Was measuring a hole to see if it was ready to be stemmed. The weight on the end of the tape got hung up. Employee pulled on the tape causing it fly out of the hole at a high velocity and strike the employee in the groin area.

EE was trying to get a cable when EE stepped off the concrete onto what EE thought was dirt and mud. EE stepped on a board with a nail in it that was under the dirt. The nail went through EE's boot and into EE's foot.

Operator noticed a hydraulic leak on track hoe. Got out of equipment to see about leak and clean up the oil when ee slipped on track hoe and fell hitting right side of body on track hoe and landing on pile of rocks.

Moving screens to ground and the balance of the screen was off and it tilted the wrong way and the corner hit just above thee left ankle, causing bruising and swelling.

Miner was working on the wheel inside the track of a track hoe. He was trying to take the metal wheel off, and it fell on his left foot.

Employee was installing big electrical boxes in a box when the straps securing the box slipped. He tried to jump from trailer to keep from being pinned by the box. When he jumped he landed on his side across a starter box which was on the ground below box trailer. When he landed he injured his rib cage and was taken to E.R.
Employee was descending the ladder, got to the third step and missed a step. He then slid down the rest of the ladder and hit the ground and rolled onto his belly, during this time he twisted/strained his back. He remained on his belly until ambulance personnel arrived and placed him on a backboard. He had been up and down the ladder several times that morning.
