUBING REQUIRING SUTURES.
EMPLOYEE WAS TRYING TO GET BIN #2 UNFROZEN & WAS CHECKING TO SEE IF THE BIN WAS CLEAR WHEN A ROCK FELL & HIT HIM IN THE BACK OF THE HEAD.
EMPLOYEE WAS WELDING CRUSHER HAMMERS WHEN HOT SLAG HIT HIS GLOVE AND BURNT HIS HAND.
While driving on the haul road he hit a chug hole in the road causing him to strain his neck and shoulder.
EE WAS CLOSING THE LID ON THE HAMMER MILL WHEN HE WAS PINNED BETWEEN THE LID & THE U-BELT GUARD.
EMPLOYEE DID NOT HAVE THE TRUCK BED COVERED, THEREFORE HIT THE BIN HOUSE.
WHILE DRIVING A STOCKPILE TRUCK EE ALLEGEDLY HIT A POTHOLE IN THE QUARRY ROADWAY CAUSING A NECK STRAIN. THIS CLAIM WAS ULTIMATELY DENIED AS IT WAS DETERMINED TO BE A PRE-EXISTING CONDITION AND NOT WORK RELATED.
EE REPORTED AT START OF SHIFT THAT A DR TOLD HIM HE HAS CARPAL TUNNEL SYNDROME AND THE EE ALLEGES IT IS DUE TO DRIVING STOCK PILE TRUCK TO BE DETERMINED IF ILLNESS IS RELATED TO WORK.
EE ALLEGES THAT HE WAS LIFTING A 5 GALLON CAN OFOIL ONTO THE SHAKER ABOUT CHEST HIGH AND FELT A "POP" IN HIS RIGHT SHOULDER.
CHANGING SCREENING IN BIN HOUSE #1 DIESTER WHEN FINGER WAS SMASHED BETWEEN SCREEN AND HANDRAIL.
